OBJECTIVE: The objective of the present study was to establish whether copper (Cu)-deficient rats fed a diet containing fructose as their sole carbohydrate source require more biotin than the recommended 2 mg/kg diet when egg-white serves as the dietary protein.
METHODS: Eighty weanling male Sprague-Dawley rats were randomly divided into 8 groups according to type of dietary carbohydrate (starch or fructose), level of Cu (0.6 micrograms Cu/g diet or 6.0 micrograms Cu/g diet) and level of biotin (2 mg/kg diet or 10 mg/kg diet).

The authors report 2 familial cases of biotin deficiency. The first neurological signs appeared at the age of 2 years in a boy. The diagnosis was established in his sister in the neonatal period. A review of 41 published cases summarizes the neurologic signs (seizures, ataxia, hypotonia and later, developmental delay and deafness) and the cutaneous signs (rash, alopecia). An early treatment with biotin cures or prevents the clinical signs of the disease in most cases.

The allegedly beneficial effect of biotin in the treatment of infantile 'flexural' seborrhoeic dermatitis was evaluated in a double-blind controlled study. No beneficial effect could be confirmed, and it is therefore unlikely that this disease is caused by biotin deficiency. Nor could this scaly dermatitis be ascribed to an essential fatty acid deficiency. Serum and adipose tissue fatty acids resembled each other and reflected the fatty acid composition of the diet.
