Placentae or uteri from pregnant rats (days 12-21) contained no detectable alpha-subunit of the glycoprotein hormones (CG, TSH, FSH, and LH) when assayed in either a rat or human alpha-RIA. To determine whether ectopic secretion of a protein hormone can occur normally, we studied plasma from normal, nonpregnant subjects for the presence of a placental hormone, human chorionic gonadotropin. We extracted and purified this hormone from other plasma proteins. We identified the hormone in the final residue on the basis of its dose-response curves in a specific radioimmunoassay and calculated the plasma concentration after correction for losses. 191 patients with signs of threatened abortion were assessed by means of ultrasound (Kretz-Technik, Vidoson, Siemens) and semiquantitative assay of urinary HCG-excretion. Sonar examination was able to establish an accurate diagnosis of fetal life (heart action and/or movements) in all pregnancies showing a gestation sac suitable to the 11th week. The remainder cases required a repeated investigation including HCG-levels and growth rate of the gestational sac. Thus the diagnosis of blighted ovum could be made within one or occassionally two weeks.

OBJECTIVES: To provide clarity on the use of qualitative and quantitative human chorionic gonadotropin (hCG) tests for the assessment of pregnancy.
METHODS: A case scenario and a brief review of the relevant literature describing clinical and analytical considerations regarding hCG testing are presented.

We present a 9-year-old boy with central precocious puberty and hydrocephalus caused by a human chorionic gonadotropin (beta-hCG) secreting pineal tumor. High levels of beta-hCG in the serum and cerebrospinal fluid were observed in this patient. The patient received radiotherapy and chemotherapy without surgical intervention.
