Diarrhoea, vomiting, and dehydration are frequently encountered in neonatal emergency. However, it is challenging to manage resistant hypernatraemia and metabolic acidosis associated with it. Diagnosing the exact cause is even more difficult. Glucose-galactose malabsorption commonly presents with hypernatraemia and repeated dehydration. Thirty-nine of forty neonates with mean dehydration equivalent to 6.7% of body-weight were orally rehydrated with a glucose/electrolyte solution. Only one patient required any intravenous fluids for rehydration. Hypernatraemia and acidosis present at admission were corrected within a few hours without complications. It seems that oral rehydration, is suitable for neonates as well as for children and adults.

The factors controlling aldosterone secretion were measured in 12 patients with moderate to severe dehydration during the first year of life. Secondary hyperaldosteronism was present in all cases (mean plasma aldosterone concentration 414.6 ng/dl), as well as increased plasma cortisol levels (mean 49.7 microgram/dl).
