Hem-o-lok clip migration to the common bile duct after laparoscopic common bile duct exploration: A case report.

BACKGROUND: Laparoscopic cholecystectomy (LC) and laparoscopic common bile duct exploration (LCBDE) has been widely used for management of gallbladder and common bile duct (CBD) stones. Post-operative clip migration is a rare complication of laparoscopic biliary surgery, which can serve as a nidus for stone formation and cause recurrent cholangitis. CASE SUMMARY: A 59-year-old female was admitted to hospital because of fever and acute right upper abdominal pain. She has a history of LC and had a LCBDE surgery 2 mo ago. Physical examination revealed tenderness in the upper quadrant of right ab

Laparoscopic transcystic common bile duct exploration (LTCBDE): a safer method to treat common bile duct stones than laparoscopic choledochotomy.

We read the article published in the Spanish Journal of Gastroenterology and its accompanying editorial about the laparoscopic treatment of both cholecystolithiasis and choledocholithiasis in a single stage procedure. We would like to make some comments. Common bile duct (CBD) stones can lead to serious complications such as cholangitis and pancreatitis. So far, there is no standard surgical treatment for choledocholithiasis. Although, there are some ways to deal with CBD stones. Each method has its own advantages and disadvantages. In the era of advancements in minimally invasive technology,

Long-term follow-up of common bile duct diameter after endoscopic sphincterotomy in patients with common bile duct stones.

BACKGROUND AND AIMS: To determine the time to normalization of common bile duct (CBD) diameter after endoscopic sphincterotomy and stone extraction in patients with choledocholithiasis. METHODS: Patients with CBD dilation due to choledocholithiasis were enrolled. CBD diameter was measured by transabdominal ultrasonography before, and repeated after one, three, six and twelve months after endoscopic sphincterotomy and stone extraction, until normalization of CBD diameter. RESULTS: Of 115 cases enrolled over a 36-month period, CBD diameter reversed to normal in 71 (61.7%) patients after one mo

Experience with robotic common bile duct exploration for common bile duct stones in comparison to a conventional laparoscopic approach.

INTRODUCTION: Common bile duct (CBD) stones are present in up to 20% of patients with gallstones and are associated with complications such as obstructive jaundice, cholangitis, and pancreatitis. Current guidelines support both endoscopic and surgical CBD stone extraction. While laparoscopic common bile duct exploration is an established technique, evidence on robotic CBD exploration remains limited. METHODS: A retrospective cohort study was conducted, including all adult patients who underwent CBD exploration between January 2020 and December 2024. Demographic, operative, and outcome data we

Common bile duct and Oddi sphincter pressure before and after endoscopic papillotomy in patients with common bile duct stones.

Pressures in the Oddi sphincter and common bile duct were determined before, immediately after and several weeks after endoscopic papillotomy. Fifteen patients were included in the study. Significant decreases in Oddi sphincter and common bile duct pressures were found and the decrease in Oddi sphincter pressure was to some extent dependent upon the length of the papillotomy.

[Adenomyoma of the distal common bile duct. An uncommon cause of stenosis of the extrahepatic bile duct].

We report the case of a 68-year-old man with adenomyoma of the distal common bile duct, fortuitously discovered on a surgical specimen of a Whipple procedure. To our knowledge, three similar cases have been reported; all patients were operated on with the diagnosis of malignancy: adenomyoma was always discovered on the surgical specimen. Adenomyomas are rare tumors and their origin is discussed. Adenomyoma of the distal common hile duct should be considered as enteropancreatic heterotopia.

Laparoscopic transcystic bile duct exploration: the treatment of first choice for common bile duct stones.

BACKGROUND: This study was designed to explore the role of transcystic bile duct exploration (TCE) as a first line of treatment for patients with suspected or incidental common bile duct (CBD) stones. METHODS: A prospective, case-control study of clinically comparable groups of patients who underwent laparoscopic cholecystectomy (LC) alone (n = 1,854) and combined LC/TCE for CBD stones (n = 253) under the care of one surgeon was performed. Other than ultrasonography, no routine preoperative imaging was used; however, we performed routine intraoperative cholangiography on all patients. RESULT

SpyGlass-guided laser lithotripsy versus laparoscopic common bile duct exploration for large common bile duct stones: a non-inferiority trial.

BACKGROUND: Laparoscopic common bile duct exploration (LCBDE) is the first choice of treatment for large common bile duct (CBD) stones. Recently, single-operator cholangioscopy (SpyGlass system) has been introduced widely in referral and large medical centers. Several studies have reported favorable results for treatment of large CBD stones guided by SpyGlass. We evaluated the clinical efficacy and safety of SpyGlass-guided laser lithotripsy LCBDE for treatment of large CBD stones. METHODS: From August 2015 to August 2018, 157 patients with large bile duct stones who met the inclusion criteri

Alterations of the Bile Microbiome in Recurrent Common Bile Duct Stone.

OBJECTIVE: Common bile duct stone (CBDS) recurrence is associated with bile microbial structure. This study explored the structure of bile microbiome in patients with recurrent CBDS, and its relationship with the recurrence of CBDS. METHODS: Patients with recurrent CBDS (recurrence group) and controls without CBDS (control group) requiring endoscopic retrograde cholangiopancreatography (ERCP) were prospectively included. The control group was noncholelithiasis patients, mainly including benign and malignant biliary stenosis. Bile samples were collected, and bile microbiome structure was analy

Bile acids trigger cholemic nephropathy in common bile-duct-ligated mice.

UNLABELLED: Tubular epithelial injury represents an underestimated but important cause of renal dysfunction in patients with cholestasis and advanced liver disease, but the underlying mechanisms are unclear. To address the hypothesis that accumulation and excessive alternative urinary elimination of potentially toxic bile acids (BAs) may contribute to kidney injury in cholestasis, we established a mouse model for detailed in vivo time course as well as treatment studies. Three-day common bile duct ligation (CBDL) induced renal tubular epithelial injury predominantly at the level of aquaporin 2

Double common bile duct malformation with choledocholithiasis treated by laparoscopic common bile duct exploration with primary suture: a case report.

Double common bile duct (DCBD) represents an exceedingly rare congenital anomaly of the biliary anatomy. Recently, a 46-year-old female patient diagnosed with DCBD was admitted to our hospital, with her condition further complicated by the presence of common bile duct stones. Preoperative imaging studies indicated a Type II classification. The patient underwent a laparoscopic exploration of the common bile duct for the purpose of stone extraction, followed by primary suture closure. The patient was discharged following a successful recovery.

A Prospective Comparative Study of Laparoscopic Common Bile Duct Exploration and Endoscopic Retrograde Cholangiopancreatography for Managing Common Bile Duct Calculi.

INTRODUCTION: Common bile duct (CBD) calculi are typically managed by either laparoscopic CBD exploration (LCBDE) or endoscopic retrograde cholangiopancreatography (ERCP). The superiority of one method over the other remains a matter of debate. This study was designed to compare the efficacy and morbidity profile of LCBDE and ERCP in the management of CBD calculi. MATERIALS AND METHODS: This analytical cross-sectional study was conducted from January 2021 to July 2024. A total of 72 patients with CBD calculi were included, with 36 undergoing LCBDE and 36 undergoing preoperative ERCP followed

Comparing Stone Recurrence Following Surgical Common Bile Duct Exploration or Endoscopic Stone Extraction for Patients with Common Bile Duct Stones.

Background: Laparoscopic common bile duct exploration (LCBDE) and endoscopic retrograde cholangiopancreatography (ERCP) are two methods of retrieving common bile duct (CBD) stones. Aspects of CBD stone management, such as sphincterotomy, have been implicated as risks for CBD stone recurrence although evidence is weak. The aim of this study was to compare stone recurrence following LCBDE and/or ERCP. Methods: Data were collected retrospectively for patients undergoing LCBDE and/or ERCP for CBD stones at a single center from 2008 to 2018. Primary outcome was stone recurrence (>6 months after duc

Transduodenal exploration of the common bile duct in patients with nondilated ducts.

Exploration of the small common bile duct can be technically difficult and is associated with a significant risk of ductal injury or late stricture, or both. Transduodenal common duct exploration after sphincteroplasty (TCDE/S) is an alternative method of duct exploration that avoids choledochotomy. Cholecystectomy followed by TCDE/S was performed upon 28 patients with nondilated ducts and suspected choledocholithiasis. Common duct stones were retrieved in 17 patients. Failure to retrieve stones in the remaining 11 patients was attributed to either false-positive results of cholangiography, fo
