A 7-year-old, female Huacaya alpaca presented with a 3-week history of left-sided head tilt and mild ataxia. Despite aggressive supportive therapy with empirical antibiotic, anti-inflammatory, thiamine, and anthelminthic therapy, neurologic signs progressed to a right-sided head tilt, positional nystagmus, and episodes of severe ataxia and obtundation. On necropsy, discrete expansile to poorly delineated infiltrative masses with tan, creamy to grumous exudate were observed in the right petrous temporal bone, right middle and inner ear, right cerebellum and medulla as well as in lung, kidney, l

Intraoperative consultation via frozen section is an important part of modern day surgical pathology. Recognizing fungi in tissues on frozen and permanent sections is not always a simple task, and correctly identifying the agent can be a significant challenge, even for experienced microscopists. We present a case of a 17-year-old boy with chronic osteomyelitis involving the right proximal ulna. During an irrigation and debridement operation, a frozen section was sent to surgical pathology for evaluation. A limited patient history coupled with sparse organisms present in the frozen section led

The precise ecological niche of Blastomyces dermatitidis is unknown. The related dimorphic fungus, Paracoccidioides brasiliensis, has been isolated from South American ground-dwelling insectivorous armadillos. We attempted to isolate Blastomyces from shrews, North American ground-dwelling insectivores that have been shown to harbor Histoplasma capsulatum in endemic areas. Forty-seven masked shrews (Sorex cinereus) and 13 northern short-tailed shrews (Blarina brevicauda) were collected in endemic areas of northern Wisconsin and Michigan using pitfall traps. Specimens were collected between 1998

The isolation of Blastomyces dermatitidis in India is reported from the bronchial aspirate of a female asthmatic patient who had never travelled abroad. The patient was a resident of Rodhain, a small town in the District of Badaun (Uttar Pradesh), situated about 250 km south-east of Delhi. Apart from its demonstration by culture and direct microscopy of a bronchial aspirate, B. dermatitidis was seen microscopically on two occasions in KOH wet mounts and smears of sputum stained with periodic acid-Schiff's reagent. Anti-B. dermatitidis serum precipitins were shown by immunodiffusion in 3 of 4 s

Based on epidemiologic data during a blastomycosis outbreak, exposure within the home was suspected for two case patients that resided together. Soil and air samples were collected from the basement of their residence. Samples were tested for Blastomyces by culture and polymerase chain reaction (PCR) to compare with an available clinical isolate. An air sample from the basement of the residence was PCR positive for Blastomyces. Sequence data from the air sample and the outbreak clinical isolate were identified as different Blastomyces spp. Despite this, our findings suggest that the basement w

Relapse or recurrence of blastomycosis in patients is rare. Re-infection of a patient with blastomycosis has not been previously reported. In this report, we describe relapse or reinfection with Blastomyces in 2 immunocompetent patients. This is the first study in which genetic typing was performed on paired Blastomyces isolates from the same patient obtained months apart.

Whole-body positron emission tomography (PET) imaging with fluorine-18 fluorodeoxyglucose (F-18 FDG) was performed in a dog with blastomycosis. Prior to PET scanning, blastomycotic lesions had been identified in both eyes, lungs, and tracheobronchial lymph nodes. Increased F-18 FDG uptake was present in these sites in addition to multiple organs without previous clinical evidence of disease. All sites of F-18 FDG uptake were confirmed histologically to be blastomycotic granulomas. F-18 FDG-PET scanning may be useful for defining the extent of fungal disease and detecting persistent foci of inf

Eighteen isolates of Blastomyces dermatitidis were evaluated for their in vitro susceptibilities to ketoconazole, itraconazole, and fluconazole. The MIC ranges were 0.1 to 0.4 microg/ml for ketoconazole, < or =0.018 to 0.07 microg/ml for itraconazole, and 2.5 to 4.0 microg/ml for fluconazole. The ranges for the minimal lethal concentrations were 0.2 to 0.8 microg/ml for ketoconazole, < or =0.018 to 0.07 microg/ml for itraconazole, and 10 to 40 microg/ml for fluconazole. Itraconazole was the most active agent against B. dermatitidis in vitro, while fluconazole was the least active. These result

A 4-year-old male Golden Retriever was evaluated because of chronic non-weight-bearing lameness of the right hind limb associated with penetrating tarsal wounds. Arthritis of the tarsal joint and osteomyelitis of the talus were initially evident. Tarsal arthrodesis was performed 7 months after initial injury, but the lameness persisted. Ten months later, blastomycosis was diagnosed on the basis of results of histologic examination of bone biopsy specimens and serologic tests. No other site of involvement was detected. The limb was amputated, and Blastomyces dermatitidis was isolated from the a

In areas where tuberculosis is endemic other causes of respiratory infection are often overlooked. A pulmonary infiltrate associated with skin lesions without a microbiological diagnosis of tuberculosis should arouse suspicion of a fungal infection. This was demonstrated in a patient in whom Blastomyces dermatitidis, a rare pathogen in southern Africa, was isolated.

In investigating six cases of blastomycosis in two school groups that had separately visited an environmental camp in northern Wisconsin in June 1984, we identified a large outbreak of the disease and isolated Blastomyces dermatitidis from soil at a beaver pond near the camp. Of 89 elementary-school children and 10 adults from the two groups, 48 (51 percent) of the 95 evaluated in September had blastomycosis. Of the cases, 26 (54 percent) were symptomatic (the median incubation period was 45 days; range, 21 to 106 days). No cases were identified in 10 groups that visited the camp two weeks bef

A purified A-antigen preparation of Blastomyces dermatitidis was determined to be composed of five major glycoprotein bands, visible with Coomassie blue and periodic acid-Schiff staining of polyacrylamide gels. At least 20 additional protein bands were detected by using a silver stain, which was 100 times more sensitive than the Coomassie method. Two components of this mixture were determined to be associated with the A-antigenic activity of B. dermatitidis. Of several antigen preparations examined in Ouchterlony precipitation tests, those reactive with a reference anti-A antiserum contained t

Blastomycosis is an endemic disease in North America and commonly manifests with pulmonary symptoms. Blastomycosis should be consider when patients have persistent infiltrates on imaging in an endemic area. We present a case of a 46-year-old male who presented to the pulmonary clinic with fever, cough with production of yellowish-green sputum and culture of BAL-fluid showed growth of Pseudomonas spp. Antimicrobial therapy was started accordingly, but was not effective. A repeat bronchoscopy was performed and BAL-fluid culture was positive for Blastomyces dermatitidis and liposomal amphotericin

Blastomyces dermatitidis is a thermally dimorphic fungus that can cause serious and sometimes fatal infections, including blastomycosis. After spore inhalation, a pulmonary infection develops, which can be asymptomatic and have lethal effects, such as acute respiratory distress syndrome. Its most common extra-pulmonary sites are the central nervous system, bones, skin, and genito-urinary systems. Currently, no vaccine has been approved by the FDA to prevent this infection. In the study, a peptide-based vaccine was developed against blastomycosis by using subtractive proteomics and reverse vacc

BACKGROUND: Blastomyces dermatitidis, the etiologic agent of blastomycosis, has 2 genetic groups and shows varied clinical presentation, ranging from silent infections to fulminant respiratory disease and dissemination. The objective of this study was to determine whether clinical phenotype and outcomes vary based on the infecting organism's genetic group.
METHODS: We used microsatellites to genotype 227 clinical isolates of B. dermatitidis from Wisconsin patients. For each isolate, corresponding clinical disease characteristics and patient demographic information were abstracted from electro

BACKGROUND: Blastomyces dermatitidis is a dimorphic fungus; its usual port of entry is the lung. Although any organ system can be secondarily involved, non-pulmonary sites of primary infection have been reported.
CASE: This is the first reported case of primary isolated splenic B dermatitidis. The diagnostic value of fine needle aspiration cytology was examined by comparing it with the only previously reported case of primary (not isolated) splenic B dermatitidis.
CONCLUSION: In patients with splenic involvement by B dermatitidis, cytologic evaluation can be prompt and diagnostic. FNA biopsy
