Integrated defense system overlaps as a disease model: with examples for multiple chemical sensitivity.

The central nervous, immune, and endocrine systems communicate through multiple common messengers. Over evolutionary time, what may be termed integrated defense system(s) (IDS) have developed to coordinate these communications for specific contexts; these include the stress response, acute-phase response, nonspecific immune response, immune response to antigen, kindling, tolerance, time-dependent sensitization, neurogenic switching, and traumatic dissociation (TD). These IDSs are described and their overlap is examined. Three models of disease production are generated: damage, in which IDSs fu

Improving understanding of trigger points and widespread pressure pain sensitivity in tension-type headache patients: clinical implications.

The underlying etiology of tension type headache (TTH) is not understood. The current paper highlights the etiologic role of muscle trigger points (TrPs) to the development and maintenance of central sensitization in TTH and its clinical repercussion for proper management of these patients. Areas covered: A literature search on Pub Med for English-language published papers between 1990 and May 2017 to provide the most updated data on the topic was conducted. Current literature suggests that the referred pain elicited by active trigger points (TrPs) contributes to the manifestations of TTH. The

The role of neurotrophic factors in psychostimulant-induced behavioral and neuronal plasticity.

Several neurotrophic factors influence the development, maintenance and survival of dopaminergic neurons in the mammalian central nervous system (CNS), including neurotrophin-3 (NT-3), brain derived neurotrophic factor (BDNF), ciliary neurotrophic factor (CNTF), basic fibroblast growth factor (bFGF) and glial derived neurotrophic factor (GDNF). This review focuses on the role of these neurotrophic factors in psychostimulant-induced behavioral sensitization, a form of dopamine-mediated neuronal plasticity that models aspects of paranoid schizophrenia as well as drug craving among psychostimulan

Regression of central sensitization after bilateral focused ultrasound medial thalamotomies in atypical orofacial pain? - a case report.

Orofacial pain syndromes are a heterogenous group of pain disorders resulting in substantial morbidity. They are often refractory to medical treatment and can have an enormous impact on quality of life. Central sensitization plays a major role in chronic pain disorders and poses a diagnostic and therapeutic challenge. Transcranial magnetic resonance-guided focused ultrasound bilateral medial thalamotomy is an emerging treatment in chronic pain syndromes with central sensitization. We here describe a 76-year-old female patient with a long history of atypical orofacial pain and central sensitiza

[Central sensitisation and chronic organ-related pain].

Central sensitisation and chronic organ-related pain Central sensitisation is an essential mechanism in chronic musculoskeletal pain. In recent years, increasing evidence has come to light suggesting that central sensitisation is also a cause of chronic organ-related pain. This has several important implications. One is the need for broader professional education in mechanism-based pain classification. Today, simple clinical diagnostic procedures are used for the identification of central sensitisation in chronic musculoskeletal disorders, and these may also prove useful in the classification

Endometriosis and chronic pain syndrome: The role of stress mechanisms.

Patients with endometriosis report increased levels of perceived stress, which correlates with disease severity and pain intensity, creating feedback loops that perpetuate the cycle of endometriosis inflammation, chronic pain syndrome, and pathological stress response. Stress and chronic pain form an integrated biopsychosocial system in which neuroendocrine dysregulation, central sensitization, neuroinflammation, large-scale network reorganization, and maladaptive threat learning mutually reinforce each other. Sex differences, developmental experiences, and resilience factors modulate this int

Cytokine and immune system abnormalities in fibromyalgia and other central sensitivity syndromes.

The nervous system as well as the immune system use common signaling molecules for intra- and inter-system communications. Specifically, both entities produce a similar array of peptide and non-peptide transmitters that act on a common set of receptors present in the two systems. One important set of such signaling molecules are cytokines. The wide distribution of cytokine receptors throughout the body, including the immune and the nervous system allows direct communication between these two entities. In addition to cytokines the nervous system and immune system also communicate with each othe

Glial cells in neuropathic pain.

Neuropathic pain is a chronic, debilitating condition that affects ∼7-8% of the global population and remains difficult to treat because of limited therapeutic options. Over the past two decades, mounting preclinical evidence has highlighted the crucial role of glial cells in the development, maintenance, and resolution of neuropathic pain. Conditions such as peripheral nerve injury are associated with reactive responses of glial cells, namely microglia and astrocytes in the central nervous system and satellite glial cells in the peripheral ganglia. Under physiological conditions, glial cells

Integrating the Central Sensitization Inventory (CSI) into Neuropelveological Practice: A Systematic Review of Endometriosis and Overlapping Pelvic Pain Syndromes.

Background: The surgical management of chronic pelvic pain (CPP), particularly in endometriosis, often focuses on lesion excision or nerve decompression. However, persistent pain frequently occurs despite "anatomical perfection," suggesting central nervous system involvement. Neuropelveology faces a "surgical paradox" when dealing with central sensitization (CS), where peripheral interventions fail to address a systemic nociplastic state. Methods: This systematic review followed PRISMA guidelines and was registered in PROSPERO (CRD420261335008). A search across PubMed, Embase, and Cochrane (20

Is the Central Sensitization in Chronic Nonspecific Low Back Pain Structural Phenomenon or Psychological Reaction? A Narrative Review.

Lower back pain (LBP) is a common condition affecting primarily populations in developed countries, placing a significant burden on public health systems around the world. A high rate of pain recurrence increases the risk of developing a chronic syndrome and the occurrence of complex psychosocial and professional problems. Symptoms lasting longer than 12 weeks are associated with the risk of sleep problems, depression, and anxiety. In the 21st century, the development of knowledge about central sensitization (CS) to pain allowed for a better understanding of the pathomechanism of pain and the

The causes of the syndromes of Landry (1859) and of Guillain, Barré and Strohl (1916).

Post-infectious and post-vaccinal peripheral neuritis or encephalomyelitis have frequently been considered the human equivalents of experimental allergic neuritis (EAN) or encephalomyelitis (EAE). The major basis for these comparisons between diseases in humans and experimental animals rests on the classical observations of "paralytic accidents" of Pasteur-type vaccination against rabies. These old observations in humans injected with brain tissue indicate a remarkable heterogeneity of periphéral as well as central nervous system syndromes, quite in contrast with the remarkable specificity for

Measurement Properties of the Central Sensitization Inventory: A Systematic Review.

BACKGROUND AND OBJECTIVE: Central sensitization (CS) is a phenomenon associated with several medical diagnoses, including postcancer pain, low back pain, osteoarthritis, whiplash, and fibromyalgia. CS involves an amplification of neural signaling within the central nervous system that results in pain hypersensitivity. The purpose of this systematic review was to gather published studies of a widely used outcome measure (the Central Sensitization Inventory [CSI]), determine the quality of evidence these publications reported, and examine the measurement properties of the CSI. [Myofascial approach in tension-type headache management: a scientific assessment].

Tension-type headaches (TTH) are a very common condition. The most recent theories on TTH occurrences suggest that a myofascial component, through trigger points (TP), gives rise to pain signals from the periphery to the central nervous system (CNS). These nociception could lead to CNS sensitization and headaches. Studies show that identification and treatment of TP is a valid therapeutic option for TTH. Amongst the treatment techniques, dry needling (DN) and ischemic compression (IC) seem to be the most effective. These technics can be easily acquired and applied by any doctor.

Implication of system xc- in neuroinflammation during the onset and maintenance of neuropathic pain.

BACKGROUND: Despite the high prevalence of neuropathic pain, treating this neurological disease remains challenging, given the limited efficacy and numerous side effects associated with current therapies. The complexity in patient management is largely attributed to an incomplete understanding of the underlying pathological mechanisms. Central sensitization, that refers to the adaptation of the central nervous system to persistent inflammation and heightened excitatory transmission within pain pathways, stands as a significant contributor to persistent pain. Considering the role of the cystine

Pain Modulation and Central Sensitization in Painful Diabetic Peripheral Neuropathy: Updated Narrative Review and Future Directions.

Painful diabetic peripheral neuropathy (DPN) arises from damage to sensory neurons, manifesting in persistent pain. Progressive nerve degeneration can result in sensory loss and, in severe cases, amputation, exacerbating emotional distress and diminishing quality of life. Continuing investigation into DPN mechanisms is essential for advancing effective pain management. Our previous narrative review identified oxidative stress and central sensitization as key pathways linking hyperglycemia to nerve injury and pain. The present review provides an updated synthesis of evidence in this area. The o

[MRI sequences in the detection of cavernous angiomas].

We prospectively evaluated 30 patients with CNS cavernous angioma with a high field (1.5T) magnetic resonance imager. In all patients the MRI protocol included: gradient echo T1*-weighted imaging, spin echo T2*-weighted imaging and gradient echo T2*-weighted imaging. We evaluated each case for the number of lesions detected on each sequence. Gradient echo T2*-weighted imaging with a long TE (TE: 35ms) was the most sensitive sequence for the detection of cavernous angiomas allowing detection of small lesions that were not identified on the other sequences.

Social Factors in Ethanol Sensitization.

Behavioral sensitization is a neuroadaptive process characterized by an increase in a particular behavior after repeated exposure to drugs or other stimuli, such as stress. Sensitization can also be extended to neurochemical and neuroendocrine sensitization. Several factors can influence sensitization to the effects of ethanol. For instance, stress is an important component in addiction that can strengthen ethanol-induced behaviors. In animal models, stressful situations can be induced by alterations in social aspects of the animal's environment, such as maternal separation, social conflicts,

The role of muscles in tension-type headache.

The tenderness of pericranial myofascial tissues and number of myofascial trigger points are considerably increased in patients with tension-type headache (TTH). Mechanisms responsible for the increased myofascial pain sensitivity have been studied extensively. Peripheral activation or sensitization of myofascial nociceptors could play a role in causing increased pain sensitivity, but firm evidence for a peripheral abnormality still is lacking. Peripheral mechanisms are most likely of major importance in episodic TTH. Sensitization of pain pathways in the central nervous system due to prolonge
