Nutritional care for patients with Crohn's disease.

Protein calorie malnutrition, in addition to deficits of other nutrients, occurs frequently among patients with Crohn's disease. In most instances the mechanisms by which these deficits occur are multifactorial. The most appropriate method to assess nutritional status includes a nutritionally oriented medical history and physical examination combined with a measurement of serum albumin. If the gut can be used safely, enteral nutrition is the preferred feeding method for Crohn's disease patients needing nutritional support. The advantages of enteral nutrition are stimulatory effects on gastroin

Efficacy of ready-to-use therapeutic food in malnourished children with cancer: Results of a randomized, open-label phase 3 trial.

BACKGROUND: The adverse influence of undernutrition in children with cancer may be remediated by early nutritional intervention. This study assessed the efficacy of ready-to-use therapeutic food (RUTF) in improving nutritional status and reducing treatment-related toxicities (TRTs) in such children. METHODS: In a randomized controlled phase-3 open-label trial, severely and moderately undernourished children with cancer were randomized 1:1 to receive standard nutritional therapy (SNT) or SNT+RUTF for 6 weeks. The primary outcome (weight gain >10%) and secondary outcomes (improved/maintained nu

Nutritional status of individuals with autism spectrum disorders: do we know enough?

The incidence of individuals with autism spectrum disorders (ASDs) is on the rise; therefore, well-timed screening is important. Given that this is a nutritionally vulnerable population, it is imperative to conduct a detailed nutritional assessment so that timely and intensive interventions can be recommended. This review article summarizes the research, focusing on the nutritional status of individuals with ASDs based on their anthropometric measurements, biomarkers, and dietary assessments. Research examining anthropometric measurements reveals an abnormally accelerated rate of growth among

Persistent left superior vena cava discovered during central line insertion in a patient with Joubert syndrome: a case report.

BACKGROUND: Persistent left superior vena cava is the most common thoracic venous anomaly. It is usually asymptomatic and discovered incidentally during diagnostic or therapeutic interventions. It can complicate central line insertion, causing arrythmias and thromboembolic complications. Joubert syndrome and related disorders are a group of rare congenital disorders associated with developmental abnormalities and multiple organ system involvement including renal, hepatic, and neurological manifestations. CASE PRESENTATION: We report a case of a 12-year-old Kuwaiti Arab male child with Joubert

Children with Poor Linear Growth Are at Risk for Repeated Relapse to Wasting after Recovery from Moderate Acute Malnutrition.

BACKGROUND: Nutrition programs frequently approach wasting and stunting as 2 separate conditions with distinct causes and effects. Although several cross-sectional studies have identified an association between the 2 conditions, longitudinal studies are useful to quantify the risk of acute malnutrition based on the trajectory of linear growth. OBJECTIVE: We analyzed data from a longitudinal study to explore associations between linear growth and relapse to acute malnutrition in high-risk children during the year after recovery from moderate acute malnutrition (MAM). METHODS: This was a secon

A Cross-Sectional Study of Pediatric Feeding Disorder in Children with Cystic Fibrosis.

BACKGROUND: The exact prevalence of feeding problems in children with cystic fibrosis (CF) is unknown. Pediatric feeding disorder (PFD) encompasses poor oral intake with associated medical, nutrition, psychosocial, or feeding skill dysfunction. We hypothesized that PFD is common in CF and aimed to categorize feeding dysfunction across various domains in children with CF. METHODS: An observational cross-sectional study was conducted in children with CF. Data collected included anthropometrics, nutrition data (including need for tube feeding/enteral nutrition [EN] or high-energy beverages, diet

Indigenous newborn care.

Infant mortality and morbidity disparities occur between non-Indigenous and Indigenous populations of Australia, Canada, New Zealand, and the United States. Neonatal mortality is due to high-risk births, which vary according to prevalence of the maternal risk factors of smoking, alcohol consumption, infection, and disorders of nutritional status, whereas postneonatal mortality is predominantly influenced by environmental factors. Aside from changing socioeconomic conditions, a continuum of maternal and child health care is likely to be the most effective measure in reducing these health dispar

Clinical Dietitians' Perspectives on Nutritional Management in Children's Palliative Care: A Qualitative Study.

BACKGROUND: Feeding disorders and gastrointestinal problems are among the most prevalent concerns in children who are eligible for palliative care. Many of these children require medical nutrition therapy and benefit from the involvement of healthcare professionals with expertise in nutrition, such as dietitians. Dietitians may help alleviate the practical and emotional burden parents experience when caring for their sick child at home. However, research is limited regarding dietitians' roles and contributions in children's palliative care. AIM: This study explores dietitians' experiences and

Influence of complex childhood diseases on variation in growth and skeletal development.

The study of human growth and skeletal development by human biologists is framed by the larger theoretical concerns regarding the underpinnings of population variation and human evolution. This unique perspective is directly relevant to the assessment of child health and well-being at the individual and group level, as well as the construction of growth charts. Environmental, behavioral (nutrition and physical activity), and disease-related factors can prevent attainment of full genetic potential for growth. Undernutrition is most often the cause of growth faltering and poor skeletal developme

Pediatric floating-harbor syndrome: clinical features and treatment outcomes in a cohort of Chinese children.

UNLABELLED: Floating-Harbor syndrome (FHS) is a rare disorder characterized by facial dysmorphism, short stature, and delayed language development. We evaluated the clinical features and treatment outcomes of 10 Chinese children with FHS who received recombinant human growth hormone (rhGH) therapy or nutritional intervention. We retrospectively extracted the clinical features, height standard deviation score (SDS), genetic characteristics, and treatment outcomes from the medical records of 10 Chinese children with FHS. The treatment response was classified as good, moderate, or poor based on a

The impact of environmental pollution on the quality of mother's milk.

Breastfeeding is a gold standard of neonate nutrition because human milk contains a lot of essential compounds crucial for proper development of a child. However, milk is also a biofluid which can contain environmental pollution, which can have effects on immune system and consequently on the various body organs. Polychlorinated biphenyls are organic pollutants which have been detected in human milk. They have lipophilic properties, so they can penetrate to fatty milk and ultimately to neonate digestive track. Another problem of interest is the presence in milk of heavy metals-arsenic, lead, c

A rare case of colorectal duplication with rectourethral fistula in a child: diagnostic challenges and surgical management from Northwest Ethiopia.

BACKGROUND: Alimentary tract duplications are rare congenital anomalies. Colorectal duplication is particularly uncommon and may present with nonspecific features, posing diagnostic challenges. An association with rectourethral fistula is extremely rare. CASE PRESENTATION: A 2-year-old male child presented with chronic constipation, abdominal distension, failure to thrive, and recurrent urinary tract infections. The later development of pneumaturia and fecaluria suggested an enterourinary fistula. Contrast enema demonstrated a reversed rectosigmoid ratio without a clear transition zone, initi

Continuous renal replacement therapy in combination with oXiris haemofilter in a paediatric patient with sodium valproate-induced acute pancreatitis.

Some clinical trials have validated a new perspective on continuous renal replacement therapy (CRRT) in combination with oXiris haemofilter to more effectively enhance endotoxins clearance and circulating cytokines adsorption, stabilise vital signs, correct haemodynamic disorders and accelerate the recovery of organ function, thus providing survival benefits. However, no such combination mode has been reported in paediatric patients with sodium valproate (SV)-induced acute pancreatitis (AP).An early childhood boy was admitted to the department of critical care medicine, due to sudden upper abd

Association of low vitamin B12 levels with depressive and schizophrenia spectrum disorders in child and adolescent psychiatric inpatients.

Folate and vitamin B12 are associated with neurodevelopment and neurotransmitter synthesis and insufficiencies of these nutrients could be linked to psychiatric disorders in children and adolescents. To assess serum levels of folate and B12 in child and adolescent psychiatric inpatients and examine possible links between these levels and different psychiatric disorders. Child and adolescent psychiatric inpatients admitted in a general hospital during a 3-year period were included for analysis. Folate and B12 levels were measured when the subjects were admitted. Psychiatric diagnoses were made

[Problems posed by vitamin D in early childhood].

More and more functions related to vitamin D and more pathologies related to its deficiency are known. The deficiency that exists in vitamin D is known at all ages, sexes and throughout the world. But beyond the existing deficiencies in each population group, in this article we intend to analyze how the nutritional situation of this vitamin in pregnant women and during lactation can have influence on the future health of their offspring. Vitamin D deficiency during pregnancy can be associated with maternal (preeclampsia, gestational diabetes, premature birth), fetal and neonatal complications

Nutritional Deficiencies in Children with Celiac Disease Resulting from a Gluten-Free Diet: A Systematic Review.

BACKGROUND: A strictly gluten-free diet (GFD) is the basis for managing celiac disease (CD). Numerous studies have reported nutritional deficiencies/imbalances ascribable to a GFD. The aim of this review is to describe nutritional deficiencies observed in children with celiac disease on a GFD, to discuss the clinical consequences related to these nutritional imbalances, and to identify strategies that may be adopted to treat them. METHODS: We reviewed the MEDLINE and EMBASE databases between January 1998 and January 2019. RESULTS: Children are, regardless of whether they are on a gluten-free

Current trends in pediatric total parenteral nutrition.

When a pediatric patient is unable to meet his/her caloric needs, TPN is indicated. This therapy in the pediatric patient, as in adults, must promote anabolism, but also must provide for normal growth and development. In these patients, nutritional support may be considered an emergency due to their limited body stores of essential nutrients. Included in this manuscript are the historical evolution of pediatric TPN, indications for therapy, components of the solution, potential complications, monitoring guidelines, and nursing care. Furthermore, it will be shown how a multidisciplinary approac
