Barriers to artificial intelligence applications in ophthalmology: beyond diagnostic accuracy toward patient outcomes, workflow integration, ethics, and trust.

Artificial intelligence (AI) has become a major area of ophthalmic innovation, with a rapidly expanding literature, yet routine clinical adoption remains limited. Diagnostic accuracy alone is an insufficient marker of clinical value because even technically strong systems may fail to improve referral, follow-up, treatment, clinician workload, or health-system value. This focused narrative review examines the barriers that separate ophthalmic AI performance from real-world implementation. We reviewed ophthalmic AI literature indexed in PubMed, Web of Science, and Google Scholar, with emphasis o

Event-based safety governance and the systematic neglect of functional decline: a case for rebalancing quality measurement in the care of older adults.

Falls prevention is a cornerstone of quality improvement in the care of older adults, yet the metrics through which it is operationalised warrant scrutiny. Current quality frameworks in acute and post-acute settings overwhelmingly treat falls as discrete sentinel events while treating functional decline as a background condition unworthy of equivalent measurement. This commentary identifies this asymmetry as a distinct governance problem, which we term event-based safety governance: the systematic privileging of countable adverse events over longitudinal harms in quality oversight. In this con

Responsible and innovative AI for mental health care: five priority themes.

Artificial intelligence (AI) has entered psychiatry at scale, yet its clinical impact remains constrained by a sizable gap between technical validation and real-world implementation. The central barriers are no longer computational, but infrastructural: unreliable measurement systems, incomplete governance frameworks, and insufficient standards for clinical evidence and integration. This paper synthesizes insights from a 2026 American College of Neuropsychopharmacology (ACNP) study group examining how to responsibly translate AI into clinical mental health care. Building on this perspective, w

From principles to binding rules: A one health governance readiness analysis of Saudi Arabia's AI health policy against global regulatory benchmarks.

BACKGROUND: AI is entering clinical care, disease surveillance, and health-data systems faster than governance frameworks can accommodate. Stakes for public health systems include algorithmic safety in diagnosis and triage, data interoperability, and workforce capacity to supervise AI tools safely. Saudi Arabia has built substantial AI health governance infrastructure through its Data and Artificial Intelligence Authority and Food and Drug Authority, but no binding, AI-specific health law had been enacted as of mid-2026. METHODS: A ten-dimension governance scoring framework anchored in WHO an

Ethical governance of artificial intelligence in digital health for Indigenous populations: A narrative review and conceptual framework.

Artificial intelligence (AI) integration into digital health care delivery is rapidly expanding and is increasingly viewed as a promising approach for improving access to care, strengthening diagnostic decision-making, and addressing persistent health inequities experienced by Indigenous populations. Although AI-enabled digital health applications offer potential benefits such as scalability, personalization, and improved clinical decision support, they also raise critical ethical concerns related to equitable implementation, culturally grounded design, representative data usage accountability

Bridging Worlds: Can Evidence, Regulation and Indigenous Leadership Shape the Future of Traditional, Complementary and Integrative Medicine?

BACKGROUND AND NEED: As the WHO Global Traditional Medicine Strategy 2025-2034 encourages countries to consider integrating traditional, complementary and integrative medicine (TCIM) into health systems, there is a need to clarify the principal scientific, regulatory, ethical, and governance challenges involved. Renewed global attention to TCIM, including at the Second Global Summit on Traditional Medicine held in New Delhi in December 2025, has intensified calls for approaches that are safe, effective, equitable, and culturally respectful. OBJECTIVE: This commentary examines the principal ch

Surgery 4.0: From the Smart Operating Room to the Learning Operating Room.

The connected operating room captures, transmits, and displays data, but it does not learn. This perspective presents Chirurgie 4.0 (C40), a French-led initiative born within the Commission Innovation of the Académie nationale de chirurgie, which proposes not only a concept but a method for the safe adoption of artificial intelligence (AI) in surgery. At its core is the C40 Maturity Model of the Operating Room, a human-governed "surgical world model" describing the transition from the Smart OR to the Learning OR across six levels, from the conventional operating room to a sovereign, federated

[The Digital Gym in the neuromotor rehabilitation field].

The digitization of processes has become a necessity to make health management remain sustainable. Current organizational models do not enable the increasing efficiency in process governance required by the increasing demand for rehabilitation services. Vice versa, new technologies have an innovative potential for compensation of disability and for operations management. This is context in which the concept of a "comprehensive digital rehabilitation" also said "digital gym" is developed: as a governance system for the whole inpatient rehabilitation path. The author describes the fundamentals o

Regulating algorithmic tools in reproductive health: ethical and legal challenges.

Artificial intelligence and algorithmic tools are increasingly integrated into reproductive healthcare, including fertility tracking applications, diagnostic systems, and clinical decision support tools. While these technologies may support improved access, personalisation, and decision making, their expansion is also associated with significant ethical and governance challenges in a domain shaped by legal risk, social norms, and deeply personal health decisions. These challenges include limited transparency, which may constrain informed consent and autonomy; risks of bias arising from unrepre

Heritable Genome Editing in a Global Context: National and International Policy Challenges.

A central problem for the international governance of heritable germline gene editing is that there are important differences in attitudes and values as well as ethical and health care considerations around the world. These differences are reflected in a complicated and diverse regulatory landscape. Several publications have discussed whether reproductive uses would be legally permissible in individual countries and whether clinical applications could emerge in the context of regulatory gaps and gray areas. Systematic comparative studies that explore issues related to the governance of this te

How strong is the administrative backbone of ethical governance? A descriptive cross-sectional study of health research ethics committees in Tanzania.

BACKGROUND: Health research ethics committees (HRECs) provide the administrative backbone of ethical governance by safeguarding the rights, safety and well-being of research participants and ensuring the ethical conduct of health research. Effective ethical oversight depends not only on rigorous protocol review but also on robust governance, competent personnel, adequate infrastructure, sustainable institutional support and efficient administrative systems. Despite their critical role, evidence on the administrative functionality of HRECs in Tanzania remains limited. OBJECTIVE: To assess the

Training for departure? Policy, professional portability, and the fragile future of Nepal's nursing workforce.

Workforce migration is often cast as a matter of personal choice or national shortage. This policy and practice review contends that the prevailing framing inadequately captures the dynamics of nursing mobility throughout the workforce lifecycle. It emphasises that this mobility is influenced by various stages, including education, early professional socialisation, active practice, and international migration. These stages ultimately contribute to systemic challenges by resulting in structural losses that affect the overall workforce. Using Nepal as a case study with wider relevance for low- a

From validation trials to clinical reality: implementing AI-supported prostate MRI pathways in the NHS.

Prostate cancer remains the most common cancer diagnosed in men in the UK. The adoption of pre-biopsy multiparametric MRI (mpMRI) has improved diagnostic stratification, although it has concurrently increased the burden on radiology services. Artificial intelligence (AI) systems for analysing prostate MRIs have demonstrated significant potential in validation tests. Integrating these systems into the NHS will be exceedingly challenging because of concerns around governance, safety, and workflow. This article provides a perspective on AI-assisted prostate MRI and draws on practical experience f

Harmonizing spatially fractionated radiation therapy: consensus and roadmap from an ESTRO workshop.

BACKGROUND AND PURPOSE: Spatially fractionated radiation therapy (SFRT) intentionally delivers nonuniform dose distributions characterized by alternating high-dose peaks and low-dose valleys, with therapeutic potential for complex, inoperable bulky tumors. Although clinical adoption is expanding, clinical implementation and practices remain heterogeneous across institutions. This consensus aims to identify system-level barriers to harmonized clinical implementation of SFRT and formulate consensus-based recommendations to enable reproducible multi-institutional integration. METHODS: The 2025 E

Refined Exclusion in Medical AI: Data Justice and Patient Safety Governance.

Medical AI is often evaluated using aggregate measures of discrimination, calibration, and accuracy. However, these measures can obscure clinically important variation across patient groups, institutions, devices, and workflows. This viewpoint defines refined exclusion as a governance condition in which an AI system appears successful in aggregate, while uncertainty, error, or reduced clinical reliability is concentrated in populations that are insufficiently represented, measured, validated, or monitored. The concept does not replace algorithmic fairness, hidden stratification, dataset shift,
