PURPOSE: To evaluate the effectiveness of a nurse-driven indwelling urinary catheter removal protocol in an acute care setting.
BACKGROUND: The most common hospital-acquired infection is catheter-associated urinary tract infection (CAUTI). These infections account for 40% of all hospital-acquired infections. Although indwelling urinary catheters may be a needed intervention, studies have demonstrated over half of the patients who receive a urinary catheter do not have a valid indication for its use.
METHODOLOGY: A retrospective chart review allowed measurement of prevalence of catheter usage

This paper describes a series of cases with severe hand injury that required antebrachial flaps as part of treatment, and their functional results. The clinical records of patients with a diagnosis of traumatic hand injury and major skin cover losses, reconstructed with a reverse-flow forearm flap, were reviewed. The following variables were studied: type of flap, sex, age, mechanism of injury, receiver site, size, adjacent injuries and their treatment, vascular integrity test prior to flap placement, operative time, follow-up and complications. A total of 25 patients were included, with 25 re

Patients in the setting of the intensive care unit can develop intra-abdominal complications that may worsen outcome. Clinical suspicion of such complications coupled with early diagnosis and treatment may reduce morbidity and mortality associated with these processes. This article addresses the diagnosis and management of some of the common causes of intra-abdominal catastrophes.

A 25-year-old woman with a history of systemic lupus erythematosus who was taking steroids came to the hospital because of vague signs and symptoms of weight loss, constipation, and oral ulcers. Multiorgan dysfunction developed, and catastrophic antiphospholipid syndrome was suspected. She was treated with an intravenous infusion of heparin, but she experienced a subdural hemorrhage and died on day 10 of the hospitalization. An autopsy revealed disseminated invasive aspergillosis. This case illustrates that invasive aspergillosis is a frequently missed diagnosis and should be part of the diffe

Iodine, as a trace element, is a necessary and limiting substrate for thyroid gland hormone synthesis. It is an essential element that enables the thyroid gland to produce thyroid hormones thyroxine (T4) and triiodothyronine (T3). Synthesis of Thyroid Hormones and Iodine Metabolism. Three iodine molecules are added to make triiodothyronine, and four for thyroxine - the two key hormones produced by the thyroid gland. Iodine deficiency The proper daily amount of iodine is required for optimal thyroid function. Iodine deficiency can cause hypothyroidism, developmental brain disorders and goiter.

Thrombotic disease of the newborn is uncommon but usually associated with serious morbidity and mortality. Although the operating mechanisms of coagulation and fibrinolysis are the same in all age groups, plasma concentrations of the two systems' components are significantly different in neonates compared to children and adults. This places neonates at greater risk for thrombosis that may rise considerably if a predisposing factor is present or a genetic or medical condition predisposing to thrombosis coexists. While marginal, the possibility of abnormal bleeding secondary to congenital prothr

Various modes of failure of primary and revision total hip arthroplasty have been well documented in the literature over the past 30 years. Concerns over polyethylene wear, osteolysis, and hypersensitivity reactions leading to component loosening and early revision have been evaluated and reported in the literature. Routine follow-up is important to monitor for postoperative issues that might lead to the subsequent need for revision. This article describes a case of a 64-year-old man who initially presented 11 years prior with an intertrochanteric fracture, which failed secondary to varus align

Calciphylaxis is a vascular calcification-cutaneous necrosis syndrome, usually seen in patients with end-stage renal disease and secondary hyperparathyroidism. We report a 57-year-old polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes (POEMS) syndrome patient complicated with extensive skin ulcers due to calciphylaxis. He first noted a painful cutaneous ulcer on his left thigh, and then skin lesions rapidly worsened, resulting in multiple intractable ulcers with gangrene on his legs and trunk in a few months. Serum vascular endothelial growth factor (VEGF) wa

This prospective study investigated how pain catastrophizing was related to labor pain intensity and physical recovery after childbirth. Eighty-eight women giving birth for the first time completed the first questionnaire before delivery. Eighty-two of those returned the second questionnaire after delivery. Participants were classified as catastrophizers (n=38) or non-catastrophizers (n=44) based on their scores on the Pain Catastrophizing Scale. Comparison of the groups showed that catastrophizers anticipated and experienced more intense pain (p<.0125) and had poorer physical recovery (p<.012

Catastrophic antiphospholipid syndrome (CAPS), described by Asherson in 1992, is a rare form of antiphospholipid syndrome resulting in multiorgan failure with a mortality rate of about 50%. The syndrome occurs in patients with either systemic lupus erythematosus and other rheumatic diseases (systemic sclerosis, rheumatoid arthritis, primary Sjogren syndrome) or alone. Whereas in "classic" antiphospholipid syndrome (APS), medium-large vessels are involved, a diffuse small vessel ischemia and thrombosis (microangiopathic disease) leading to a severe multiorgan dysfunction is predominant in CAPS.

Toxocariasis, a common helminthozoonosis with a worldwide prevalence, usually manifests as 'visceral larva migrans' (VLM). Among its wide range of clinical presentations, large vessel thrombosis has never been described before. We report a case of aortic thrombosis caused by Toxocara canis infection in a young male who was successfully treated with albendazole.

INTRODUCTION: The catastrophic antiphospholipid syndrome (CAPS) is a rare expression of the antiphospholipid syndrome (APLS). The all predicting factors of happening of CAPS are not yet discovered. Some women presenting an APLS with infertility have recourse to ovulation induction (OI).
EXEGESIS: After an OI, a woman is admitted for acute renal failure, hypertension, seizures, respiratory failure. History and immunologic tests are in favour of APLS. Renal biopsy highlights a thrombotic microangiopathy. Diagnosis of CAPS is made. Patient improves after respiratory assistance, parenteral treatm

Polyarteritis nodosa (PAN) is a vasculitis of small- and medium-sized vessels, characterized by vascular aneurysms that can lead to ischemia and infarction. We present the case of a patient with classic polyarteritis nodosa in abdominal organs who additionally demonstrated antitreponemal and antiphospholipid antibodies, resulting in a severe cerebral thrombosis. This case highlights: 1. The potential for false-positive syphilis serology in PAN patients, and 2. The rare coexistence of polyarteritis nodosa and the catastrophic antiphospholipid syndrome.

We describe a young woman whose initial presentation was dominated by acute diarrhoea. Life-threatening multiorgan failure rapidly ensued and necessitated mechanical ventilation and dialysis treatment. An initially elongated activated partial thromboplastin time prompted further coagulation tests that led to the detection of positive lupus anticoagulant, a highly elevated IgG-anticardiolipin (aCL) antibody titre, and prolonged dilute Russell's viper venom time. Histological examination of samples obtained during endoscopy revealed widespread intestinal thrombotic microangiopathy. In view of th

Catastrophes on an international scale with an increasingly pronounced terrorist factor (for example, the terrorist attacks in the United States of America on September 11, 2001) are occurring with increasing frequency. This fact compels the international forensic medical community--especially at the present moment--to undertake coordinated and rapid measures in the area, closely related to our profession, of issues connected with problems of identifying the victims of catastrophes. To date organizational issues have not been harmonized to the required at an international level degree. The sub

The aim of this contribution is to examine the effect of an indiscriminate epidemic on a population to assess whether or not a catastrophic event can be identified from examination of paleodemographic data. Using paleodemographic techniques, the death assemblage from the Royal Mint site, London, a Black Death cemetery dated 1349 AD, is compared with that from St. Helen-on-the-Walls, York, which dates from the twelfth to the sixteenth centuries AD. The Royal Mint site represents a catastrophic cemetery, while that of St. Helen-on-the-Walls is of an attritional type. Certain features of the pale

A 13-year-old female patient with life-threatening asthma was treated with the inhalational anesthetic agent, 1% isoflurane, for 202 h (140 minimum alveolar concentration (MAC) hours). The patient survived and exhibited no significant side effects attributable to the medication. The present patient report provides additional clinical information supporting the utilization of long-term isoflurane general anesthesia in the management of refractory status asthmaticus that have not responded to aggressive medical management.
