Cholestasis is a reduction in bile flow that occurs from a variety of causes in humans. This produces hepatocellular injury and fibrosis. Considering that there are limited therapies for this disease, there has been interest in understanding the mechanism by which cholestasis produces injury. Studies have demonstrated that oxidative stress occurs in livers of humans with cholestasis. In vitro studies have demonstrated that bile acids kill hepatocytes by a mechanism that depends upon reactive oxygen species (ROS). Further studies, however, have demonstrated that this mechanism is of limited imp…

Cholestasis occurring in infancy should be evaluated completely to exclude hepatic as well as endocrine or metabolic causes. A rapid diagnosis should be made to ensure that hepatic and neurologic complications are prevented. We describe a rare case of infant cholestasis due to panhypopituitarism resulting in hypoglycemia and liver biopsy findings consistent with bile duct paucity. Existing bile ducts were noted to be small, and electron microscopy demonstrated diminutive and atrophied biliary cells with a diminished bile duct lumen size. Hypoglycemia and cholestasis resolved with treatment of …

OBJECTIVE: To report the case of an 85-year-old man with asymptomatic massive hypertriglyceridemia (MHTG).
CLINICAL PRESENTATION AND INTERVENTION: Our case was a non-smoker, healthy 85-year-old Caucasian male, with no excessive alcohol intake and no evidence of an excessive sedentary lifestyle, body mass index = 23.2 kg/m(2), BP = 125/85 mm Hg and plasma triglyceride (TG) >1,000 mg/dl. The MHTG was an incidental finding at the age of 70. He had no cardiovascular disease, xanthomas, xanthelasmas or keratic precipitate. During the last 15 years, his average TG plasma levels showed a significant

One of the most common causes of extrahepatic cholestasis is bile duct obstruction by gallstones, bile duct strictures in chronic pancreatitis involving the head of the pancreas, or tumors in the region of the pancreas, bile ducts or gallbladder. While choledocholithiasis usually gives rise to classical clinical signs (obstructive jaundice, typical pain, and fever in the case of cholangitis), tumors often become symptomatic only when far advanced. In addition to laboratory parameters, diagnostic imaging techniques, in part with a therapeutic intervention option (ERCP), are of central importanc…

Four cows with cholestasis, whose general condition was moderately to severely disturbed, were examined. Two of them had signs of colic, and one had severe pruritus. All of them had jaundice and three had dermatitis solaris. Bilirubinuria occurred in three of the cows and they all had increased concentrations of serum bilirubin and serum bile acids and increased activities of hepatic enzymes. An ultrasonographic examination of the liver and gallbladder revealed that in one cow only the extrahepatic bile ducts were dilated, in another only the intrahepatic bile ducts were dilated and in the oth…

The authors make a review of paediatric-neonatal cholestasis, a troublesome field in clinical and hepatopathology. They focus the main topics and stress the differential aspects in extrahepatic biliary atresia, "neonatal hepatitis", cholestatic syndromes associated with paucity of interlobular biliary ducts and metabolic diseases. The "overlap" areas and differential diagnosis in these chapters are a challenge for the pathologist. The importance of a close collaboration with clinicians is stressed.

Four infants with conjugated hyperbilirubinemia who were brought for treatment primarily because of a hemorrhage are reported. Underlying disorders included extrahepatic biliary atresia, choledochal cysts, and alpha 1-antitrypsin deficiency. Prodromal signs of disturbed coagulation and diminished bile excretion were not recognized. The increased bleeding tendency was probably caused by vitamin K deficiency, resulting from a combination of cholestasis-induced fat malabsorption, absence of vitamin K supplementation after birth, and low vitamin K intake as a result of breast feeding.

Aim of a rational diagnostic approach is keeping the risks and challenges of the patient at a minimum, shortening the time interval till the diagnosis is established and putting the charges to a low level. Anamnesia, clinical examination and laboratory results allow the experienced clinician in about 90% a correct diagnosis if an extrahepatic cholestasis is the case in question. On the other hand the result is only 75-80% if there is an intrahepatic cholestasis. Non dilated bile ducts do not exclude an extrahepatic cholestasis and additional diagnostic measures like ERCP or/and PTC are recomme…

The infant with cholestasis presents a series of diagnostic challenges which require careful delineation. Correct diagnosis, permitting selection of appropriate management and therapy, can be made in the vast majority of patients. When the diagnosis remains obscure, exploratory surgery to distinguish intrahepatic from extrahepatic cholestasis should be performed by 60 days of age in order to provide the best possible outcome. Nevertheless, in both intrahepatic and extrahepatic cholestatic disorders, the characteristics of the original insult determine the prognosis, which may often be predicte…

With China's outstanding achievements in the prevention and treatment of hepatitis, hereditary cholestasis caused by genetic variants has gradually become an important cause of death or transplantation in children with liver disease. The continuous identification of new pathogenic genes expands the disease spectrum and clinician's understanding of disease. The disease characteristics and clinical manifestations of hereditary cholestasis caused by different gene variants vary, and the severity of diseases caused by the same gene variants and the response to treatment are also significantly diff…

Experimental bile duct ligation (BDL) in rodents causes cholestatic liver injury characterized by structural and functional alterations that … nal alterations induced in this model are similar to that in humans suffering from diverse forms of cholestasis including primary biliary cirrhosis (PBC) or primary sclerosing cholangitis (PSC). Therefore, this extrahepatic biliary obstruction model is used in many laboratories worldwide. Nevertheless, BDL can result in significant variations and high mortali …

Intrahepatic cholestasis of pregnancy (ICP) is the most common liver disease during pregnancy, characterized by otherwise unexplained pruritus in late second and third trimester of pregnancy and elevated bile acids and/or transaminases. ICP is associated with an increased risk of adverse perinatal outcomes for the fetus and the later development of hepatobiliary disease for the mother. Bile acids should be monitored throughout pregnancy since fetal risk is increased at serum bile acids >40 µmol/l. Management of ICP consists of treatment with ursodeoxycholic acid, which reduces pruritus. Early …

INTRODUCTION: The causes of intrahepatic cholestasis include cholestatic viral hepatitis, primary biliary cirrhosis, benign recurrent cholestasis, primary sclerosing cholangitis and sepsis. During sepsis, proinflammatory cytokines and nitric oxide cause cholestasis by impairing hepatocellular and ductal bile formation.
CASE OUTLINE: We report a 48-year-old woman who was admitted to hospital due to malaise, jaundice, fever and pain in the neck. Physical examination revealed jaundice, tachycardia (pulse rate was 120/min), hypotension 90/60 mm Hg. Laboratory findings showed normocytic normochrom…

Intrahepatic cholestasis of pregnancy is a rare disorder, predominately occurring during the third trimester of pregnancy and characterized by pruritus, jaundice, and biochemical disturbances in liver enzymes. While intrahepatic cholestasis of pregnancy poses little maternal risk, there is significant risk to the fetus such as preterm delivery, non-reassuring fetal status, meconium staining, and perinatal mortality. Current evidence proposes susceptibility to derangements in the sulfation of steroid compounds, affecting the metabolism of progesterone and bile acids in the fetal/placental compa…
