One hand is better than two: conversion from pure laparoscopic to the hand-assisted approach during difficult nephrectomy.

INTRODUCTION: The incidence of conversion from a laparoscopic to an open approach during nephrectomy is reported at 6-8%. Conversion to an open procedure may be necessary to control haemorrhage or allow progress in dissection but the well established benefits of minimally invasive surgery (MIS) are obviously lost. Hand-assisted laparoscopy (HAL) also offers the benefits to the patient of MIS. We have used HAL to convert from the pure laparoscopic approach during difficult nephrectomies, rather than converting to traditional open surgery. MATERIALS AND METHODS: A review of our prospective data

Open Conversion with Explantation of Stent Grafts After Endovascular Aneurysm Repair for Abdominal Aortic Aneurysm.

BACKGROUND: Although endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA) is widely used worldwide, the fact that it is associated with increased rates of reintervention has been considered a problem. This study aimed to analyze the outcomes of primary open AAA repair and open conversion with explantation of stent grafts after EVAR. METHODS: In this retrospective study, we enrolled 1,120 patients (open repair, n = 664; EVAR, n = 456) who underwent AAA repair at Kobe University from 1999 to 2019. Of the 664 patients who underwent open repair, 121 (patients who underwent prim

Tailored surgery for large adrenal tumors: the minimally invasive to open (hybrid) approach.

Conversion from minimally invasive techniques to open surgery has mostly been considered as an undesirable event associated with intraoperative complications or poor preoperative planning. However, the impact of conversion to open surgery during adrenalectomy for large adrenal tumours remains unclear. This study investigates the outcomes of conversion from minimally invasive to open surgery for adrenal masses ≥ 60 mm with an additional focus on the identification of survival predictors in patients with large adrenocortical carcinoma (ACC). We retrospectively analyzed 97 patients who underwent …

A delayed conversion may increase the risk of major complications in acute cholecystitis approached laparoscopically. A retrospective comparative cohort study.

AIM: Aim of this study was to evaluate whether timing of laparoscopy lasting longer than two hours before converting to open surgery can worsen the postoperative course during laparoscopic cholecystectomy (LC) for acute cholecystitis (AC). MATERIALS AND METHODS: Medical records of 1,161 patients who underwent urgent laparoscopic cholecystectomy for AC during the period 2001-2017 were retrospectively analyzed. A conversion to open surgery was performed in 70 (6%) patients. Among these, two groups of patients were identified: group 1 (n=51; 73%) included patients who underwent laparotomy within…

Pre-operative prediction of difficult laparoscopic cholecystectomy using clinical and ultrasonographic parameters.

BACKGROUND: Pre-operative prediction of a difficult laparoscopic cholecystectomy (LC) can help the patient as well as the surgeon prepare better for the intra-operative risk and the risk of conversion to open cholecystectomy. METHODS: In 105 eligible patients who underwent LC during May 2001 to January 2003, patient characteristics, clinical history, laboratory data, ultrasonography results and intra-operative details were prospectively analyzed to determine predictors of difficult LC. RESULTS: Of 105 patients, 12 (11.4%) required conversion to open cholecystectomy. Significant predictors of

Predictive Factors for Conversion From Single Incision Appendectomy to Multiport Appendectomy for Pediatric Acute Appendicitis: A Single-Center, Retrospective Cohort Study.

INTRODUCTION: Single-incision appendectomy (SIA), including laparoscopic and laparoscopic-assisted techniques, offers superior cosmetic outcomes and is increasingly applied to both uncomplicated and complicated cases of acute appendicitis. In our practice, transumbilical laparoscopic-assisted appendectomy is the initial approach for all patients with acute appendicitis. This retrospective cohort study aimed to identify risk factors associated with conversion from SIA to multiport appendectomy (MPA). METHODS: We retrospectively reviewed medical records of patients aged < 15 years who underwent

Predictors and reasons for conversion of laparoscopic to open cholecystectomy: a five-year cohort study.

BACKGROUND: Conversion from laparoscopic to open cholecystectomy (LC to OC) is an important surgical outcome that reflects procedural complexity and patient risk. While conversion predictors have been studied internationally, context-specific data from Iraq remain limited. OBJECTIVES: To determine the conversion rate in an Iraqi cohort, identify independent preoperative predictors, develop a simplified risk score, and systematically categorize intraoperative conversion reasons. METHODS: A retrospective cohort study was conducted on 945 consecutive patients who underwent LC between December 2

Laparoscopic right hemicolectomy: short- and long-term outcomes of intracorporeal versus extracorporeal anastomosis.

BACKGROUND: The use of laparoscopy for right hemicolectomy has gained popularity allowing the option of a totally laparoscopic intracorporeal anastomosis (IA) for intestinal reconstruction. This technique may alleviate some of the technical limitations that a surgeon faces with a laparoscopic-assisted extracorporeal anastomosis (EA). METHODS: A retrospective chart review of 195 consecutive patients who underwent laparoscopic right hemicolectomy by four colorectal surgeons at three institutions from March 2005 to June 2014 was performed. Multivariate regression analysis was used to compare pos

What necessitates the conversion to open cholecystectomy? A retrospective analysis of 5164 consecutive laparoscopic operations.

OBJECTIVE: Laparoscopic cholecystectomy (LC) has become the gold standard for the surgical treatment of gallbladder disease, but conversion to open cholecystectomy is still inevitable in certain cases. Knowledge of the rate and impact of the underlying reasons for conversion could help surgeons during preoperative assessment and improve the informed consent of patients. We decided to review the rate and causes of conversion from laparoscopic to open cholecystectomy. METHOD: This study included all laparoscopic cholecystectomies due to gallstone disease undertaken from May 1999 to June 2010. T
