Out-of-Pocket Costs for Advanced Imaging Across the US Private Insurance Marketplace.

PURPOSE: The aim of this study was to characterize out-of-pocket patient costs for advanced imaging across the US private insurance marketplace. METHODS: Using the 2017 CMS Health Insurance Marketplace Benefits and Cost Sharing Public Use File, which details coverage policies for qualified health plans on federally facilitated marketplaces, measures of out-of-pocket costs for advanced imaging and other essential health benefits were analyzed for all 18,429 plans. RESULTS: Independent of deductibles, 48.0% of plans required coinsurance (percentage fees) for advanced imaging, 9.7% required cop…

PKRxiv: A Best Practice Model for Advancing Pharmacoequity Through Open Pharmacokinetic Data Sharing.

Model-informed drug development is increasingly integrated across the drug development continuum, enabling more efficient, cost-effective, and targeted trials while reducing reliance on animal studies. Achieving pharmacoequity requires not only equitable access to medicines but also to the data and knowledge that inform drug development and regulatory decisions. To address challenges in pharmacokinetic data sharing, PKRxiv (https://pkrxiv.org/) was developed as a discipline-specific repository designed around Findable, Accessible, Interoperable, Reusable (FAIR) principles. This tutorial introd

A Retrospective, Single-Center Comparative Cost Analysis of OnabotulinumtoxinA and AbobotulinumtoxinA for Cervical Dystonia Treatment.

BACKGROUND: Chemodenervation with botulinum neurotoxin (BoNT) is recommended as first-line treatment for the management of cervical dystonia. The choice of BoNT for treatment is subject to the consideration of several factors, including cost. OBJECTIVE: To compare the costs incurred by patients and payers for onabotulinumtoxinA (ONA) or abobotulinumtoxinA (ABO) for the treatment of cervical dystonia. METHODS: We conducted a retrospective, noninterventional closed cohort study of cervical dystonia patients within a single U.S. private neurological practice. Patient and payer incurred costs fr

It's the cost that counts! Young children's reciprocity is sensitive to subjective cost.

Three experiments investigated whether reciprocity in children aged 4-7 (N = 192; 53.12% female; 65.63% White) is sensitive to the cost of a gift from the benefactor's perspective. In the main study, 6- and 7-year-olds, but not 4- and 5-year-olds, preferred to reward a benefactor who valued a gift highly over one who gave an identical gift but valued it less. A follow up study found that 5- but not 4-year-olds at least expected object valuation to impact sharing. In the third study, 6- and 7-year-olds even rewarded a benefactor whose gift they themselves (the children) valued less (than anothe

The costs of treating and not treating patients with chronic myeloid leukemia with tyrosine kinase inhibitors among Medicare patients in the United States.

BACKGROUND: Patients with high cost-sharing of tyrosine kinase inhibitors (TKIs) experience delays in treatment for chronic myeloid leukemia (CML). To the authors' knowledge, the clinical outcomes among and costs for patients not receiving TKIs are not well defined. METHODS: Using the Surveillance, Epidemiology, and End Results (SEER)-Medicare database, the authors evaluated differences in TKI initiation, health care use, cost, and survival among patients with CML with continuous Medicare Parts A and B and Part D coverage who were diagnosed between 2007 and 2015. RESULTS: A total of 941 pati

Sharing meals: promising nutritional interventions for primary health care including nursing students and elderly people.

BACKGROUND: The risk of malnutrition among elderly people is high and living alone increases the risk. As the number of older persons living alone is expected to increase due to the demographic development of an increasing older population, more knowledge about low-cost, sustainable nutritional interventions is needed. The purpose of this study was to investigate how nursing students can be a resource in the nutritional care of older persons living alone by sharing weekly meals. METHODS: Twenty-three nursing students and 23 elderly people who lived alone and received home nursing care partici

Health economic evaluations of genomic newborn screening: Approaches by studies within the international consortium on newborn sequencing.

Affordability and value-for-money are key factors that will inform decisions about implementation of genomic newborn screening (gNBS) as a population-based program. Given the methodological and data-related challenges to evaluating health and economic outcomes of gNBS, there is a need for discussion and knowledge sharing amongst investigators responsible for conducting such evaluations. The International Consortium on Newborn Sequencing (ICoNS) includes academic and commercial gNBS pilot and implementation programs, and the ICoNS Economics Subcommittee assembles health economists who are invol

Best practices in supplier relationship management and response when supply is disrupted by cyber attack : An incident response framework.

This paper explores the growing dependency of organisations on suppliers and the importance of supplier relationship management (SRM) in achieving sustainable competitive advantage. It highlights the various reasons organisations engage with suppliers, including accessing specialised expertise, cost savings, flexibility, risk mitigation and improved quality. The paper emphasises the need for organisations to adopt best practices in SRM to enhance their resilience to disruptions, particularly those caused by cyber attacks. It introduces a threat assessment process for organisations to evaluate

Cost-effectiveness of Financial Incentives for Patients and Physicians to Manage Low-Density Lipoprotein Cholesterol Levels.

IMPORTANCE: Financial incentives shared between physicians and patients were shown to significantly reduce low-density lipoprotein cholesterol (LDL-C) levels in a randomized clinical trial, but it is not known whether these health benefits are worth the added incentive and utilization costs required to achieve them. OBJECTIVE: To evaluate the long-term cost-effectiveness of financial incentives on LDL-C level control. DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation, a previously validated microsimulation computer model was parameterized using individual-level data from the ran

Optimization problem of bike-sharing system rebalancing based on dynamic pricing.

Bike-Sharing Systems (BSS) help address first- and last-mile travel challenges. As a limited public resource, the current pricing mechanism fails to fully exploit the service potential of bike-sharing or adequately capture heterogeneous user price sensitivity. We propose a Dynamic pricing-based Stochastic Demand-Response Optimization (DSDRO) method to address this gap. The method balances service fairness under varying demand stickiness while maximizing enterprise resource utilization. The DSDRO framework leverages spatiotemporal patterns in bike-sharing demand data and introduces a dynamic pr

Toward equitable access to cell and gene therapies: rethinking co-payments.

OBJECTIVE: Augmentation implants pose a challenge for radiologists. Displacing the implant allows slightly more breast tissue to be visualized than the standard compression views. The objective of this study was to verify the agreement between craniocaudal (CC) views and CC with implant displacement (CC-ID), mediolateral oblique (MLO) and MLO-ID and the inter- and intraobserver agreement of mammographic images for finding abnormal images. METHODS: The main outcomes [BI-RADS® normal=1,2; abnormal=3,4,5)] were analysed by 3 readers (reader1; reader 2, 1st round, reader 2, 2nd round) in 360 wome

Accelerating Health Data Sharing: A Solution Based on the Internet of Things and Distributed Ledger Technologies.

BACKGROUND: Huge amounts of health-related data are generated every moment with the rapid development of Internet of Things (IoT) and wearable technologies. These big health data contain great value and can bring benefit to all stakeholders in the health care ecosystem. Currently, most of these data are siloed and fragmented in different health care systems or public and private databases. It prevents the fulfillment of intelligent health care inspired by these big data. Security and privacy concerns and the lack of ensured authenticity trails of data bring even more obstacles to health data s

A comparative study of the role of disease severity in drug reimbursement decision making in four European countries.

Considerations beyond cost-effectiveness are important in reimbursement decision making. We assessed the importance of disease severity in drug reimbursement decision making in Belgium, France, The Netherlands and Sweden. We investigated scientific literature and policy documents and conducted three interviews in each country (four in The Netherlands) with persons involved in drug reimbursement. Disease severity is an important consideration, especially where the level is high. The Netherlands operationalizes disease severity using the proportional shortfall approach. Sweden uses categories to

The Budget Impact of Increased Use of Febuxostat in the Management of Gout: A US Health Plan Managed Care Pharmacy and Medical Costs Perspective.

PURPOSE: Gout is a chronic disease characterized by the deposition of urate crystals in the joints and throughout the body, caused by an excess burden of serum uric acid (sUA). The study estimates pharmacy and medical cost budgetary impacts of wider adoption by US payers of febuxostat, a urate-lowering therapy (ULT) for the treatment of gout. METHODS: A US payer-perspective budget impact model followed ULT patients from a 1,000,000-member plan over 3 years. The current market share scenario, febuxostat (6%) and ULT allopurinol (94%), was compared with an 18% febuxostat market share. Data were

From Breakouts to Bargains: Strategies for Patient-Centered, Cost-effective Acne Care.

A range of treatment options are available for both mild to moderate and moderate to severe acne, and these options vary widely in their clinical uses, effectiveness, and costs. With the continued rise of dermatologic drug prices and increased cost-sharing due to high-deductible health plans, the importance of cost-effective treatment continues to grow. Failure to consider cost-effective, patient-centered care may lead to increased financial toxicity, reduced adherence, and ultimately worse outcomes and patient satisfaction. Combination topical products offer improved efficacy and convenience,

Across six societies children engage in costly third-party punishment of unfair sharing.

Third-party punishment of unfairness shows striking cross-societal variation in adults, yet we know little about where and when in development this variation starts to emerge. When do children across societies begin to pay a cost to prevent unfair sharing? We present an experimental study of third-party punishment of unfair sharing across N = 535 children aged 5-15 from communities in six diverse countries: Canada, India, Peru, Uganda, USA, and Vanuatu. We tested whether children were more likely to punish equal or selfish (maximally unequal) distributions between two absent peers. We also tes
