Diameter of the cochlear nerve in deaf humans: implications for cochlear implantation.

Although the parameters that are most important for postoperative speech perception in cochlear implantation have not been identified, it is assumed that the numbers of remaining cochlear neurons and spiral ganglion cells in the implanted deaf ears are critical. In this study, we evaluated the correlation of the maximum diameter of the cochlear and vestibular nerve trunks with the number of spiral ganglion cells in horizontal sections of the temporal bone of 42 patients who were profoundly deaf during life, and in 5 patients with normal hearing. The maximum diameters of the cochlear, vestibula…

European consensus statements on pediatric cochlear implantation.

Existing European national guidelines agree to provide cochlear implantation to children with bilateral severe to profound hearing loss and to offer bilateral treatment depending on the hearing status of the best hearing ear and progression of hearing loss, and in the case of congenital hearing loss to do this at the youngest possible age. However, studies and reports demonstrate substantial and persistent disparities in the provision of pediatric cochlear implantation across Europe. Variations in national policies, service quality, and access to timely diagnosis and intervention continue to a…

Cross-modal functional plasticity after cochlear implantation.

Despite evidence that cross-modal effects after hearing loss and cochlear implantation are primarily driven by synaptic gain and efficacy, few studies have evaluated cross-modal functional connectivity (CMFC) to assess plasticity. This study, inspired by the psychophysiological interaction (PPI) method, addresses its limitations and provides a robust approach for assessing task-induced CMFC. Twenty-three postlingually deafened cochlear implant (CI) recipients and 17 normal-hearing (NH) participants took part in the study. Functional near-infrared spectroscopy was used to measure brain activity…

Bilateral cochlear implantation in infantile Bartter's syndrome.

Bartter's syndrome (BS) is a rare group of hereditary salt losing tubulopathies due to impairment of renal transport mechanism. Herein, we report a boy in early childhood who was diagnosed to have infantile Bartter's syndrome. Laboratory investigations revealed hypokalaemia and metabolic alkalosis which was managed with oral potassium chloride supplementation. The inner ear showed no anatomical abnormalities. Sequential cochlear implantation was performed 4 months apart. Postoperative electrical stimulation yielded good response and a symmetric mapping. The patient was on regular audio-verbal …

Cochlear Implantation After Intralabyrinthine Schwannoma Microsurgical Resection.

Cochlear implantation after microsurgical resection of intralabyrinthine schwannomas has shown promising results in a limited number of small studies with safe and favorable outcomes. Cochlear implantation is a mainstream treatment for advanced bilateral sensorineural hearing loss and, more recently, has been applied to patients with single-sided deafness. The case presented is a 66-year-old gentleman with a history of progressive right-sided sensorineural hearing loss and bothersome tinnitus with a right intralabyrinthine schwannoma involving the semicircular canals, vestibule, and basal turn …

Residual hearing preservation for cochlear implantation surgery.

Cochlear implantation (CI) has developed for more than four decades. Initially, CI was used for profound bilateral hearing impairment. However, the indications for CI have expanded in recent years to include children with symptomatic partial deafness. Therefore, CI strategies to preserve residual hearing are important for both patients and otologists. The loss of residual low-frequency hearing is thought to be the result of many factors. All surgical methods have the same goal: protect the delicate intracochlear structures and preserve residual low-frequency hearing to improve speech perceptio…

Cochlear Implantation After Radiotherapy of Vestibular Schwannomas.

BACKGROUND AND OBJECTIVES: Vestibular schwannomas (VS) frequently lead to ipsilateral sensorineural hearing loss (HL) as part of its natural history or as a result of treatment. Cochlear implantation represents a well-documented treatment of profound HL that cannot be treated adequately with a conventional hearing aid, thus being offered to selected VS patients. A functional cochlea and cochlear nerve are prerequisites for sound perception with a cochlear implant (CI). The potential impact of radiotherapy on these structures is thus an important issue for subsequent CI hearing outcomes. The ob…

[Update cochlear-implantation: indications and surgical aspects].

In childhood, inadequately rehabilitated hearing loss leads to impaired language acquisition and social, mental and emotional development. In adults, social withdrawal due to limited communication skills is often a consequence of unsatisfactory hearing rehabilitation. Therefore, in patients with profound hearing loss, the indication for cochlear implantation should be considered. Technical advances in cochlear implant development, as well as in microsurgical techniques and the rehabilitation process, have led to an expansion of indications in recent years. Adequate hearing rehabilitation is as…

Cochlear Implantation in Siblings With Refsum's Disease.

OBJECTIVES: Whether the origin of severe hearing loss in Refsum's syndrome is caused by cochlear impairment or retrocochlear degeneration remains unclear. This case report aims to investigate hearing performance before and after cochlear implantation to shed light on this question. Also, identification of new mutations causing Refsum's syndrome would be helpful in generating additional means of diagnosis. METHODS: A family of 4 individuals was subjected to genetic testing. Two siblings (56 and 61 years old) suffered from severe hearing and vision loss and received bilateral cochlear implants.…

Endoscopic-Assisted Cochlear Implantation: A Case Series.

The aim of the present study was to describe the use of the endoscopic-assisted cochlear implantation in cases with an unsuccessful standard surgical technique because of not achieving adequate exposure to the round window (RW). Three patients with a bilateral profound hearing loss were operated using an endoscopic-assisted cochlear implant procedure at our tertiary university referral center between 2012 and 2014. In all of the patients, a retroauricular "c" shaped incision was performed and a subperiosteal pocket was created. Standard cortical mastoidectomy and posterior tympanotomy were acc…
