Pseudomass of the sternal manubrium in osteogenesis imperfecta.

Skeletal abnormalities such as hypertrophic callus formation and "popcorn" calcifications are rare radiological findings of osteogenesis imperfecta, causing tumor-like appearances on imaging. We report on a 7-year-old girl with osteogenesis imperfecta presenting with hepatomegaly and palpable lymphadenopathy in the left inguinal region on physical examination. Computed tomography examination revealed a high-density mass-like lesion of the manubrium sterni. Ultrasonography and a lateral roentgenogram of the chest verified that this was a pseudomass caused by a bowed sternal manubrium.

Radiologic change around bilateral femoral stems in nonHodgkin's lymphoma.

Bilateral pathologic femoral neck fractures caused by nonHodgkin's lymphoma occurred consecutively in a 66-year-old man. Spontaneous bony stabilization after initial extensive bone resorption around the femoral stems after hybrid total hip arthroplasties was observed. This is the first well-documented report on bone change around the femoral stems after bilateral total hip arthroplasties in pathologic femoral neck fractures caused by nonHodgkin's lymphoma.

Distraction lengthening of the forearm for congenital and developmental problems.

Thirty-five callus distractions of the forearm were carried out in 23 patients, who were divided into two groups according to the cause of the shortening. Eleven cases in group A had congenital dysplasia, and the 12 cases in group B had growth disturbances from tumours or infection. The mean amount of lengthening was 27 mm in group A and 25 mm in group B. The mean percentage increase in length was 35% and 17%, respectively. The mean duration of fixation was 159 days and 127 days, respectively. The Healing Index had a positive correlation with the age at surgery in group A, but not in group B.

Changes of callus diameter during axial loading and after fixator removal in leg lengthening.

We retrospectively reviewed nine tibial lengthenings in seven achondroplastic patients. The callotasis method was used, and a unilateral type lengthener, either the Dynamic Axial Fixator (Orthofix, Italy; eight legs) or the High Functional Fixator (Matsumoto Co., Japan; one leg), was applied. The distracted length averaged 14.6 (range 10-18) cm. The minimum diameter of the callus was measured using a ruler on anteroposterior and lateral radiographs. The callus diameter ratio (%) was calculated as the callus diameter divided by the original diaphysis diameter. For periods during axial loading a

Filling the bone defect with osteogenic material. An experimental study.

In this experimental study with bone defects, we focussed on the one hand on external and internal osteogenic callus formation after filling the defect and on the other on the osteochondrogenic differentiation capacity of 4-day-old fibrous-like callus grafts and 12-day-old woven bone grafts in an osteogenic environment. A standard cortical bone defect of the femur was created in 95 young rats. The defect was filled with a cortical bone graft and 4- and 12-day-old callus grafts. The grafts were transplanted as such or in Nucleopore chambers. Follow-up was done at 1, 2, 3 and 6 weeks. The osteoc

Metatarsal lengthening by callotasis during the growth phase.

Congenital or acquired shortness of a metatarsal may cause pain in adjacent metatarsals. From 1983 to 1990, we performed nine metatarsal lengthenings in seven adolescent patients by metaphyseal osteotomy followed by gradual distraction of callus (callotasis). Two patients required bone grafts after the lengthening. We used a rigid, unilateral external fixator designed for use in the hand and foot. At follow-up, from three to ten years later, healing had been achieved in all with an average healing index of 50 days/cm, and metatarsalgia had been relieved by the restoration of correct metatarsal

[Callus luxurians in osteogenesis imperfecta].

A case is introduced with osteogenesis imperfecta, suffered from left femoral fracture and a consecutive hypertrophic callus formation, with extreme swelling on the thigh. Concerning the laboratory tests the only significant difference was the elevation in alkaline phosphatase activity and a moderate elevation in ESR. The case is reported because of it's accidental appearance.

Early experience with leg lengthening by callotasis and chondrodiastasis.

Over a four year period, 20 segments in 16 patients were treated by lengthening by callotasis or chondrodiastasis. The indications for treatment were leg length discrepancy in 13 patients and short stature in three. Patients with leg length discrepancy were all treated by femoral lengthening; the mean length gained was 4.4 cm (range 2.5 to 6.Ocm). For patients with short stature, the mean femoral length gain was 10.0 cm (9.0 to 10.5 cm), and the mean tibial length gain was 8.0 cm (6.5 to 9.0 cm). The commonest problem was pin tract infection, but this always settled with antibiotic therapy. Mo

Enhanced Bone Formation in Segmental Defect Healing Using 3D Printed Scaffolds Containing Bone Marrow Stromal Cells and Small Molecules Targeting Chondrogenesis and Osteogenesis.

Background/Objectives: Nonunion bone healing results from a critical size defect that fails to bridge a bone injury to produce bony union. Novel approaches are critical for refining therapy in clinically challenging bone injuries, but the complex and coordinated nature of fracture callus tissue development requires study outside of the simple closed murine fracture model. Methods: We have utilized a three-dimensional printing approach to develop a scaffold construct with layers designed to sequentially release small molecule therapy within the tissues of a murine endochondral segmental defect

Innovative Callus Release Technique for Closed Reduction in Old Fractures: Case Studies and Methodology.

BACKGROUND In modern limb fracture management, particularly in children, image-guided closed reduction and percutaneous fixation are commonly employed. However, for older, displaced fractures, achieving closed reduction may be challenging or even unfeasible, often necessitating open reduction, which carries a higher risk of complications. We utilized K wires to fragment the fibrous callus and facilitate callus fragmentation in 2 patients with old fractures. In both cases, successful fracture reduction was achieved using a closed technique, followed by percutaneous K wire fixation. CASE REPORT

CXCR4 mediates the effects of IGF-1R signaling in rodent bone homeostasis and fracture repair.

Non-union fractures have considerable clinical and economic burdens and yet the underlying pathogenesis remains largely undetermined. The fracture healing process involves cellular differentiation, callus formation and remodeling, and implies the recruitment and differentiation of mesenchymal stem cells that are not fully characterized. C-X-C chemokine receptor 4 (CXCR4) and Insulin-like growth factor 1 receptor (IGF-1R) are expressed in the fracture callus, but their interactions still remain elusive. We hypothesized that the regulation of CXCR4 by IGF-1R signaling is essential to maintain th

Comparison of accordion maneuver with compression technique alone for the treatment of atrophic fracture nonunion: A rabbit model.

OBJECTIVE: The aim of this study was to compare the histological results of Accordion Maneuver (AM) with Compression Technique Alone (CA) in the treatment of atrophic fracture nonunion in a rabbit model. METHODS: The study was performed on 91 male New Zealand rabbits aged six months, weighing 3.1 to 3.6 kg. The standardized models of atrophic nonunion with a 3-mm fracture were created in the tibias of rats. For the histomorphological study, 22 rabbits were randomly selected and sacrificed. The remaining 69 rabbits were divided into two groups based on the treatment technique: Group AM (n = 36

Delivery of RNAi-Based Therapeutics for Bone Regeneration.

PURPOSE OF REVIEW: The clinical significance, target pathways, recent successes, and challenges that preclude translation of RNAi bone regenerative approaches are overviewed. RECENT FINDINGS: RNA interference (RNAi) is a promising new therapeutic approach for bone regeneration by stimulating or inhibiting critical signaling pathways. However, RNAi suffers from significant delivery challenges. These challenges include avoiding nuclease degradation, achieving bone tissue targeting, and reaching the cytoplasm for mRNA inhibition. Many drug delivery systems have overcome stability and intracellul

Periosteal Mesenchymal Progenitor Dysfunction and Extraskeletally-Derived Fibrosis Contribute to Atrophic Fracture Nonunion.

Atrophic nonunion represents an extremely challenging clinical dilemma for both physicians and fracture patients alike, but its underlying mechanisms are still largely unknown. Here, we established a mouse model that recapitulates clinical atrophic nonunion through the administration of focal radiation to the long bone midshaft 2 weeks before a closed, semistabilized, transverse fracture. Strikingly, fractures in previously irradiated bone showed no bony bridging with a 100% nonunion rate. Radiation triggered distinct repair responses, separated by the fracture line: a less robust callus forma

Finite-Element Syntheses of Callus and Bone Remodeling: Biomechanical Study of Fracture Healing in Long Bones.

Computational simulations of fracture healing are a valuable tool to improve fracture treatment and implants. Several finite-element models have been established to predict callus formation due to mechanobiological rules. This work provides a comprehensive simulation for virtual implantation through the combination of callus simulation and finite-element structural synthesis (FESS) of (re-)modeling during and after healing based on Pauwel's theory of histogenesis and Wolff's law. The simulation is based on a linear elastic material model and includes generation of fracture hematoma and initial

Metatarsal lengthening by callotasis in adults with first brachymetatarsia.

BACKGROUND: Brachymetatarsia is a rare clinical entity that presents a challenging problem for surgeons. One-stage lengthening with an autologous bone graft has been preferred for metatarsals that require a limited lengthening. With a gradual lengthening of metatarsals, callus distraction (callotasis) can achieve a greater length gain and a concomitant lengthening of the soft tissue. This article presents results of callotasis for adults with first brachymetatarsia. METHODS: The outcomes of nine cases of first brachymetatarsia in five adult patients who underwent metatarsal lengthening by cal

Spontaneous femoral shaft fracture after long-term alendronate.

Alendronate is used as first-line treatment for osteoporosis in postmenopausal women worldwide. The evidence supporting its benefit in reducing the risk of fragility fracture among this high-risk group is well established. Due to its mechanism of action, there were concerns about potential reduction in bone turnover, repair of microdamage and hence reduced bone toughness and increased risk of fractures with long-term use. A number of cases have been reported from other countries describing a pattern of atraumatic mid-femoral fractures in women on long-term alendronate therapy. We present a cas
