A 68-year-old male presented with progressive quadriparesis. Twelve years previously he had undergone anterior decompression with bone grafting for cervical ossification of the posterior longitudinal ligament associated with spinal trauma. Radiological examination showed ossification of the transverse ligament of the atlas (TLA) and severe stenosis of the upper cervical canal. Anterior dislocation of the atlas was also present, but the occiput/atlas/axis unit was perfectly stable due to the prior anterior fusion. Suboccipital decompression and laminectomy of the atlas were performed, and his s

A 20-year-old male incurred a vertex injury of the skull, disruption of the transverse ligament of the atlas, and a Jefferson fracture--lateral displacement of the lateral masses of the atlas in relation to the lateral margins of the second cervical vertebra. The combination of Jefferson fracture with transverse ligament disruption was not appreciated initially (or even late) because of the head injury. The hospital course was complicated by the patient's initial unconsciousness and restlessness caused by associated intracranial injury. Roentgenograms in all views of the cervical spine were es

BACKGROUND: Atlas is the first cervical vertebra. Rarely, does the atlas vertebra fuse with the occipital bone, and this occipitalization of the atlas is known as the assimilation of the atlas. Congenital fusion of the atlas with the occipital bone is the common denominator of a galaxy of skull defects, several examples of bony distortion of the foramen magnum were reported by early anatomists.
MATERIALS AND METHOD: A total of 210 dry skulls of unknown sex and age from Dr. Rajendra Gode Medical College, Amravati, Department of Anatomy, and various other medical, Dental, and Ayurveda colleges

It has hitherto been assumed without exception that A-P films of acute torticollis demonstrate neither malposture nor subluxation or (worse still) luxation of the atlas. In contrast, there is ventral subluxation of the atlas in all cases, although in about one-half of them this only become manifest in ventral flexion, with a tendency to form an angle between the odontoid process and the first cervical vertebra. In conformity with the head posture in acute torticollis, X-ray findings in the head joints are normal on A-P films. Fundamental to acute torticollis is that when the head is held in ce

OBJECTIVE: To provide a comprehensive review of the biomechanics, pathophysiology, and clinical management of atlas fractures.
METHODS: Selected literature review.
RESULTS: Atlas fractures account for 25% of craniocervical injuries, 3% to 13% of cervical spine injuries, and 1% to 3% of all spinal injuries. Motor vehicle accidents account for 80% to 85% of atlas fractures, and the mechanism of injury is axial loading. Isolated atlas fractures are more common; however, 40% to 44% of atlas fractures have concomitant axis fractures. Fractures of isolated anterior or posterior arches are more com

OBJECTIVE: To describe a case of a vasomotor, vascular form of thoracic outlet syndrome that causes upper extremity thermal asymmetry, and to discuss a single subject case study (N-of-1) comparing the correlation of a subjective test for putative atlas vertebral subluxation complex (supine leg-length inequality) with a single blinded objective measurement [temperature differential (delta degree T)] on the dorsum of the hands.
CLINICAL FEATURES: A 71-yr-old woman with a cold, painful right hand and chronic neck pain sought chiropractic evaluation. There was a left head tilt and muscular hypert

Upper cervical fracture-dislocations are series of pathological injuries occurred in occipital, atlas and axis caused by traumatic impact, which often results in severe clinical consequences, such as paraplegia, quadriplegia, and even death. In light of the potential severe clinical outcomes, it is important to define the type of upper cervical fracture-dislocations depending on which the clinician can deal with the disease properly. Owing to its specific anatomical structures, the upper cervical fractures occurr to the unique osseous structures or ligamentous connections always present in pre

Cervical fractures are rare in paediatric population. In younger children, cervical fractures usually occur above the level of C4; whereas in older population, fractures or dislocations more commonly involve the lower cervical spine. Greater elasticity of intervertebral ligaments and also the spinal vertebrae explains why cervical fractures in paediatric ages are rare. The injury usually results from a symmetric or asymmetric axial loading. In paediatric cases, most fractures occur through the synchondroses which are the weakest links of the atlas. The prognosis depends on the severity of the

STUDY DESIGN: A case report of a patient with a combined anterior and posterior arch anomaly of atlas leading to compressive myelopathy.
OBJECTIVE: To describe a rare presentation of cervical stenosis at the atlas where congenital defects of the posterior arch with an incurving portion exists. This report includes a feasible hypothesis for the development of this anomaly on the basis of previous hypotheses and surgical findings.
SUMMARY OF BACKGROUND DATA: Congenital defects of the posterior arch of atlas are uncommon and congenital symptomatic cervical stenosis is very rarely seen at this l

A 2-year-old girl fell off a bunk bed onto a parquet floor. She immediately reported neck pain and presented with muscle spasm and limited motion of the cervical spine (C-spine). Plain X-rays of the C-spine showed no osseous lesion. Owing to persisting pain and limited motion in the neck, MRI of the C-spine was obtained which revealed intact ligaments and cervical spinal cord, as well as soft tissue swelling in front of the anterior arch of the atlas. Subsequent CT of the C-spine confirmed a complete, undisplaced fracture of the anterior arch of the atlas (Gehweiler type I fracture). A Minerva

This case report represents the finding of an abnormal atlas vertebra during dissection of 34 cadavers. The case which we describe appeared to have an atlas with a mass at a position similar to that of a centrum. This mass may have been a developmental abnormality of the centrum of the atlas or may have represented a congenital abnormality, known as the os odontoideum. Alternatively the abnormality may have been due to non-union of an odontoid fracture, with subsequent fusion to the atlas. The case report will discuss which of these three alternatives is more probable, and the embryology and p

OBJECT: Isolated cervical canal stenosis at the level of the atlas (C-1) is a rare cause of cervical myelopathy in the pediatric population. It has been associated with several genetic disorders including spondyloepiphysial dysplasia congenita, Down syndrome, and Klippel-Feil syndrome. The purpose of this study is to highlight the authors' experience with 4 additional pediatric cases, review the literature, and report a new association of this disease with Williams syndrome.
METHODS: The medical records and radiological imaging studies of 4 patients treated at Texas Children's Hospital for sy

The first cervical vertebra (atlas, C1) is an important element of the vertebral column because it connects the cranial base with the cervical column, thus helping to maintain head posture and contributing to neck mobility. However, few atlases are preserved in the fossil record because of the fragility of this vertebra. Consequently, only eight well-preserved atlases from adult Neandertals have been recovered and described. Here, we present nine new atlas remains from the El Sidrón Neandertal site (Asturias, Spain), two of which (SD-1643 and SD-1605/1595) are sufficiently well preserved to al
