A new method for applying cryosurgery in depth has been performed in a trial in order to achieve more effectiveness and avoid many of the disadvantages of conventional techniques. A mastery of advancement flap design, selection, and execution greatly aids the surgeon in solving reconstructive dilemmas. Advancement flaps involve carefully planned incisions to most efficiently close a primary defect in a linear vector. Advancement flaps are subcategorized as unipedicle, bipedicle, V-to-Y, and Y-to-V flaps, each with their own advantages and disadvantages. When selecting and designing an advancement flap, the surgeon must account for primary and secondary movement to prevent distortion of important facial structural units and boundaries.

A volar advancement flap based on V-Y concept for fingertip reconstruction is described. The crescent flap utilises curved incision to preserve fingertip contour and distal digital crease. Satisfactory outcome was achieved in two patients who underwent fingertip reconstruction using this technique. Its advantages are technical simplicity, minimal donor morbidity, and may be used in situation where conventional V-Y incision is unsuitable.

BACKGROUND: Notching of the soft triangle is a complication of excisional and some reconstructive surgery that is difficult to repair.
OBJECTIVE: This article provides a simple single-stage procedure to correct the complication of notching of the soft triangle.
METHODS: A detailed description of a turnover flap from the inferior tip covered with a full-thickness skin graft is provided.

In modern orthopedic surgery, there are still indications for the guillotine amputation of the thigh. Immediate postoperative management by effective skin traction can promote rapid healing and eliminate the need for revision surgery. Revision surgery necessitates further shortening of the stump; skin grafting likewise can be avoided as a secondary procedure. Most skin tractions for above-the-knee amputations loosen within 24 hours. The suggested method of applying skin traction on A-K amputations, if applied properly, will be effective for periods ranging from five to seven days.
