A New Approach - Home Waking Salivary Cortisone to Screen for Adrenal Insufficiency.

Adrenal insufficiency is a common and potentially life-threatening endocrine disorder that can be drug induced or endogenous and of adrenal (primary) or pituitary/hypothalamic (secondary/tertiary) origin.1,2 Of particular concern in drug-induced disease is the patient with glucocorticoid- or opioid-induced adrenal insufficiency. Adrenal insufficiency of any cause is typically diagnosed biochemically with a subnormal morning serum cortisol (the circadian, awakening peak) and serum dehydroepiandrosterone sulfate, followed by or simultaneously with an assessment of the acute (30 and 60 minutes) s

Hormonal responses to a stress load and state anxiety, mood, tiredness, and recovery in Portuguese police cadets.

INTRODUCTION: Police officers are repeatedly exposed to conditions and incidents unique to their work. OBJECTIVES: To describe the hormonal response to a stress load (circuit fitness test) in police cadets, and to verify the association of hormone responses (cortisol and testosterone) with state anxiety, mood, tiredness, and recovery. METHODS: This is an analytical, observational, cross-sectional study with a quantitative approach. A total of 31 police cadets (all male; age 21.0 ± 4.4 years) were evaluated on February 4, 2022, at Instituto Superior de Ciências Policiais e Segurança Interna,

Salivary cortisol as predictor for depression characteristics and remission in electroconvulsive therapy in older persons.

Objectives: Electroconvulsive therapy (ECT) is the most effective treatment for depression; however, consensus on predictors for ECT outcome is lacking. We aim to examine the relation between pre-ECT salivary cortisol values and clinical characteristics and ECT outcome in depressed, older persons. Methods: A total of 102 inpatients meeting DSM-IV criteria for depression and referred for ECT were selected. Salivary cortisol was assessed at five time points during the day, providing insight into the cortisol awakening curve to the ground (AUCg) and to the increase (AUCi) and evening cortisol leve

Identification of biopsychological trait markers in functional neurological disorders.

Stress is a well-known risk factor to develop a functional neurological disorder, a frequent neuropsychiatric medical condition in which pat … remains to be elucidated. We set out to study the hypothalamic-pituitary-adrenal axis analysing the cortisol awakening response and diurnal baseline cortisol in 86 patients with mixed functional neurological symptoms compared to 76 healthy controls. We then examined the association between cortisol regulation and the severity and duration of traumatic life events. Finally, w …

Stress-Hormone Dynamics and Working Memory in Healthy Women Who Use Oral Contraceptives Versus Non-Users.

BACKGROUND: Women who use oral contraceptives (OCs) may have a higher risk of developing a depression, which is associated with both vulnerability to stress and cognitive dysfunction. OCs disrupt the hypothalamic-pituitary-gonadal (HPG) axis by suppressing endogenous sex steroid production including estradiol. The HPG axis and the hypothalamic-pituitary-adrenal (HPA) axis are known to interact, possibly through modulations driven by estradiol. OCs may affect HPA regulation capacity, i.e., disturb cortisol dynamics such as the cortisol awakening response (CAR), and influence cognition such as w…

Biological stress, self-rated stress, anxiety and depression in women with takotsubo syndrome.

AIMS: Takotsubo syndrome is a condition associated with stress, anxiety and depression. Biological stress in takotsubo syndrome is scarcely described in the literature. The aim of this study was to investigate if biological and self-rated stress, anxiety and depression in women with takotsubo syndrome reflect a long-term state of psychological burden, with the hypothesis that saliva cortisol is increased six months after the acute event. METHOD AND RESULT: Forty-seven women were diagnosed with takotsubo syndrome in a diagnostic cross-sectional study, of those 29 had saliva cortisol measured at

Dissociable hormonal profiles for psychopathology and stress in anorexia and bulimia nervosa.

BACKGROUND: Anorexia nervosa (AN) and bulimia nervosa (BN) are complex psychiatric conditions, in which both psychological and metabolic factors have been implicated. Critically, the experience of stress can precipitate loss-of-control eating in both conditions, suggesting an interplay between mental state and metabolic signaling. However, associations between psychological states, symptoms and metabolic processes in AN and BN have not been examined. METHODS: Eighty-five women (n = 22 AN binge/purge subtype, n = 33 BN, n = 30 controls) underwent remote salivary cortisol sampling and a 2-day,

How to Deal With Temporal Relationships Between Biopsychosocial Variables: A Practical Guide to Time Series Analysis.

OBJECTIVE: Longitudinal data allow for conclusions about the temporal order of events and interactional dynamics between several processes. The aim of this article is to provide a concise and pragmatic description of time series analyses (TSAs) of patient samples with numerous (or daily) repeated biological, behavioral, or psychological measurements. In addition, the article demonstrates how to implement the described analyses with the software R. METHODS: To illustrate the concrete application of the time series method, we use two case series of patients with anorexia nervosa. Upon awakening

Cortisol, cortisone and inflammation in relation to burnout symptoms.

Burnout is highly prevalent in high-demand healthcare professions, with medical students and trainees already at increased risk. The pathophysiological processes associated with burnout remain insufficiently understood as previous research is mostly limited to cross-sectional studies focusing on specific biological correlates. This study aims to investigate a wide range of biological correlates associated with burnout symptoms by exploring cortisol, cortisone, and multiple inflammatory markers in medical students and physicians-in-training. A three-year open cohort study was carried out in med

Emotional Processing Coping Methods and Biomarkers of Stress in Young Adult Testicular Cancer Survivors.

Coping through emotional processing (EP) with cancer-related circumstances can take several forms, including methods thought to be constructive (e.g., planning, meaning making) and unconstructive (e.g., rumination). These forms can have differential relationships with experiences of stress. Associations of coping through constructive and unconstructive EP in expressive writing with salivary stress biomarkers were examined among young adult testicular cancer survivors. Constructive processing was significantly associated with less overall daily cortisol output and smaller salivary alpha-amylase

Endocrine stress responses in TH1-mediated chronic inflammatory skin disease (psoriasis vulgaris)--do they parallel stress-induced endocrine changes in TH2-mediated inflammatory dermatoses (atopic dermatitis)?

In previous research we reported attenuated responsiveness of the hypothalamus-pituitary-adrenal (HPA) axis and further, an increased reactivity of the sympathetic adrenomedullary (SAM) system to stress in patients suffering from atopic dermatitis (AD). AD is a chronic inflammatory skin disease mainly triggered by TH(2)-dependent inflammatory processes. The specific goal of the present study was to investigate whether altered HPA axis and SAM system responsiveness to stress can also be found in TH(1)-mediated inflammatory conditions. Patients with psoriasis (PSO; n=23), a TH(1)-mediated inflam

Effect of family presence on stress response after bypass surgery.

BACKGROUND: Coronary artery bypass grafting (CABG) is a major surgery that may cause severe surgical stress response (SR). Although the presence of family members in intensive care unit (ICU) is known to benefit intensive care patients socially and emotionally, its effects on surgical SR are unknown. OBJECTIVES: To investigate the effect of an informed family member (IFM)'s presence in the awakening process in ICU on patients' SR after CABG. METHODS: A nonrandomized controlled clinical study was completed with a total of 73 patients: 37 patients in the control (CG) and 36 in the intervention
