Recurrent acute bronchitis: the association with undiagnosed bronchial asthma.

Forty-six consecutive patients with a history of recurrent acute bronchitis (chest colds) referred from primary care clinics were studied prospectively to determine if hyperreactive airways (mild bronchial asthma) was a concomitant entity. Mild bronchial asthma was diagnosed in 30/46 (65%) patients by history, physical examination, and routine spirometry, or by methacholine bronchial provocation. The incidence of mild bronchial asthma was markedly increased in patients with a history of recurrent acute bronchitis over that seen in the general population. These results emphasize a need to inves

Asthma in Patients With Confirmed Pulmonary Sarcoidosis.

Pulmonary sarcoidosis and asthma can present identical symptoms, making clinical evaluation difficult if the two diseases overlap. Diagnostic challenges often lead to either overdiagnosis of asthma in patients with confirmed sarcoidosis or withholding appropriate asthma treatment. The true prevalence of patients with bronchial hyperresponsiveness, the hallmark of asthma, among sarcoidosis patients remains unknown, although it is suspected to be significantly higher compared to the general population. Therefore, a positive bronchial challenge test, often considered crucial for confirming asthma

Formaldehyde asthma--rare or overlooked?

A total of 230 persons who had been exposed to formaldehyde and suffered from asthma-like respiratory symptoms were examined between January 1, 1977, and May 31, 1983. All the subjects had a bronchial provocation test with formaldehyde. On the basis of the medical and occupational history of the patients, the specific bronchial provocation test, and other test results, 12 cases were considered to be caused by specific sensitization to formaldehyde. All subjects had been exposed occupationally. An exposure period of between 1 mo and 19 yr preceded the onset of symptoms. Three persons displayed

[Anesthetic Management for Patients with Bronchial Asthma].

Bronchial asthma is a major disease with bronchial hyperreactivity. In Japan, patients with bronchial asthma are increasing in number. Bronchospasm may be evoked by tracheal intubation, and it may increase perioperative complications. In some cases, broncho- constriction may lead to a poor outcome. The treatment of asthmatic attack and the long-term management for bronchial asthma are described in the Japa- nese Asthma Prevention and Management Guideline 2015 (JGL2015). There is no specific strategy for anesthesia in patients with bronchial asthma, but we should know its treatment and manageme

Decrease of beta-receptors after the antigen-specific bronchial provocation test in bronchial asthma.

Airway hyperreactivity to different stimuli is a characteristic feature of bronchial asthma. The beta-adrenergic theory, as proposed by Szentivanyi in 1968, tried to explain with beta-adrenergic dysfunction, the cause not only of bronchial hyperreactivity but of all atopic diseases. In a previous work, we found that asymptomatic asthmatic patients presented a number of beta-receptors similar to that of a normal control group, while symptomatic asthmatic patients presented a significantly decreased number when compared to both groups (40% decrease). In view of these results, we proposed to stud

Inflammation and functional outcome in diisocyanate-induced asthma after cessation of exposure.

BACKGROUND: The clinical outcome of diisocyanate-induced asthma has been found to be poor despite cessation of exposure. Our aim was to study the outcome of diisocyanate-induced asthma after initiation of inhaled steroid treatment at a mean period of 7 months (range 2-60 months) after cessation of exposure by following up lung function and bronchial inflammation. METHODS: Bronchoscopy was performed on 17 patients 2 days after a positive inhalation challenge test, after which budesonide 1600 mug a day was started. Bronchoscopy, spirometry, and histamine challenge tests were repeated at 6 month

Bronchial provocation with metacholine in young children: a simplified method.

Although pointed out as a characteristic of bronchial asthma, bronchial hyperreactivity (BH) has been documented in other clinical situations such as fibrocystic disease of pancreas and chronic obstructive pulmonary disease. BH is diagnosed in 90% of asthmatic patients and in almost all of those who are symptomatic. BH can be measured by specific bronchial provocation tests (BPT) (antigens and occupational sensitizing agents) and by exercise. Since their introduction, nonspecific BPT have been extensively used as a preliminary method for the evaluation of BH and also to diagnose bronchial asth

[THE RELATIONSHIP AMONG BRONCHIAL HYPERSENSITIVITY, BRONCHIAL HYPER REACTIVITY, AND BRONCHIAL HYPERRESPONSIVENESS IN ASTOGRAPH].

BACKGROUND: Bronchial hyperresponsiveness testing is useful for diagnosing and predicting the risk of bronchial asthma attacks. The Astograph is a tidal breathing method often used in as bronchial provocation testing in Japan. The minimum methachorine dose (Dmin) indicates bronchial sensitivity and is used mainly as an index of bronchial hyperresponsiveness. However, Dmin does not measured hyperresponsiveness, it cannot be compared directly with PC20 in standard methods using FEV1. METHODS: We investigated the relationship among sensitivity, reactivity, and hyperresponsiveness with the Astogr

Effect of anti-IL-5 monoclonal antibody on allergic bronchial eosinophilia and airway hyperresponsiveness in mice.

The effect of pretreatment with rat anti-murine interleukin-5 (IL-5) antibody on antigen-induced bronchial eosinophilia and bronchial reactivity to acetylcholine in mice were studied. Three inhalations of an antigen by actively sensitized animals resulted in an increase in airway reactivity to acetylcholine. Twenty-four hours after the final inhalation, the number of leukocytes (mononuclear cells and eosinophils) and the amount of IL-5 in BALF increased significantly. Anti-IL-5 monoclonal antibody inhibited the antigen-induced increase of eosinophils with little effect on bronchial hyperreacti

[Bronchial hyperresponsiveness and factors of bronchial inflammation under the exercise-induced asthma in schoolchildren].

88 schoolchildren with bronchial asthma were observed at the department of pulmonary disease and allergology of the Chernivtsi regional children clinical hospital. The study showed that the content of aldehyde- and ketoderivatives of 2,4-dinitrophenylhydrazones of neutral character in the expired air condensate (AKDNPH) was presumably higher in patients with the exercise-induced bronchial asthma thus indicating the more significant activity of the inflammatory process in bronchi under the studied phenotype of the disease. Non-specific bronchial hyperresponsiveness was presumably higher in chil

[Study of the protective action of nedocromil sodium with bronchial cold-air provocation in children with bronchial asthma].

12 children with known bronchial hyperreactivity and asthma had isocapnic cold air hyperventilation challenges of their bronchial airways. In a double-blind cross-over study they inhaled either placebo or nedocromil sodium (2 puffs à 2 mg) from a MDI before challenge. 2 subjects were found to be non-responders during the two days of the study. The remainder all reacted on the placebo days, while 5 subjects were completely protected on the nedocromil-days and 5 subjects were partially protected. Nedocromil is a potent agent for the suppression of bronchial hyperresponsiveness.

Adult linear IgA bullous dermatosis with bronchial involvement.

A 54-year-old man is described, suffering from adult linear IgA bullous dermatosis with involvement of the bronchial mucosa. The main respiratory symptoms were recurring haemoptysis, episodic narrowing of the airways and persistent non-specific bronchial hyperreactivity. On CT scan the trachea had a saber-sheath shape with tracheal ring calcification. Endoscopically the tracheo-bronchial mucosa was diffusely purpuric and hyperaemic and also showed pale elevated plaques, bullous lesions and ulceration. Histological examination of biopsies of skin and nasal and tracheo-bronchial mucosa showed su

Effect of a peptide leukotriene receptor antagonist, ONO-1078, on guinea-pig models of asthma.

Peptide leukotrienes have been suggested to play an important role in bronchial asthma. As antigen-induced bronchoconstrictions, airway hyperreactivity, and pulmonary eosinophil accumulation are characteristics of the pathology of asthma, we investigated the effect of a peptide leukotriene receptor antagonist, ONO-1078, on these responses using guinea-pig models of asthma. Oral administration of ONO-1078 (3 mg/kg) significantly inhibited slow-reacting substance of anaphylaxis-mediated bronchoconstriction induced by i.v. administered ovalbumin. ONO-1078 (30-100 mg/kg), when administered orally

No remission in 60% of those with childhood-onset asthma - A population-based cohort followed from 8 to 28 years of age.

BACKGROUND: Although remission occur, childhood-onset asthma may persist until adulthood. Since few longitudinal population-based studies have followed a cohort from childhood until adulthood, the knowledge on predictors of persistence of asthma is sparse. AIM: To estimate persistence of asthma from 8 to 28 years and its associated factors. METHODS: Within the OLIN (Obstructive Lung Disease in Northern Sweden) studies, a cohort was recruited in 1996 (age 8y, n = 3430) and followed annually with questionnaires about asthma and risk factors until 19y. Clinical examinations included skin prick

[Bronchial asthma and sports].

Physical exercise can induce an acute attack in most asthmatics. Exercise-induced asthma (EIA) is therefore a common clinical presentation of bronchial asthma. EIA is likely a consequence of bronchial hyperreactivity whereby alterations in bronchial osmolarity, heat-loss by hyperventilation and physical activity per se are discussed as pathogenic triggers. Clinical presentation, pathogenesis and diagnosis of exercise-induced asthma are summarized. Pharmacologic and non-pharmacologic possibilities are covered in some depth. In this context the value of sports-therapy in treatment of asthma is r

Exercise, methacholine, and adenosine 5'-monophosphate challenges in children with asthma: relation to severity of the disease.

Bronchial hyperreactivity is a characteristic feature of asthma and can be evaluated by different challenges. The aim of this study was to compare exercise, methacholine (MCH), and adenosine 5'-monophosphate (AMP) challenges in 135 children and young adults (mean age +/- SD, 12.4+/-3.9 years) with asthma, and to examine the utility of the different challenges in predicting those children with asthma likely to require prophylactic antiinflammatory treatment. The sensitivity of MCH challenge in detecting bronchial hyperreactivity (at or below 8 mg/mL) was 98%, that of AMP challenge (at or below

Laryngeal hypersensitivity and abnormal cough response during mannitol bronchoprovocation challenge.

BACKGROUND AND OBJECTIVE: Inhalational challenge with dry mannitol powder may potentially induce cough by two mechanisms: airway bronchoconstriction or laryngeal irritation. This prospective observational study investigated laryngeal and bronchial components of cough induced by mannitol challenge. METHODS: We recruited consecutive patients referred for clinical mannitol challenge. The Newcastle Laryngeal Hypersensitivity Questionnaire (LHQ) was administered. Throughout testing, coughs were audio-recorded to derive a cough frequency index per time and dose of mannitol. Relationships between co

[Pharmacologic aspects of the effect of air pollutants on the respiratory system].

The principal pharmacologically acting pollutants of the atmosphere in this country are sulfur dioxide and ozone. Sulfur dioxide produces bronchial hyperreactivity, damages the bronchial epithelium and reflexively increases the secretion of phlegm. The explanation of pharmacological mechanisms of its effect can be activation of cholinergic or neuropeptidic reflex mechanisms, release of histamine, or other, for the time being less known mechanisms. Ozone increases the reactivity of the smooth muscle to the inhaled bronchoconstrictive substances most probably by releasing the mediators of inflam

[Strongyloides stercoralis infection in a cortico-dependent patient with chronic airflow obstruction].

We report a case of Strongyloides stercoralis infection in a male agricultural worker with corticoid dependent chronic obstructive air flow disease and bronchial hyperreactivity. The clinical picture initially involved digestive symptoms and bronchospasm resistant to steroid treatment. Diagnosis was established by demonstrating the larvae of S. stercoralis in stool. We emphasize that this infection should be suspected in patients living in endemic areas such as Spain, and describe S. stercoralis-caused respiratory manifestations that can be masked by bronchial disease.

Health and environmental consequences of the world trade center disaster.

The attack on the World Trade Center (WTC) created an acute environmental disaster of enormous magnitude. This study characterizes the environmental exposures resulting from destruction of the WTC and assesses their effects on health. Methods include ambient air sampling; analyses of outdoor and indoor settled dust; high-altitude imaging and modeling of the atmospheric plume; inhalation studies of WTC dust in mice; and clinical examinations, community surveys, and prospective epidemiologic studies of exposed populations. WTC dust was found to consist predominantly (95%) of coarse particles and
