Peripheral distribution of the increased cardiac output by secretin during acute ischemic left ventricular failure.

Secretin, a 27-amino acid peptide with both vasodilating and inotropic properties, was investigated according to its effects on peripheral flow distribution in acute ischemic left ventricular failure accomplished by embolization of the left coronary artery in anesthetized dogs. Electromagnetic monitoring of renal, carotid, femoral and superior mesenteric artery flow was used. During the induction of heart failure a general reduction of flow with preponderance of that of the carotid and femoral arteries occurred. The secretin infusion caused increases in renal flow by 58%, carotid flow by 63%,

Severe, but completely reversible heart failure in an adult with meningococcal sepsis.

A 25-year-old woman developed meningococcal septicaemia, among others complicated by a combination of cardiogenic and distributive shock requiring full haemodynamic support in the acute phase. Besides clinical course and treatment in the acute phase, we describe treatment in the recovery phase and make some comments concerning the pathophysiology. This case demonstrates the importance of intensive treatment for cardiac failure in the initial period and long-term therapy afterwards because myocardial function can be fully restored.

[ECMO and cardiopulmonary support].

ECMO (extracorporeal membrane oxygenation) may be viewed as a prolonged cardiopulmonary bypass allowing for a prolonged cardio-respiratory support. Since its introduction in the 60's, its indication has expanded from acute respiratory failure to acute cardiac failure refractory to conventional treatments. The target group involves mainly those patients presenting with a cardiogenic shock following cardiac surgery or acute myocardial infarction. The advantages of this form of circulatory support, when compared with long-term ventricular assist devices, are its less invasive aspect, its ease of

The haemodynamic effects of sublingual nitroglycerin spray in severe left ventricular failure.

The use of sublingual nitroglycerin spray has been studied in twenty patients referred to our Intensive Care Unit for further treatment of severe left ventricular failure. Within five minutes of spray administration significant falls in pulmonary artery occlusion pressure, mean arterial pressure, heart rate, right atrial pressure and systemic vascular resistance occurred. Similarly significant increases in cardiac index, stroke volume index and left ventricular stroke work index were found. These results show a considerable improvement in the haemodynamic parameters of invasively monitored pat

Temporary Mechanical Circulatory Support and Shock Teams in High-Risk Cardiac Surgery: The Strategic Evolution of Protected Cardiac Surgery.

BACKGROUND: Protected cardiac surgery is a proactive strategy that applies temporary mechanical circulatory support to prevent perioperative low cardiac output syndrome in high-risk cardiac surgery. Rather than escalating vasoactive agents after hemodynamic decline, the approach emphasizes early identification of physiologic vulnerability and timely initiation of support. METHODS: We conducted a structured literature review of peer-reviewed studies published from 2000 to 2024 on temporary mechanical circulatory support in adult cardiac surgery. There were 52 studies selected based on relevanc

[Intensive Care in Heart Surgery - is All Now Different?].

With approximately 100000 operations performed in Germany per year, cardiac surgery is among the surgical specialties that require intensive care tratment most frequently. Although all therapeutic aspects of ICU treatment are of high importance among cardiac surgery patients, there is a focus on hemodynamics with the overarching goal of sufficient oxygen delivery. Patients undergoing cardiac surgery are particularily prone to hemodynamic instability and low cardiac output syndrome, potentially culminating into cardiogenic shock. This article presents an overview of essential elements of intens

[Thiamine deficiency. A cause of cardiac insufficiency not to be ignored].

Thiamine deficiency is one of the classical causes of high out put for heart failure. Deficiency of this vitamin may be nutritional or secondary to alcoholic intoxication. We felt it would be interesting to describe a typical case of cardiac beriberi in order to recall the clinical presentation and the pathophysiology.

Circulatory management of the unstable cardiac patient.

The objective of this article is to provide an orderly and concise approach to the treatment of the unstable cardiac surgery patient. The common causes of hemodynamic instability in this patient population are reviewed. The various pharmacologic, mechanical, and electric therapeutic options available for each clinical situation are explored, and a sequential treatment algorithm is developed.

[Intensive care treatment of cardiac surgery patients: focus on hemodynamics].

Hemodynamic treatment is a core task in the intensive medical care of cardiac surgery patients. The patient's underlying disease, the type of surgical procedure and the patient's individual characteristics play key roles in the selection of the treatment regimen. The basis of any targeted hemodynamic treatment is the differential diagnosis of the underlying pathological disorder. The established basic monitoring can be expanded if necessary. Postoperative circulatory dysfunction can occur as low cardiac output syndrome (LCOS) predominantly with left or right heart failure or as vasoplegic synd

Extending the 'host response' paradigm from sepsis to cardiogenic shock: evidence, limitations and opportunities.

Recent clinical and research efforts in cardiogenic shock (CS) have largely focussed on the restoration of the low cardiac output state that is the conditio sine qua non of the clinical syndrome. This approach has failed to translate into improved outcomes, and mortality has remained static at 30-50%. There is an unmet need to better delineate the pathobiology of CS to understand the observed heterogeneity of presentation and treatment effect and to identify novel therapeutic targets. Despite data in other critical illness syndromes, specifically sepsis, the role of dysregulated inflammation a

The confounding issue of comorbid renal insufficiency.

The United States is currently beleaguered by twin epidemics, heart failure (HF) and renal insufficiency (RI). HF and RI frequently coexist in the same patient, and this conjunction, often called the "cardiorenal syndrome," has important therapeutic and prognostic implications. Approximately 60% to 80% of patients hospitalized for HF have at least stage III renal dysfunction as defined by the National Kidney Foundation (NKF), and this comorbid RI is associated with significantly increased morbidity and mortality risk. Numerous studies have demonstrated that in patients with HF, indices of rena

Pathophysiology of the beta-adrenoceptor system in chronic heart failure: consequences for treatment with agonists, partial agonists or antagonists?

The human heart contains both beta 1-adrenoceptors and a considerable number of beta 2-adrenoceptors, both of which bring about positive inotropic and chronotropic effects of beta-adrenoceptor agonists in vitro and in vivo. In chronic heart failure, the decrease in beta-adrenoceptor function is related to the severity of the disease. However, both cardiac beta 1- and beta 2-adrenoceptors seem to be differentially affected depending on the type of heart failure and its aetiology. beta 1-adrenoceptor function decreases in all forms of chronic heart failure. beta 2-adrenoceptor function, on the o

Refractory right ventricular failure due to granulocytic sarcoma.

A 72-year-old white man had granulocytic sarcoma (chloroma) characterized by right ventricular failure. Seven years before his final hospitalization, a diagnosis of agnogenic myeloid metaplasia was made on bone marrow biopsy, and the patient was treated with phosphorus 32. Three months before his death, the patient developed refractory right ventricular failure. Biopsy of a mass in the anterior abdominal wall demonstrated granulocytic sarcoma. The patient died 10 days after the biopsy was performed. Autopsy showed disseminated granulocytic sarcoma with massive encasement of the heart and the g
