A 7-year-old girl developed epiphora, recurrent purulent conjunctivitis, and dacryocystitis 32 months after the repair of a traumatic orbital floor fracture. Dacryocystography and surgical exploration revealed that migration of the orbital floor implant had caused obstruction of the nasolacrimal duct at the sac. Migration of the implant was probably due to a failure to anchor the implant properly to bone.

Orbital cyst from lacrimal sac epithelium is rare and thought to originate from diverticulum of the lacrimal sac. A comparative study was made of endonasal dacryorhinostomy using either a 0 degree or 30 degrees rigid endoscope or a Zeiss operating microscope in 25 patients with chronic epiphora and complete stenosis of the nasolacrimal duct confirmed by dacryocystography. The success rate in both groups was similar (80%). Computed tomography is useful in post-traumatic cases.

A series of patients with obstructive epiphora was evaluated by a new method of dacryocystography. The technique involved intubation and injection of radiopaque contrast material into each lacrimal system under the control of direct sequential digitally subtracted (realtime) imaging. Simultaneous rapid sequence digital subtraction macrodacryocystography yields instantaneous high resolution magnified bone-free radiographs of the lacrimal drainage system.

We established a chronic dacryocystitis model by injecting of 0.05, 0.1, and 0.15 ml self-curing resin via the lacrimal punctum in rabbits. Lacrimal dacryoscintigraphy facilitates definite diagnosis of obstructions and stenosis of the lacrimal drainage system with little stress to the patient. Together with x-ray dacryocystography it is an important diagnostic tool especially for pre- and postoperative evaluation of the drainage apparatus. Dacryoscintigraphy is the best method for measuring the dynamics of tear drainage especially in the canaliculi. Although it is not a substitute for other methods in general use, it complements them and expands their diagnostic accuracy.

PURPOSE: To report a rare cause of epiphora and chronic discharge: lacrimal canalicular diverticulum.
METHODS: Case report.
RESULTS: A distended left lower eyelid canaliculus was associated with chronic epiphora and discharge. A suspected canalicular diverticulum was noted on dacryocystogram.
CONCLUSIONS: Canalicular diverticula should be considered in the differential diagnosis in patients that present with epiphora and discharge with a patent nasolacrimal system. Dacryocystography is diagnostic.

A 73-year-old woman on topical pilocarpine and adrenaline for chronic simple glaucoma for three years presented with a mass in the medial canthus of the right eye. Although dacryocystography showed a dilated and partially obstructed nasolacrimal system, ultrasound examination was able to demonstrate a mass in the nasolacrimal duct. At operation a black dacryolith was found, of the diameter predicted by ultrasound. Histological examination of the dacryolith suggested its derivation from breakdown products of adrenaline.

Previously, the standard procedure in the treatment of patients having had unsuccessful lacrimal surgery has been Jones tube bypass, regardless of the anatomic site of obstruction. However, many of these patients may be treated by a reconstructive approach, namely, the canaliculodacryocystorhinostomy operation. Dacryocystography is helpful in determining which of these patients are amenable to this treatment modality. Success rates are encouraging and eliminate the necessity of a permanent prosthesis and long-term follow-up.

Five primary epithelial lesions of the lacrimal sac representing a range of epithelial tumors are presented. The benign lesions initially presented without pain or significant mass but with epiphora and a bloody discharge. The malignant lesions presented insidiously, were large, firm masses and displayed overlying skin telangiectasis rather than the diffuse erythema of dacryocystography revealed a sac partially obstructed by a subtotal space-occupying mass in benign tumors and completely obstructed with malignant lesions.
