Ultrasound anatomy in the neonatal clubfoot.

Our objective was to assess clubfoot anatomy by US in newborn babies before and in the early phase of treatment. Reproducible US projections and measurements were carried out on 30 untreated clubfeet in 22 children, consecutively included in the study. The position of the navicular in relation to the head of the talus was visualised in all feet. The mean distance between the medial malleolus and the navicular was significantly shorter in the clubfeet than in normal feet. There was a tendency to medial displacement of the cuboid. Soft tissue thickness at the medial side of the foot was increase…

A method of assessment of the clubfoot deformity.

The literature on clubfeet is inadequate because a common method language for assessing the deformity is lacking. Different severities of clubfoot deformity will give different results for a standard procedure: a less severe deformity can be corrected by limited releases, whereas a severe deformity requires radical procedures. This paper presents a language of assessment that has been used for a number of years. The importance of developing a language of assessment to be able to identify the various types of clubfoot deformity is important if the treatment of this condition is to develop withi…

Radiological score in the assessment of clubfoot.

The clinical appearance and the radiographs of 31 patients with 47 clubfeet taken before the start of treatment have been studied. In spite of making a careful clinical evaluation, 14 feet (29.8%) remained "borderline" and could not be definitely classified into the easy or resistant groups. The radiological score of the foot, which is an objective representation of the severity of the deformities at all important levels and in all planes, has been found to be an accurate and sensitive index which enables such a distinction to be made.

The role of major gene in clubfoot.

The roles of major gene and multifactorial inheritance in the etiology of clubfoot (talipes equinovarus) were studied using Caucasian clubfoot families ascertained in Indiana. The method of analysis used was complex segregation analysis under the mixed model, in which five genetic parameters were examined to test hypotheses on major gene by displacement (t), degree of dominance (d), gene frequency (q), transmission probability (tau2), and multifactorial inheritance by heritability (H). The analysis showed that the segregation pattern of clubfoot in these families is best explained by assuming …

The roentgenographic appearance of the "corrected clubfoot".

Roentgenographic changes associated with a corrected clubfoot may not be readily apparent. In some individuals with minimal residual deformity, these roentgenographic changes are quite subtle. The roentgenographic abnormalities of the corrected clubfoot are described.

Sonographic diagnosis of clubfoot in utero.

The in utero sonographic diagnoses of 13 clubbed feet in eight fetuses are reported, and five representative cases are described in detail. The sonographic findings and clinical features of fetal clubfoot are discussed. The prenatal sonographic detection of clubfoot should alert the sonographer to search for other congenital anomalies associated with syndromes involving clubfoot.

Availability and readability of online patient information on clubfoot: assessment of paediatric hospital clubfoot web pages.

PURPOSE: To determine the availability and readability of online patient information (OPI) provided by paediatric hospitals in the United States using clubfoot as a model condition. METHODS: The websites of the top 95 paediatric hospitals identified using US News & World Report were included. The names of paediatric hospitals and the terms "clubfoot", "clubfeet" and "talipes equinovarus" were entered into the Google search engine. Readability was assessed using five validated metrics and the composite grade level (CGL). The number of unpaid monthly visits was calculated with the Ahrefs Organi…

Difficulties in maintenance of clubfoot abduction brace and solutions - maintenance of clubfoot abduction brace, locks and keys.

OBJECTIVE: To determine the frequency of early relapse after achieving good initial correction in children who were on clubfoot abduction brace. METHODS: The cross-sectional study was conducted at the Jinnah Postgraduate Medical Centre, Karachi, and included parents of children of either gender in the age range of 6 months to 3years with idiopathic clubfoot deformities who had undergone Ponseti treatment between September 2012 and June 2013, and who were on maintenance brace when the data was collected from December 2013 to March 2014. Parents of patients with follow-up duration in brace less…

Interventions for congenital talipes equinovarus (clubfoot).

BACKGROUND: Congenital talipes equinovarus (CTEV), also known as clubfoot, is a common congenital orthopaedic condition characterised by an excessively turned-in foot (equinovarus) and high medial longitudinal arch (cavus). If left untreated it can result in long-term disability, deformity and pain. Interventions can be conservative (such as splinting or stretching) or surgical. Different treatments might be effective at different stages: at birth (initial presentation); when initial treatment does not work (resistant presentation); when the initial treatment works but the clubfoot returns (re…

Analytical radiography of clubfoot after tenotomy.

UNLABELLED: The study was undertaken to determine the radiographic impact of the tenotomy after the Ponseti method. We report a retrospective and prospective radiographic analysis of 37 clubfeet after the Achilles tenotomy. The talocalcaneal angle was measured on lateral and anteroposterior views; the tibiotalar and the tibiocalcaneal angles were evaluated on the lateral view. The mean age at the time the last radiographic analysis was performed was 19 months (range, 13-35 months). Both studies have the same conclusion: the Achilles tenotomy decreases the lateral tibiocalcaneal angle and incre

Correcting residual deformity following clubfoot releases.

UNLABELLED: There are many possible pitfalls of clubfoot releases and it is important to recognize the problems and provide proper timely treatment. Late residual deformity following clubfoot releases include: dynamic or stiff supination and forefoot adduction deformities, intoeing gait, overcorrection, rotatory dorsal subluxation of the navicular, vascular insult to the talus with collapse, and dorsal bunion. We reviewed 134 clubfeet in 95 children who had primary clubfoot releases between 1988 and 1991. In general, the patients who underwent surgery before 6 months of age had poorer results …

The adult clubfoot (congenital pes cavus).

Clubfoot occurs in approximately 1 of every 1000 live births, with multiple theories proposed regarding the cause. The pathologic anatomy of the adult clubfoot consists of four components (cavus, adductus, varus, and equinus) and the specific soft tissue and bony procedures indicated for correction depend completely on the constellation of residual deformity that may exist. If the patient was successfully treated at a younger age and has only limited deformity, all that may be required is symptomatic treatment or bracing. Flexible deformities may be treated with a combination of soft tissue pr…

[Early functional treatment of congenital clubfoot].

The concept of conservative clubfoot treatment beginning in the neonatal period has changed considerably in the last 2 decades. The rate of clubfeet requiring surgery (20%) and the extent of surgical interventions have dropped significantly. The early functional Montpellier method has proved to be highly effective for all degrees of deformity. It is used for patients from birth until the age of 1 year. The treatment during the first 3 months of life is decisive and consists of daily physical therapy, use of a continuous passive motion machine, and taping. This method represents an efficient al…
