Cement-associated signs of inflammation: retrospective analysis of the effect of excess cement on peri-implant tissue.

PURPOSE: Excess cement left in the peri-implant sulcus after the placement of prosthetic restorations risks inflammation in the peri-implant tissue. While many current studies deal with the question of how to avoid undetected excess cement, relatively little is known about the clinical consequences of this complication. This study analyzed the clinical findings associated with excess cement. Further, the influence of the sojourn time of undetected excess cement in the peri-implant pocket on clinical findings was investigated. MATERIALS AND METHODS: Within the scope of a retrospective clinical

Effect of hand packing versus cement gun pressurization on cement mantle in total knee arthroplasty.

BACKGROUND: Gun pressurization in total knee arthroplasty (TKA) may result in better cement penetration than hand packing, leading to fewer tibial plate failures. We compared cement intrusion characteristics between vacuum mixing and gun pressurization versus hand mixing and packing in the proximal tibia among patients undergoing TKA. METHODS: We analyzed 6-week radiographs from 77 consecutive patients for cement area and zone-specific intrusion using computer-assisted image analysis. RESULTS: Penetration into tibial anteroposterior zones 1-6 was not significantly different between the techn

Er:YAG Laser Versus Sandblasting for Build-Up Conditioning in Adhesive Cementation: A Retrospective Study of 187 Posterior Indirect Restorations.

Background: Adhesive indirect restorations have become increasingly common in daily clinical routine in most dental practices. Before etching and adhesive application, a sandblasting procedure is essential to clean and increase the microporosity of the surface. Air abrasion with aluminum oxide particles significantly improves the bond strength. However, this procedure may have some limitations, such as the presence of powder particles. Recently, the Er:YAG laser in QSP mode has been proposed for conditioning build-ups prior to adhesive cementation. The aim of this study was a retrospective ana…

Polyethylene liner cementation into a well-fixed metal acetabular shell for the management of periacetabular osteolysis: a systematic review.

PURPOSE: Although various papers have reported on the clinical performance of cup retention with cementation of a new liner and bone grafting in the management of well-fixed cups with polyethylene wear and periacetabular osteolysis after total hip arthroplasty (THA), no systematic review of this topic has been published to date. METHODS: Medline, EMBASE and Cochrane Library were searched for articles published from January 1999 to January 2019 using "osteolysis" AND "well-fixed", "osteolysis" AND "retro-acetabular", "bone graft" AND ("retention" OR "retained" OR "stable") AND "cup", and "ceme…

Evaluation of Cement Leakage and Complications After Cement-augmented Pedicle Screw Fixation: A Retrospective Case Series.

BACKGROUND/AIM: Pedicle screw fixation is a standard technique for vertebral fractures but shows high loosening and pull-out rates, especially in osteoporotic bone. Cement augmentation can improve the screw-bone interface and construct stability. However, cement leakage rates of up to 94% have been reported, with rare but potentially serious complications such as pulmonary cement embolism. This study aimed to retrospectively evaluate the rate of cement leakage and serious complications after polymethylmethacrylate (PMMA)-augmented pedicle screw fixation. PATIENTS AND METHODS: We retrospective

Cement leakage into the paravertebral venous system and pulmonary cement embolism following percutaneous vertebroplasty: a case report.

UNLABELLED: Pulmonary cement embolism is a potentially serious complication following percutaneous vertebroplasty. The risk factors for pulmonary cement embolism have not been clearly identifed. Cement leakage into the azygos vein or in the inferior vena cava has been identifed as a risk factor for pulmonary cement embolism. In addition cement leakage into the paravertebral venous system also seems to be related to pulmonary cement embolism. Here we report a case with detailed preoperative data, intraoperative findings, and imaging evidence to demonstrate the relationship between cement leakag

Fractures of Corin 'Taper-Fit' CDH stems used in 'cement-in-cement' revision total hip replacement.

We describe two cases of fracture of Corin Taper-Fit stems used for cement-in-cement revision of congenital dysplasia of the hip. Both prostheses were implanted in patients in their 50s, with high offsets (+7.5 mm and +3.5 mm), one with a large diameter (48 mm) head and one with a constrained acetabular component. Fracture of the stems took place at nine months and three years post-operatively following low-demand activity. Both fractures occurred at the most medial of the two stem introducer holes in the neck of the prosthesis, a design feature that is unique to the Taper-Fit stem. We would u

[2- to 7-year results of cement-free and cemented joint replacement in femoral neck fractures and coxarthrosis].

In 43 patients with cementless and 43 patients with cemented hip prosthesis (S+G hip system) a clinical and radiological follow up was performed 2-7 years after total hip replacement. In average the follow up period was 43 or 48 months. We found good or excellent results in 88% after cementless and 95% after cemented fixation. The results after cementless hip replacement were caused by loosening in the early postoperative period in 3 cases and more frequently thigh pain. In conclusion of our results and the expected later results after 10 years follow up we prefer the cementless fixation in pa

Chromium content in human skin after in vitro application of ordinary cement and ferrous-sulphate-reduced cement.

The amount of chromium found in human skin after in vitro application of cement suspensions on full-thickness human skin in diffusion cells was investigated. Cement suspensions made from ordinary Portland cement or Portland cement with the chromate reduced with added ferrous sulphate were used. The cement suspensions were either applied on the skin surface under occlusion for 48 h or applied repeatedly every 24 h for 96 h. No statistically significant difference in chromium content of skin layers between skin exposed to ordinary Portland cement, skin exposed to cement with added ferrous sulpha

Failure of partial cementation to achieve implant stability and bone ingrowth: a long-term roentgen stereophotogrammetric study of tibial components.

Thirty patients with gonarthrosis were operated on with the PCA primary total knee prosthesis and had the tibial component fixed to the bone by partial cementation. In the first two groups of patients, cementation was by a peripheral rim of high and low-viscosity cement, respectively. In the third group, the pegs were cemented with the low-viscosity cement. Follow-up was performed with use of clinical parameters and roentgen stereophotogrammetric analysis. Clinically, the series was successful apart from a problem with tibial component wear, necessitating revision in five patients. At 8 years,…

An Intra-Patient Cohort Study on the Effect of Resin-Cement Types on Cement Remnants in Implant-Supported Restorations.

OBJECTIVES: The choice of retention mechanism in implant-supported restorations (ISRs) strongly influences biological outcomes, with cement-retained ISRs linked to peri-implant complications due to excess cement. Factors such as crown design, margin location, and cement type affect the extent of residual cement and its clinical implications. MATERIAL & METHODS: This prospective, two-arm, non-randomized clinical trial compared cement remnants of self-adhesive resin (RC) and resin-modified glass ionomer (RMGIC) cement in 30 patients, and 60 ISR. Comparisons were performed on identical pairs of

The impact of cementing technique in polished taper fit hip stems : a modelling analysis of implant-cement interface.

AIMS: Cemented polished taper fit (PTF) stems are the femoral implant of choice for total hip arthroplasty (THA) in many locations worldwide. There is increasing evidence that periprosthetic fracture may be the single major contributor to reoperation with these stems. The aim of this study was to demonstrate how mismatches at the implant-cement interface may occur and the subsequent effect of these incongruities on the contacting area and the forces transmitted to the cement mantle. METHODS: A parametric equation-based model was developed to determine the contact mismatch relative to axial st
