Minimally invasive dynamic screw stabilization using cortical bone trajectory.

BACKGROUND: The conventional pedicle-screw-based dynamic stabilization process involves dissection of the Wiltse plane to cannulate the pedicles, which cannot be undertaken with minimal surgical invasion. Despite some reports having demonstrated satisfactory outcomes of dynamic stabilization in the management of low-grade spondylolisthesis, the extensive soft tissue dissection involved during pedicle screw insertion substantially compromises the designed rationale of motion (muscular) preservation. The authors report on a novel method for minimally invasive insertion of dynamic screws and a mi

[Image-guided bone consolidation in oncology: Cementoplasty and percutaneous screw fixation].

Bone metastases are a common finding in oncology. They often induce pain but also fractures which impair quality of life, especially when involving weight-bearing bones. Percutaneous image-guided consolidation techniques play a major role for the management of bone metastases. Cementoplasty aims to stabilize bone and control pain by injecting acrylic cement into a weakened bone. This minimally invasive technique has proven its efficacy for bones submitted to compression forces: vertebra, acetabular roof, and condyles. However, long bone diaphysis should be treated with caution due to lower res

A computational approach to the "optimal" screw axis location and orientation in the scaphoid bone.

PURPOSE: The development of small cannulated screws permitted minimally invasive percutaneous fixation of acute scaphoid fractures. There are known mechanical advantages to increased screw length and central screw placement, as well as documented deleterious effects of screw malposition, including articular protrusion, proximal pole fracture, and nonunion. The purpose of this study was to compare 2 methods of calculating a screw axis accessible via a volar surgical approach. METHODS: To prevent screw protrusion through the surface of the scaphoid, we required the central screw axis to be cont

Results of Herbert-screw fixation with bone-grafting for the treatment of nonunion of the scaphoid.

We examined twenty-one patients in whom an established nonunion of the scaphoid had been treated with Herbert-screw fixation with bone-grafting; our purpose was to determine the long-term morbidity of the procedure (at an average of forty-eight months; range, seventeen to fifty-five months). According to the modified scaphoid outcome scoring system, eighteen patients had a good or excellent result. No patient had failure of the hardware. Radial deviation was significantly decreased (an average of 17 degrees; 55 percent) compared with that of the contralateral wrist (p = 0.02). The other ranges

Results of intraoral cortical bone screw fixation technique for mandibular fractures.

The intraoral cortical bone screw fixation technique carries many advantages over traditional methods of intermaxillary fixation. Simplicity of the technique, reduction of operative time, and reduction of risk of transmission of human immunodeficiency virus and hepatitis make this an important technique in the armamentarium of busy facial trauma surgeons. Since 1992, 45 mandibular fractures in 29 patients treated with the intraoral cortical bone screw technique for intermaxillary fixation were retrospectively analyzed. At our institution, this technique has become the workhorse method of repai

Pull-out forces of headless compression screws in variations of synthetic bone models imitating different types of scaphoid fractures in good bone quality.

Screw osteosynthesis using headless compression screws has become the accepted gold standard for the surgical treatment of scaphoid fractures. Optimal screw specifications remain controversially discussed. We aimed to investigate the influence of bone model composition on screw stability tests using headless compression screws in different scaphoid fracture models. We conducted pull-out tests using Acutrak2®mini, HCS®, HKS®, HBS®, Herbert/Whipple® and Twinfix® screws. To imitate cortical and cancellous bone, two-layer polyurethane (PU) models with two distinct densities were produced. The cyli

Bone flap fixation with clamp-like devices or with titanium plates and screws: A cost-minimization analysis.

OBJECTIVES: Clamp-like devices such as Cranial LOOP have similar performance and safety than titanium plates and screws to fix the bone flap after a craniotomy. This raises the question of which of these methods is more cost-effective. This cost-minimization analysis compares the cost associated with each of these methods in the setting of an already published RCT. MATERIAL AND METHODS: The data from a previously published RCT was analyzed to assess the mean cost of bone flap fixation after craniotomy using Cranial LOOP or plates and screws; this analysis included the cost of the devices, the

Comparative biomechanical analysis of pedicle screws versus cortical bone trajectory fixation in the treatment of ankylosing spondylitis.

OBJECTIVE: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease, in which an imbalance between loss and formation of bone can lead to ligamentous calcification or osteoporosis. Patients with severe spinal deformities are typically treated with long-segment pedicle screw (PS) internal fixation after bone resection; however, issues with the operation, such as fracture and screw loosening or pulling out, might arise. For patients with low bone quality, the cortical bone trajectory (CBT) has been suggested and developed. METHODS: In order to cure AS, this study set out to look at the

Evaluating the contact anatomy and contact bone volume of spinal screws using a novel drilled surface image.

Intraoperative navigation systems have been widely applied in spinal fusion surgery to improve the implantation accuracy of spinal screws using orthogonal tomographic and surface-rendering imaging. However, these images contain limited anatomical information and no information on bone volume contact by the implanted screw, which has been proven to affect the stability of implanted screws. This study proposed a novel drilled surface imaging technique that displays anatomical integration properties to calculate the contact bone volume (CBV) of the screws implanted along an implantation trajector

Short-Segment Instrumentation with Fractured Vertebrae Augmentation by Screws and Bone Substitute for Thoracolumbar Unstable Burst Fractures.

BACKGROUND: For thoracolumbar burst fractures, traditional four-screw (one above and one below) short-segment instrumentation is popular and has a high failure rate. Additional augmentation at the fractured vertebrae is believed to reduce surgical failure. The purpose of this study was to examine the clinical and radiographic results of patients who underwent short-segment posterior instrumentation with augmentation by screws and bone substitutes at the fractured vertebrae and to compare these data to those of patients who underwent long-segment instrumentation for thoracolumbar burst fracture

Early experience of placing image-guided minimally invasive pedicle screws without K-wires or bone-anchored trackers.

OBJECTIVE Image guidance for spine surgery has been reported to improve the accuracy of pedicle screw placement and reduce revision rates and radiation exposure. Current navigation and robot-assisted techniques for percutaneous screws rely on bone-anchored trackers and Kirchner wires (K-wires). There is a paucity of published data regarding the placement of image-guided percutaneous screws without K-wires. A new skin-adhesive stereotactic patient tracker (SpineMask) eliminates both an invasive bone-anchored tracker and K-wires for pedicle screw placement. This study reports the authors' early
