Personal experience in the observation of 177 patients suffering from acute cholecystitis and treated in the five-year period 1981-1985 is reported. In particular, problems related to surgical treatment are pointed out.

A retrospective investigation was conducted to give an account in this paper of clinical diagnoses, intra-operative findings, and therapeutic results obtained from 453 patients who had received surgical treatment for acute cholecystitis, between 1975 and 1986. The benefits of the approach may be seen from the overall lethality which had been as low as 4.2 per cent. Conservative treatment was found to be indicated only for high-risk cases.

Thirty cases of acute cholecystitis in pregnancy were identified during a 12-year period. Twenty-one patients were successfully managed with medical therapy alone. Nine underwent surgical intervention, four after failing medical therapy and five as the initial approach to therapy. Surgical therapy was complicated by preterm labor and delivery in two patients undergoing surgery during the early third trimester. One patient underwent cholecystectomy during the first trimester and subsequently aborted. There were no serious complications in patients undergoing surgery during the second trimester.

Some controlled clinical trials have demonstrated that early or immediate operation for acute cholecystitis produces better results than delayed operation or conservative treatment. From 1969 to 1983, 397 patients have been operated for acute cholecystitis at the Surgical University Hospital Würzburg, the results are discussed here.

Drugs purchased by a random sample (17 000) of the population of Jämtland county, Sweden, are continuously monitored. Twelve patients developed acute cholecystitis complicating trauma. Acute acalculus cholecystitis was present in 11 patients. Nine patients died. A review of 20 reports comprising 98 patients shows 86.7% had acute acalculus cholelithiasis, and 61.1% had necrosis, gangrene, and/or perforation of the gallbladder. The authors present 7 cases of acute non-lithiasic cholecystitis, representing 1.5% of the total number of acute cholecystitis cases in their personal statistics. The determining factor of the acute inflammatory process of the gallbladder was represented in 6 of the cases by non-lithiasic obstruction of the cystic duct. In the last case there was acute segmental cholecystitis secondary to a gallbladder septum, probably of congenital origin. Clinical and anatomo-pathological aspects are discussed, of acute non-lithiasic cholecystites, the origin of which is located in the diverticular apparatus
