Brain life and brain death: a proposal for a normative agreement.

This paper reviews moral and cultural assessments which led to the definition of brain death and calls for a similar normative consensus regarding the moral recognition and legal protection of embryonal life related to criteria of brain life. This paper differentiates between cortical brain life I, i.e., the first existence of post-mitotic stationary neurons forming the early cortical plate (54th day post conception), and cortical brain life II, i.e., the beginning of cortical neuro-neuronal synapses (after the 70th day p.c.). The latter are preconditional for establishing the communicative ne…

Lymphoreticular cells in human brain tumours and in normal brain.

The present investigation, using various rosetting assays of cell suspensions prepared by mechanical disaggregation or collagenase digestion, demonstrated lymphoreticular cells in human normal brain (cerebral cortex and cerebellum) and in malignant brain tumours. The study revealed T and B lymphocytes and their subsets (bearing receptors for Fc(IgG) and C3) in 5/14 glioma suspensions, comprising less than 15% of the cell population. Between 20-60% of cells in tumour suspensions morphologically resembled macrophages and less than or equal to 75% of these cells formed strong rosettes. Lymphocyte…

Glucosinolate Metabolites and Brain Health:

Traumatic brain injury recovery prediction by harmonizing real brain CT and synthetic brain MRI: a pilot study.

To enhance the prediction of traumatic brain injury (mTBI) outcomes, we propose a deep learning approach that integrates brain computed tomography (CT) scans with corresponding synthetic T1-weighted magnetic resonance imaging (T1-MRI). Our method significantly outperforms the prediction using CT scans alone. TRACK-TBI Pilot dataset, which includes imaging and clinical outcome data from patients with TBI, is studied. The hypothesis is brain CT and T1-MRI complement each other and together will improve TBI prognosis compared to using either CT or T1-MRI alone. Since CT and T1-MRI may not be avai…

Effect of brain cooling on brain ischemia and damage markers after fluid percussion brain injury in rats.

Although systemic cooling had recently been reported as effective in improving the neurological outcome after traumatic brain injury, several problems are associated with whole-body cooling. The present study was conducted to test the effectiveness of brain cooling without interference with the core temperature in rats after fluid percussion traumatic brain injury (TBI). Brain dialysates ischemia (e.g., glutamate and lactate-to-pyruvate ratio) and injury (e.g., glycerol) markers before and after TBI were measured in rats with mild brain cooling (33 degrees C) and in the sham control group. Bra…

From blood to brain: postmortem brain concentrations and brain-blood ratios of chlorprothixene, clozapine, olanzapine, and quetiapine.

Treatment with antipsychotics such as chlorprothixene, clozapine, olanzapine, and quetiapine can be associated with cardiovascular and metabolic risks and poisoning, making them highly relevant in toxicological investigations. Since antipsychotics primarily act within the central nervous system, brain tissue serves as an alternative or complementary matrix to femoral blood in toxicological analysis. This study aimed to retrospectively evaluate brain concentrations and brain-blood ratios across toxic and non-toxic levels in 466 postmortem cases involving chlorprothixene, clozapine, olanzapine, …

Surgical resection and whole brain radiation therapy versus whole brain radiation therapy alone for single brain metastases.

BackgroundThe treatment of brain metastasis is generally palliative, with whole brain radiation therapy (WBRT), since the majority have uncontrollable systemic cancer. In certain circumstances, such as single brain metastases, death may be more likely from brain involvement than systemic disease. In this group, surgical resection has been proposed to relieve symptoms and prolong survival.ObjectivesTo assess the clinical effectiveness of surgical resection plus WBRT versus WBRT alone in the treatment of single brain metastasis.Search strategyThe Cochrane Cancer Network Specialised trials regist…

Delayed brain hemorrhage associated with prophylactic whole brain irradiation for pediatric malignant brain tumor: a case report.

Intraparenchymal hemorrhage in the left frontal lobe suddenly occurred in a 7-year-old girl who had undergone partial removal of an undifferentiated brain stem glioma and received craniospinal (30 Gy) and posterior fossa booster (20 Gy) irradiation at the age of 20 months. The brain hemorrhage was thought to be delayed irradiation effect. Follow-up neuro-imaging at age 9 years showed two more small occult chronic and subacute hemorrhages in the brain. The possibility of repeated hemorrhage as a delayed reaction to brain irradiation is emphasized.

Gut-to-brain signaling: New frontiers in treating brain disorders.

The microbiota.gut.brain axis is essential for maintaining body balance. This review evaluates how altering this connection can treat various central nervous system disorders. First, we examine how the gut and brain communicate through neural pathways, immune responses, and chemical signals. We focus particularly on molecules produced by gut bacteria, such as short-chain fatty acids. These molecules are key regulators that maintain the blood.brain barrier and guide the development of microglia, the brain's primary immune cells. This review primarily discusses the bidirectional communication ne…

Gut-brain-immune interactions in neonatal hypoxic-ischemic brain injury.

BACKGROUND: Neonatal hypoxia-ischemia (HI) is the leading cause of childhood mortality and neurodevelopmental disability. Despite therapeutic hypothermia as the only clinically established treatment to date, outcomes remain poor for a significant proportion of affected infants. Mechanistic understanding has been brain-oriented in the past, however the gut and immune system are increasingly recognized as active modulators of brain injury, recovery and neurodevelopment. MAIN BODY: Gut microbiota regulate microglial maturation, myelination and blood-brain barrier integrity. Neonatal HI induces g…

Pediatric Traumatic Brain Injury and Microvascular Blood-Brain Barrier Pathology.

IMPORTANCE: Pediatric traumatic brain injury (TBI) is a major cause of morbidity and mortality, with an increased risk of catastrophic outcome compared with adult TBI, including diffuse brain swelling and so-called second impact syndrome. Nevertheless, the biological substrates driving adverse outcomes in pediatric TBI remain poorly described. OBJECTIVE: To compare neuropathological evidence of brain swelling and blood-brain barrier (BBB) disruption after moderate or severe acute TBI in adult vs pediatric case material. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective case series, ca…

Brain Shift during Staged Deep Brain Stimulation for Movement Disorders.

INTRODUCTION: Deep brain stimulation (DBS) is a routine neurosurgical procedure utilized to treat various movement disorders including Parkinson's disease (PD), essential tremor (ET), and dystonia. Treatment efficacy is dependent on stereotactic accuracy of lead placement into the deep brain target of interest. However, brain shift attributed to pneumocephalus can introduce unpredictable inaccuracies during DBS lead placement. This study aimed to determine whether intracranial air is associated with brain shift in patients undergoing staged DBS surgery. METHODS: We retrospectively evaluated 4…

Mother-Infant Brain-to-Brain Synchrony Patterns Reflect Caregiving Profiles.

Biobehavioral synchrony, the coordination of physiological and behavioral signals between mother and infant during social contact, tunes the child's brain to the social world. Probing this mechanism from a two-brain perspective, we examine the associations between patterns of mother-infant inter-brain synchrony and the two well-studied maternal behavioral orientations-sensitivity and intrusiveness-which have repeatedly been shown to predict positive and negative socio-emotional outcomes, respectively. Using dual-electroencephalogram (EEG) recordings, we measure inter-brain connectivity between…

Brain-inhabiting bacteria and neurodegenerative diseases: the "brain microbiome" theory.

Controversies surrounding the validity of the toxic proteinopathy theory of Alzheimer's disease have led the scientific community to seek alternative theories in the pathogenesis of neurodegenerative disorders (ND). Recent studies have provided evidence of a microbiome in the central nervous system. Some have hypothesized that brain-inhabiting organisms induce chronic neuroinflammation, leading to the development of a spectrum of NDs. Bacteria such as Chlamydia pneumoniae, Helicobacter pylori, and Cutibacterium acnes have been found to inhabit the brains of ND patients. Furthermore, several fu…

Brain-lung interaction: a vicious cycle in traumatic brain injury.

The brain-lung interaction can seriously affect patients with traumatic brain injury, triggering a vicious cycle that worsens patient prognosis. Although the mechanisms of the interaction are not fully elucidated, several hypotheses, notably the "blast injury" theory or "double hit" model, have been proposed and constitute the basis of its development and progression. The brain and lungs strongly interact via complex pathways from the brain to the lungs but also from the lungs to the brain. The main pulmonary disorders that occur after brain injuries are neurogenic pulmonary edema, acute respi…

Modeling congenital brain malformations with brain organoids: a narrative review.

BACKGROUND AND OBJECTIVE: During embryonic development, the dysregulation of the proliferation and differentiation of neuronal progenitors triggers congenital brain malformations. These malformations are common causes of morbidity and mortality in patients younger than 2 years old. Animal models have provided considerable insights into the etiology of diseases that cause congenital brain malformations. However, the interspecies differences in brain structure limit the ability to transfer these insights directly to studies of humans. In recent years, brain organoids generated from human embryon…

Brain Protection after Anoxic Brain Injury: Is Lactate Supplementation Helpful?

While sudden loss of perfusion is responsible for ischemia, failure to supply the required amount of oxygen to the tissues is defined as hypoxia. Among several pathological conditions that can impair brain perfusion and oxygenation, cardiocirculatory arrest is characterized by a complete loss of perfusion to the brain, determining a whole brain ischemic-anoxic injury. Differently from other threatening situations of reduced cerebral perfusion, i.e., caused by increased intracranial pressure or circulatory shock, resuscitated patients after a cardiac arrest experience a sudden restoration of ce…

Cortical Brain-Computer Interface for Closed-Loop Deep Brain Stimulation.

Essential tremor is the most common neurological movement disorder. This progressive disease causes uncontrollable rhythmic motions-most often affecting the patient'sdominant upper extremity-thatoccur during volitional movement and make it difficult for the patient to perform everyday tasks. Medication may also become ineffective as the disorder progresses. For many patients, deep brain stimulation (DBS) of the thalamus is an effective means of treating this condition when medication fails. In current use, however, clinicians set the patient's stimulator to apply stimulation at all times-wheth…
