Thirty-eight children were diagnosed as having a brain stem glioma at Nagoya University. Thirty-three patients in our previous series from 1957 to 1983, were treated traditionally with radiation and at late stage with shunting operation for hydrocephalus and/or suboccipital decompression, but not with direct operation for tumors. In general, tumors constantly grew regardless histology and their mean survival time was only 7.0 months even with transient neurological remission. On the other hand, recent five patients since 1984 were treated with prospective multimodality treatment. According to …

Brain-stem auditory evoked responses were measured during sleep in four infants with congenital central alveolar hypoventilation syndrome (Ondine's syndrome) and four controls matched for age and sex. Delays in peak latencies p III and interpeak latencies p I-III were consistently seen in these patients but not in the control children. These abnormalities were reproducible and suggested disruption in the normal auditory pathways at the level of the mid to upper brain stem through which fibers pass close to the area of respiratory control. These abnormalities, both electrophysiologic and metabo…

Auditory brain-stem response (ABR) was measured in 40 patients (80 ears) with confirmed hydrocephalus. Eighty-eight percent of these patients showed some form of ABR abnormality. Responses indicative of brain-stem dysfunction consisted of prolonged I-V interwave latency (38%), reduced V/I amplitude ratio (33%), and abnormalities in wave-shape of components III (27%) and V (53%). In addition, 70% of the patients had elevated ABR thresholds; 45% had responses in excess of 20 dB HL and the remaining 25% had no ABR activity. The etiology of the hydrocephalus, head circumference and brain-stem symp

A patient with long standing orthostatic hypotension with fixed pulse rate due to involvement of the medullary regulatory centres by a lower brain stem glioma is described.

A 38-year-old hypertensive woman presented with acute onset of cranial nerve and long tract signs referable to a lesion in the right dorsolateral pons. A hemorrhage in the suspected area was demonstrated by CT. Although brain stem hemorrhage was previously thought to be only rarely compatible with survival, our patient survived and recovered sufficiently to go home. CT indicates a more favorable prognosis for brain stem hemorrhage than has been previously suspected.

This is a case of a previously healthy female in her fourties presenting with a subacute presentation of bilateral horizontal gaze restricti … erum antiglutamic acid decarboxylase (anti-GAD) levels, and the final diagnosis of GAD seropositive brain stem encephalitis was made.

Cerebral palsy, the most common disability in childhood, is a devastating non-progressive ailment of the infants' brain with lifelong sequelae, e.g., spastic paresis, chronic pain, inability to walk, intellectual disability, behavioral disorders, for which there is no cure at present. CP may develop after pediatric brain damage caused, e.g., by hypoxic-ischemia, periventricular leukomalacia, intracranial hemorrhage, hypoxic-ischemic encephalopathy, trauma, stroke, and infection. About 17 million people worldwide live with cerebral palsy as a result of pediatric brain damage. This reflects both

Enterovirus 71 (EV71) was implicated in a widespread outbreak of hand-foot-and-mouth disease (HFMD) across the Asia Pacific area since 1997 and has also been reported sporadically in patients with brain stem encephalitis. Neurogenic shock with pulmonary edema (PE) is a fatal complication of EV71 infection. Among inotropic agents, milrinone is selected as a therapeutic agent for EV71- induced PE due to its immunopathogenesis. Milrinone is a type III phosphodiesterase inhibitor that has both inotropic and vasodilator effects. Its clinical efficacy has been shown by modulating inflammation, reduc…

The development of a neural stem cell (NSC) treatment for ischemic brain injury would be of great benefit. This review focuses on recent advances in knowledge and practices toward the development of a successful NSC therapy for patients who have experienced stroke. The data supporting potential mechanisms by which somatic stem cells orchestrate neuroprotection, neurogenesis and vascularization, leading to pronounced functional improvements in stroke-induced deficits are critically reviewed. A model is proposed in which the host's immune system plays a central role in stem cell-induced neural r

BACKGROUND: Inaccurate definitions of brain stem syndromes in the literature.
METHOD: The original publications of the authors during the last century and the early years of this one were studied in relation to the neuro-ophthalmological diagnosis and neuroanatomy. It is the goal of this medico-historical investigation to compare the knowledge available at this time when the first descriptions were made with the clinical experience and understanding of today.
RESULTS: The following classical brain stem syndromes are important for an ophthalmologist: diseases with III-paresis: Weber's syndrom…

Rhombencephalitis due to Listeria monocytogenes is a serious form of brainstem inflammation. It is difficult to diagnose on the basis of clinical and laboratory findings alone. We describe a fatal case of Listeria monocytogenes rhombencephalitis in a previously healthy female teenager. Clinical and pathogenetic aspects of this condition are discussed.
