Chronic wet cough: Protracted bronchitis, chronic suppurative lung disease and bronchiectasis.

The role of persistent and recurrent bacterial infection of the conducting airways (endobronchial infection) in the causation of chronic respiratory symptoms, particularly chronic wet cough, has received very little attention over recent decades other than in the context of cystic fibrosis (CF). This is probably related (at least in part) to the (a) reduction in non-CF bronchiectasis in affluent countries and, (b) intense focus on asthma. In addition failure to characterize endobronchial infections has led to under-recognition and lack of research. The following article describes our current p

An alteration in the host-parasite relationship in subjects with chronic bronchitis prone to recurrent episodes of acute bronchitis.

Acute episodes of bronchitis have been shown to be unequally distributed within a population of subjects with chronic bronchitis. Two groups were identified based on incidence of acute bronchitis--subjects who were 'infection-prone' (2-5 infections per year) and those who were 'non-infection-prone' (0-1 infections per year). Minor differences in clinical parameters existed, except for smoking experience. The non-infection-prone group included more current smokers, and the total smoking experience (in 'pack years') was significantly greater in this group. Between-year analysis demonstrated a st

Newer oral antimicrobials and newer etiologic agents of acute bronchitis and acute exacerbations of chronic bronchitis.

Although the role of antimicrobial therapy in the treatment of chronic bronchitis is unproven, physicians continue to look for microbial etiologies to explain episodes of clinical acute bronchitis and better antimicrobial agents with which to treat these episodes. The newest major pathogen of acute bronchitis is Branhamella catarrhalis, a neisseriae-like organism that has become the third most commonly recognized cause of this disease after Haemophilus influenzae and Streptococcus pneumoniae. Because it produces beta-lactamase, B catarrhalis presents a special problem in drug therapy. A number

Chronic Bronchitis Affects Outcomes in Smokers without Chronic Obstructive Pulmonary Disease (COPD).

BACKGROUND: Chronic bronchitis (CB) importantly affects outcomes in smokers with COPD, but the effects on smokers without COPD are less well known and less emphasized. The aim of our study was to investigate the possible effects of CB on clinical outcomes in smokers without COPD (noCOPD) and compare them with the effects in smokers with COPD (COPD). METHODS: For that purpose, we studied 511 smokers, 302 with and 209 without COPD, followed for 10 years in an academic COPD ambulatory setting. Chronic bronchitis was defined as the presence of cough and sputum production for at least 3 months in

Chronic cough due to chronic bronchitis: ACCP evidence-based clinical practice guidelines.

BACKGROUND: Chronic bronchitis is a disease of the bronchi that is manifested by cough and sputum expectoration occurring on most days for at least 3 months of the year and for at least 2 consecutive years when other respiratory or cardiac causes for the chronic productive cough are excluded. The disease is caused by an interaction between noxious inhaled agents (eg, cigarette smoke, industrial pollutants, and other environmental pollutants) and host factors (eg, genetic and respiratory infections) that results in chronic inflammation in the walls and lumen of the airways. As the disease advan

Chronic exposure to volcanic environments and chronic bronchitis incidence in the Azores, Portugal.

The village of Furnas, like other active volcanic areas in the world, exhibits high levels of hazardous gases. We aimed to investigate the existence of a possible association between chronic exposure to volcanic sulfur gases and chronic bronchitis. To investigate this, we used two populations, one exposed to active manifestations of volcanism (Furnas) and another from an area where no volcanic activity took place for over three million years (Santa Maria), both in the Azores. We used data on the incidence of chronic bronchitis among both populations (1991-2001), obtained from the records of ea

Mucolytic agents versus placebo for chronic bronchitis or chronic obstructive pulmonary disease.

BACKGROUND: Individuals with chronic bronchitis or chronic obstructive pulmonary disease (COPD) may suffer recurrent exacerbations with an increase in volume or purulence of sputum, or both. Personal and healthcare costs associated with exacerbations indicate that therapies that reduce the occurrence of exacerbations are likely to be useful. Mucolytics are oral medicines that are believed to increase expectoration of sputum by reducing its viscosity, thus making it easier to cough it up. Improved expectoration of sputum may lead to a reduction in exacerbations of COPD. OBJECTIVES: Primary obj

BACKGROUND: Individuals with chronic bronchitis or chronic obstructive pulmonary disease (COPD) may suffer recurrent exacerbations with an increase in volume or purulence of sputum, or both. Personal and healthcare costs associated with exacerbations indicate that any therapy that reduces the occurrence of exacerbations is useful. A marked difference among countries in terms of prescribing of mucolytics reflects variation in perceptions of their effectiveness. Bronchiectasis, Chronic Suppurative Lung Disease and Protracted Bacterial Bronchitis.

Bronchiectasis is a structural airway disease characterized by dilated bronchi and bronchioles due to severe or recurrent lower airways inflammation. Bronchiectasis can occur as a result of chronic pulmonary aspiration. Bronchiectasis may also be associated with a wide variety of systemic diseases, which should be considered in the differential diagnosis. Children with bronchiectasis typically have a chronic or recurrent productive cough and carry a significant burden of disease with a considerable impact on quality of life. The diagnosis of bronchiectasis is made by high-resolution chest comp

Hydrator Therapies for Chronic Bronchitis. Lessons from Cystic Fibrosis.

Patients with the chronic bronchitis form of chronic obstructive pulmonary disease and cystic fibrosis share similar clinical features, including mucus obstruction of airways and the development of chronic/recurrent airways infections that often manifest as disease exacerbations. There is growing evidence that these diseases may have parallels in disease pathogenesis as well, including cystic fibrosis transmembrane conductance regulator dysfunction, mucus dehydration, and defective mucociliary clearance. As progress is made in the development of therapies that target the basic defects that lea

Acute exacerbation of chronic bronchitis. Guidelines for antibiotic selection.

An acute exacerbation of chronic bronchitis (AECB) is frequently caused by viral infection. Because of the impairment of mucociliary clearance, secondary bacterial infection is likely to ensue. Haemophilus influenzae, Streptococcus pneumoniae, and Moraxella catarrhalis account for about 50% of all episodes of AECB. The best approach to antibiotic selection for bacterial exacerbations is stratification by exacerbation type, patient type, or antibiotic type.

Treatment of acute exacerbations of chronic bronchitis: antibiotic therapy.

Acute exacerbation of chronic bronchitis (AECB) is a condition associated with increased morbidity and mortality. Bacterial infections are the most frequent cause of exacerbations. The most common bacterial etiologies include Haemophilus influenzae, Moraxella catarrhalis, and Streptococcus pneumonia. The diagnosis of AECB is often based on the clinical presentation, but microbiological assessment, including Gram stain and sputum culture should be done. Antibiotic therapy should be used in patients with the following characteristics: underlying lung disease, frequent exacerbations, and comorbid

Induced sputum, eosinophilic bronchitis, and chronic obstructive pulmonary disease.

The application of sputum induction and refined methods of sputum examination has provided the opportunity to examine cell and molecular markers of airway inflammation in asthma, COPD, and other airway diseases. The measurements are relatively noninvasive and can be applied safely, with care, even in more severe exacerbations of asthma and severe COPD. Induced sputum examination can be applied at random and repeatedly and gives results that are reproducible, valid, and responsive to changes in treatment. An eosinophilic bronchitis, defined as sputum eosinophilia, is typical of asthma but can a

Amoxycillin/clavulanate in acute purulent exacerbations of chronic bronchitis.

Twenty patients, all admitted to hospital with acute purulent exacerbations of chronic bronchitis associated with Haemophilus influenzae, Streptococcus pneumoniae or beta-lactamase producing Branhamella catarrhalis were treated twice daily for ten days with amoxycillin/clavulanate. Ten patients were first given 1000 mg amoxycillin with 200 mg potassium clavulanate intravenously bd for three days, before crossing to the standard oral regimen of 1000 mg amoxycillin + 250 mg potassium clavulanate bd. Clinical results on day 10 were excellent in 16/20 patients, but 14 patients developed recurrence

Cefuroxime axetil in acute purulent exacerbations of chronic bronchitis.

51 hospitalised patients with acute purulent exacerbations of chronic bronchitis were treated for ten days with two daily 1 g doses of the orally absorbed pro-drug cefuroxime axetil. However, some patients were still infected with Haemophilus influenzae or Branhamella catarrhalis at follow-up, and sputum purulence remained. Clinical results were "excellent" or "good" in 60% of the evaluable patients one week after the end of the treatment. Mean peak serum concentrations averaged 12.8 mg/l with mean peak sputum concentrations of 1.8 mg/l. The MIC90 value for H. influenzae was 4 mg/l. Three pati

Pulmonary oedema and chronic bronchitis associated with perioperative sepsis.

The relationship of pulmonary oedema and chronic bronchitis in perioperative septic patients has been examined. There is a close association. It appears likely that the reason for this association is a reduction of the pulmonary vascular capacity due to destructive changes, hypertrophy of muscle within the walls of blood vessels and widespread regional hypoxic pulmonary vasoconstriction.

Transient hypoxaemia during sleep in chronic bronchitis and emphysema.

Arterial oxygenation, breathing pattern, and electroencephalogram were studied during sleep in patients with chronic bronchitis and emphysema and in healthy subjects. All of the 10 "blue and bloated" patients had episodes of transient hypoxaemia lasting 1--100 min, when their oxygen saturation fell by more than 10%, whereas such desaturation did not occur in 2 "pink and puffing" patients or in 4 healthy subjects. Hypoxaemic episodes usually occurred during the rapid-eye-movement stage of sleep. It is suggested that these hypoxaemic episodes result from a combination of hypoventilation and impa
