COVID-19 highlights the policy barriers and complexities of postpartum sterilization.

Multiple barriers exist to sterilization in the postpartum period. One such barrier, the Medicaid Title XIX sterilization policy, requires publicly insured patients to complete a sterilization consent form at least 30 days prior to their scheduled procedure. While this policy was set in place in the 1970s to address the practice of coerced sterilization among marginalized women, it has served as a significant barrier to obtaining the procedure in the contemporary period. The COVID-19 pandemic has highlighted specific complexities surrounding postpartum sterilization and created additional barr

A three-dimensional printed model in preoperative consent for ventricular septal defect repair.

BACKGROUND: The 3D printing technology in congenital cardiac surgery has been widely utilized to improve patients' understanding of their disease. However, there has been no randomized controlled study on its usefulness in surgical consent for congenital heart disease repair. METHODS: A randomized controlled study was performed during consent process in which guardians of candidates for ventricular septal defect repair were given detailed explanation of the anatomy, indication for surgery and potential complication and risks using 3D print ventricular septal defect model (n = 20) versus a con

Practices of Informed Consent for Emergency Procedures at a Tertiary Care Hospital in Lahore, Pakistan.

Background The purpose of obtaining informed consent is to ensure that patients undergoing any medical or surgical intervention are neither deceived nor coerced. Accurately estimating surgical risks is critical for shared decision-making and informed consent. Probable complications and alternative procedures should be presented to the patient so that they can freely choose an operative option. However, this factor is difficult to carry on in emergencies where an urgent decision is required. Objective This study aimed to assess the ongoing clinical practices of informed consent in emergency sur

Written consent for laparoscopic tubal occlusion and medico-legal implications.

OBJECTIVE: To analyse the completeness of written consent for laparoscopic tubal occlusion and to consider the medico-legal implications of incomplete written consent. METHODS: A retrospective review was undertaken of the medical records of all women who had laparoscopic tubal occlusion in 2006 in a district general hospital to elicit details of risks of the procedure as recorded on the consent form. The extent of documentation of risks and complications that were cited in the guidelines was analysed. In addition, the grade of doctor and the timing of obtaining written consent were studied.

Comprehension of written texts for the assessment of clinical competence and decision making in people with mild to moderate Alzheimer disease.

BACKGROUND: Clinical competence is the term used to describe an individual's capacity to express a choice regarding their participation in clinical procedures or experimental studies. Understanding the information provided is a prerequisite but consent forms are often lengthy and complicated. Alzheimer's disease patients may be vulnerable in written comprehension, due to cognitive deficits, but unfortunately to date, a specific evaluation of this ability is not included in periodical assessments. METHODS: One hundred thirty Italian patients with Alzheimer's disease were compared with 130 cont

Improving the Completeness of Surgical Consent Documentation Through a Closed-Loop Quality Improvement Audit at a Secondary Care Teaching Hospital.

Background Incomplete surgical consent documentation undermines informed decision-making, patient safety, and medico-legal protection. Despite clear ethical and professional standards, deficiencies in consent documentation remain common, particularly in resource-limited settings. Objective To assess the completeness of surgical consent documentation, implement targeted quality improvement interventions, and evaluate their impact through a closed-loop clinical audit. Methods A closed-loop quality improvement audit was conducted at Almanagil Teaching Hospital, Sudan. Two audit cycles were perfor

Grammar of 'understanding' in informed consent: a Wittgensteinian diagnosis.

Ethical accounts of informed consent require that patients understand the risks, benefits and alternatives of a proposed intervention. Recent philosophical work has examined what content must be understood and how much understanding is sufficient, but a prior question remains unresolved: what kind of concept 'understanding' is in this context. In current institutional practice, understanding is treated as a private mental state produced by receiving information. On this reception model, the consent process is organised around information transfer, and understanding is presumed to follow from a

Attitudes of patients and family members towards deferred and waived consent in ECPR research, an ancillary study of the INCEPTION trial.

BACKGROUND: In emergency settings, obtaining timely informed consent is not always feasible, making deferred and waived consent a potential solution. Despite its frequent use in high-risk research, the experiences and opinions of patients and (bereaved) relatives have been scarcely investigated. This study examined their attitudes towards enrolment in the INCEPTION-trial (NCT03101787) on extracorporeal cardiopulmonary resuscitation (ECPR). METHODS: Questionnaires were sent to survivors and (bereaved) relatives who had signed consent forms for follow-up research in the initial INCEPTION-trial.

Research involving patients. Summary and recommendations of a report of the Royal College of Physicians.

The careful study of disease as it occurs in patients, and the equally careful scrutiny of the effects of treatment, are an indispensable part of the continuing process of improving the efficiency of diagnosis and the effectiveness of treatment. Most patients realise that experience gained from their own case may contribute to their personal benefit or to that of society and, if asked, readily agree to take part in research into their condition. Accordingly, a patient who willingly participates in research has the status of a volunteer similar to that of a healthy person so that some of the qu

Consent for organ donation.

Improving the consent rate for solid organ donation from deceased donors is a key component of strategies in the UK and other countries to increase the availability of organs for transplantation. In the UK, the law is currently clear on what forms consent may take, with the views of the individual expressed previously in life taking priority. Such views may have been expressed prospectively, via membership of the Organ Donor Register or by talking to family members. The factors determining such actions include both positive altruistic motives and negative psychological responses. Studies have

[Living will, durable power of attorney and legal guardianship in the trauma surgery routine : Data from a geriatric trauma center].

BACKGROUND: Due to the increasing number of elderly patients, trauma surgeons are often confronted with end-of-life treatment decisions. Advance directives can help reduce the lack of clarity in those situations. OBJECTIVES: The aim of this study was to identify the presence of living wills, durable power of attorney, legal guardianship and appointment of guardianship in the geriatric trauma center of a university hospital. MATERIALS AND METHODS: The data of all patients treated in our geriatric trauma center from 01/01/2013 to 03/31/2014 were analyzed regarding the presence of a living will

Informed consent: is it more than a formality? a quality improvement project in surgical practice.

Informed consent is a fundamental element of clinical practice. A well-structured and understandable consent form not only upholds patients' rights but also protects clinicians against medicolegal risks. This quality improvement project aimed to identify deficiencies in the consenting process for surgical procedures and implement changes to enhance patient care. The project was conducted in the Ear Nose Throat Department at Holy Family Hospital, Rawalpindi, a large tertiary care centre serving a predominantly low socioeconomic population in Pakistan. A total of 80 patients were included across

Is knowledge of medicolegal issues concordant with heightened risk among doctors?

Spurred by the wave of consumerism, patients do not hesitate to drag the doctor to the court. It has now become essential for doctors to educate themselves on the laws relevant to medical practice. This study addresses some of the issues. Doctors in private practice, belonging to different specialities, were given a self administered questionnaire which was then analysed by the percentage of responses to each question. Only 3% of the respondents faced medicolegal problems at the time of the study. All of them were fighting charges of negligence from their patients. Most (97%) did not keep reco

Patients' understanding of "informed consent" in plastic surgery.

OBJECTIVE: To assess the patient's understanding of the informed consent form before and after plastic surgery. METHODS: This was a prospective analytical descriptive study that utilized a questionnaire on informed consent before and after plastic surgery procedures. RESULTS: Comprehension of informed consent was higher before surgery than after surgery (p=0.016; question 15). The higher the scholarity, the higher the comprehension (s=0.151; p=0.045) before surgery (question 4). For the other questions, it was not possible to find a difference in the pattern of understanding and in the assoc
