Chronic bursitis and tenosynovitis revealing Whipple's disease.

Joint complaints, most commonly intermittent arthritis, are the initial manifestation in about three-fourths of Whipple's disease cases. We herein report on two cases wherein Whipple's disease manifested itself as chronic bursitis and tenosynovitis at several sites. A 42 year-old man had bilateral olecranon bursitis, a right patellar bursitis and an extensor tenosynovitis on the left wrist and a 54 year-old man had extensor tenosynovitis at both wrists and a bilateral tenosynovitis of the extensors at both ankle. Methotrexate in both patients and etanercept in one of them were not effective. P…

Septic bursitis in human immunodeficiency virus infection.

Septic bursitis is a well described entity in immunocompromised patients. Common factors contributing to an immunocompromised state are alcoholism and corticosteroid therapy. We describe 2 patients with Staphylococcus aureus olecranon bursitis with evidence of human immunodeficiency virus (HIV) infection and no other contributing factors to immunodeficiency. The clinical course in these patients was refractory to conservative management. Such a persistent course of septic bursitis, especially in the absence of other contributing factors, should raise the possibility of HIV infection.

Septic subcutaneous bursitis. Report of sixteen cases.

Sixteen cases of bacterial infection of subcutaneous bursae were reviewed. Septic subcutaneous bursitis usually occurred following injury to the skin overlying the bursa. Two cases developed after bursal corticosteroid injection for treatment of aseptic bursitis. Bursal fluid culture yielded staphlococci in 14 cases and streptococci in 2. Blood cultures obtained in 8 patients were negative. No patient had clinical manifestations suggestive of bacteremia. Needle and/or incisional drainage of the bursa plus systemic antibiotics led to uneventful recovery in 14 patients. Two patients had chronic …

Interspinous Bursitis due to Staphylococcus aureus in an Adolescent.

BACKGROUND: Interspinous bursitis is exceptionally rare in children, contrasting with degenerative forms in adults. CASE PRESENTATION: A 16-year-old male presented with acute severe low back pain, fever (39°C), and focal L3-L5 tenderness. CRP was 233 mg/L; blood cultures grew methicillin-susceptible Staphylococcus aureus. Imaging progression (US, CT-scan, MRI, PET-CT) confirmed L3-L4 interspinous bursitis without bone/disc involvement, distinguishing it from spondylodiscitis. MANAGEMENT: Cefazolin IV (10 days) followed by oral ciprofloxacin (4 weeks), a lumbar brace, and rehabilitation yield…

Isolated tuberculosis of the patella mimicking as prepatellar bursitis.

Patellar tuberculosis (TB) is a rare manifestation of musculoskeletal tuberculosis, accounting for a small proportion of cases. This case re … h patellar TB. The condition often presents with non-specific symptoms, often mimicking prepatellar bursitis, necessitating a high level of clinical suspicion, particularly in patients from the endemic areas. Radiographic imaging and histopathological examination play crucial roles in establishing an accurate diagnosis. Antitubercular chemotherapy forms th …

Imaging features of reactive bursitis secondary to osteochondroma.

PURPOSE: The purpose of this study was to determine imaging features of reactive bursitis secondary to osteochondroma. MATERIALS AND METHODS: Fourteen patients with reactive bursitis secondary to osteochondroma were retrospectively reviewed. RESULTS: The 14 patients were 11 males and 3 females ranging in age from 18 to 67 years (mean, 33.6 years). The locations were as follows: scapula (n = 2), lesser trochanter (n = 6), greater trochanter (n = 1), distal femur (n = 2), distal fibula (n = 1), iliac bone (n = 2). On CT, six bursae were hypodense and 1 was isodense compared to muscle. All burs…

Spontaneous Healing of the Crust Type of Nasopharyngeal Bursitis.

The nasopharyngeal bursa is an invaginated space that links the remnants of the notochord with the pharyngeal epithelium. Nasopharyngeal bursitis can occur in the midline of the upper portion of the nasopharynx if the notochord remnants and midline recess continue to persist. Nasopharyngeal bursitis, also known as Tornwaldt cyst, can be divided into cystic and crusting types. Crust types are relatively rare. Electrocauterization of the ulcerative lesion of the crust type is an effective and minimally invasive method. This report presents a case of crust-type nasopharyngeal bursitis that heals …

Bursectomy, Curettage, and Chemotherapy in Tuberculous Trochanteric Bursitis.

We presented three patients with trochanteric tuberculosis and described the clinical and imaging findings of the infection. Histology revealed a necrotizing granulomatous bursitis and microbiology confirmed tuberculosis. All cases were successfully treated with bursectomy and curettage of the trochanteric lesion and antituberculous chemotherapy including isoniazid, pyrazinamide, rifampicin, and ethambutol.

OK-432 sclerotherapy for malleolar bursitis of the ankle.

BACKGROUND: The purpose of this study was to evaluate the clinical outcomes and usefulness of OK-432 (Picibanil) sclerotherapy as a new option in the conservative treatment of patients with malleolar bursitis of the ankle. METHODS: Retrospectively, we reviewed a total of 20 consecutive patients (20 feet) in whom OK-432 sclerotherapy had been performed between March 2009 and June 2010. After aspiration of fluid in the malleolar bursal sac, 0.05 mg of OK-432 was injected into the malleolar bursal sac. We evaluated the clinical outcomes and side effects at the following time points: 2 weeks, 1 m…

Four common types of bursitis: diagnosis and management.

Bursitis is a common cause of musculoskeletal pain and often prompts orthopaedic consultation. Bursitis must be distinguished from arthritis, fracture, tendinitis, and nerve pathology. Common types of bursitis include prepatellar, olecranon, trochanteric, and retrocalcaneal. Most patients respond to nonsurgical management, including ice, activity modification, and nonsteroidal anti-inflammatory drugs. In cases of septic bursitis, oral antibiotics may be administered. Local corticosteroid injection may be used in the management of prepatellar and olecranon bursitis; however, steroid injection i…

Giant iliopsoas bursitis: a complication of chronic arthritis.

Iliopsoas bursitis is a poorly recognized cause of hip pain that requires early recognition to avoid potentially serious complications caused by compression of adjacent structures. It can occur in the setting of trauma in athletes or those who engage in heavy labor and is also associated with acute or chronic arthritis. We describe the cases of 2 patients, one of whom developed a femoral neuropathy, while the other had marked venous compression of the lower limb resulting from enlargement of the iliopsoas bursa. Magnetic resonance imaging offers the most accurate information on the extent of t…

Gallium scintigraphy in a case of tuberculous trochanteric bursitis.

We report a 67-year-old woman with systemic lupus erythematosus and systemic disseminated tuberculosis affecting the femoral trochanteric bursae, a site rarely affected by tuberculosis. For quantification of the inflammation with gallium-67 scintigraphy, we calculated the radioisotope count ratio in the most inflamed areas, the right lateral thorax and bursa of the right greater trochanter. Systemic scanning with this modality allowed evaluation of the extent of lesions and simple quantitative determination of the severity of inflammation, yielding information useful for the follow-up of the p
