Diagnostic dilemmas of child sexual abuse.

Globally, an estimated 12-13% of children experience sexual abuse, with prevalence rates of approximately 18% among girls and 8% among boys. Clinicians evaluating pediatric patients who present with anogenital complaints or lesions face considerable diagnostic challenges. The purpose of this narrative review is to outline the most frequently encountered conditions that can mimic child sexual abuse (CSA), thereby raising clinical awareness and helping to minimize both false-positive and false-negative diagnoses. We conducted a semi-systematic literature search in PubMed covering the period from

Child Abuse: Recognition, Reporting, and Response.

Because child abuse occurs across all genders, ethnicities, and socioeconomic groups, nurses working with pediatric patients and their families need keen observation and assessment skills. The purpose of this article is to discuss the recognition, reporting, and appropriate response to child abuse. Caring for children who have been abused and their families is challenging yet presents the opportunity to demonstrate Christ's compassion. Awareness of one's own emotional responses requires meaningful self-care strategies that are also discussed.

Identifying Child Abuse Fatalities During Infancy.

When a healthy infant dies suddenly and unexpectedly, it is critical to correctly determine if the death was caused by child abuse or neglect. Sudden unexpected infant deaths should be comprehensively investigated, ancillary tests and forensic procedures should be used to more-accurately identify the cause of death, and parents deserve to be approached in a nonaccusatory manner during the investigation. Missing a child abuse death can place other children at risk, and inappropriately approaching a sleep-related death as maltreatment can result in inappropriate criminal and protective services

Emergency department evaluation of child abuse.

Child abuse presents commonly to emergency departments. Emergency providers are confronted with medical, social, and legal dilemmas with each case. A solid understanding of the definitions and risk factors of victims and perpetrators aids in identifying abuse cases. Forensic examination should be performed only after the child is medically stable. Emergency providers are mandatory reporters of a reasonable suspicion of abuse. The role of the emergency provider is to identify abuse, facilitate a thorough investigation, treat medical needs, protect the patient, provide an unbiased medical consul

Child abuse inflicted by small children.

In cases of child abuse, usually, the parents are initial suspects. A common explanation of the parents is that the injuries were caused by a sibling. Child-on-child violence is reported to be very rare in children less than 5 years of age, and thorough investigation by the police, child protective services, and medicolegal examinations are needed to proof or disproof the parents' statement. We report two cases of physical abuse of infants by small children.

Child sexual abuse: consequences and implications.

Sexual abuse is a problem of epidemic proportions in the United States. Given the sheer numbers of sexually abused children, it is vital for pediatric nurse practitioners to understand both short-term and long-term consequences of sexual abuse. Understanding consequences of sexual abuse can assist the pediatric nurse practitioner in anticipating the physical and mental health needs of patients and also may assist in the identification of sexual abuse victims. Sexual abuse typically does not occur in isolation. Implications for practice will be discussed.

Child sexual abuse: an Italian perspective.

The problem of child sexual abuse is a growing reality in Italy. The experience of over 200 children seen by the SVS (Soccorso Violenza Sessuale) Centre in Milan (the first Italian large-scale study) may give more information on the European situation. This study is a retrospective study based on information contained in the files of children beneath the age of 14 seen at the SVS Centre between May 1996 and May 2003, who arrived with a suspicion of child sexual abuse. Over 80% of all cases fell within the normal-aspecific category according to Adams' 2001 classification. This first Italian sur

Neurobehavioral sequelae of child sexual abuse.

This study examined intellectual and memory functioning in a sample of sexually abused children compared to demographically and age-matched controls. The severity of abuse and other pertinent factors were also examined in relation to cognitive performance. Elevated levels of psychopathology were present in the abused children, as well as diminished performance on tasks influenced by attention/concentration. However, after controlling for differences in IQ and socioeconomic status (SES), significant differences in memory function were not found. Results are discussed in the context of stress ef

Medical aspects of child sexual abuse.

In the 1980s the extent of child sexual abuse became widely recognized. Most doctors were ill equipped to deal with this 'new' form of abuse. Unlike physical abuse most sexually abused children have no physical signs. The close cooperation between professionals is a legacy of the experience of the last decade.
