Lower limb amputation for diabetic foot.

We amputated 35 limbs of 27 patients with diabetic foot from March 1988 to March 1998. The mean age of the patients at the time of operation was 67 years, and the mean follow-up period was 27 months. Thirteen patients died in the period from 1 day to 39 months after the operation. All patients suffering from diabetic foot were referred to our department for surgical procedures after failure of conservative treatment conducted elsewhere. Their feet were classified into grade 2-3 in 18 limbs, grade 4-5 in 11 limbs, and gangrene of the lower leg and entire foot in 2 limbs, as classified by the Wa…

Dynamic pressures on the diabetic foot.

A retrospective study was conducted in order to investigate the relation between increased plantar pressure and ulcers in the diabetic foot and contribute more information on this subject. Maximal plantar pressure (MPP) is reported on various areas of the plantar aspects of the feet in 328 diabetic patients and 75 healthy subjects. An increase in MPP under all plantar areas except for the heels was found to be related with increasing severity of the diabetes symptoms. There was also a significant decrease in the application of high pressures to the heels, the metatarsal heads II-IV and the toe…

Management of the diabetic Charcot foot.

The diabetic Charcot foot is a major limb-threatening complication of long-term diabetes mellitus and neuropathy. Although first described over 100 years ago, we are still lacking definitive studies regarding its prevalence in this population, precise etiology, or most effective treatments. Trauma in the presence of peripheral sensory neuropathy and abundant arterial perfusion seem to be the primary causal factors leading to this severe foot deformity. Misdiagnosis or delayed diagnosis of osteoarthropathy allows the destructive phase of this disorder to continue with resultant further destruct

[Diabetic foot infections: a preliminary report].

Foot infection is a common cause of hospitalization for diabetic patients. Our study aimed to analyze patients' age, sex, the primary sites of infection, presenting features, outcome, bacteriologic studies and their clinical significance in diabetic foot infections. The age of our diabetic patients with foot infections ranged from 43 to 82 years, with a mean of 62.5 years; fifty-two of them were males and twenty-eight were females. The male to female ratio was 1.9:1, but there was no statistically significant difference between the sexes (p greater than 0.05). The commonest age group was the s…

Diabetic foot problems in tertiary care diabetic clinic in Thailand.

Foot problems in patients with diabetes cause substantial morbidity and may lead to lower extremity amputations. These risks may be reduced … ss, limitation of joint mobility and insensate to monofilament was the most common manifestation of diabetic foot problems in this setting. Most patients were not protected by proper footwear. More effort is needed to educate diabetic patients about foot care and improve their choice and selection of footwear.

Imaging trends in diabetic foot infections.

Diabetic foot infections are a common complication of diabetes, associated with significant risks of amputation, recurrence and high treatment costs. Imaging plays an increasingly important role in determining medical and surgical planning. Advanced imaging such as magnetic resonance imaging (MRI) and hybrid molecular imaging such as single photon emission computed tomography with computed tomography (SPECT/CT) and positron emission tomography with CT (PET/CT), play an important role in the diagnosis of osteomyelitis and in treatment planning. This review explores the state-of-the-art and futu…

Antibiotic Therapy in the Diabetic Foot.

Empiric antimicrobial therapy for diabetic foot infections must be selected with attention to wound severity and patient-specific factors including comorbidities, antibiotic allergies, prior cultures, risk factors for antibiotic-resistant organisms and the local antibiogram. Subsequent therapy can be tailored in spectrum by culture results and its duration informed by surgical approach and pathologic data. After initial clinical improvement, most patients can be transitioned to oral step-down therapy. The duration of treatment for soft tissue infection (STI) is based on the severity of STI and…

Diabetic Foot Ulcer Identification: A Review.

Diabetes is a chronic condition caused by an uncontrolled blood sugar levels in the human body. Its early diagnosis may prevent severe complications such as diabetic foot ulcers (DFUs). A DFU is a critical condition that can lead to the amputation of a diabetic patient's lower limb. The diagnosis of DFU is very complicated for the medical professional as it often goes through several costly and time-consuming clinical procedures. In the age of data deluge, the application of deep learning, machine learning, and computer vision techniques have provided various solutions for assisting clinicians…

Review of Diabetic Foot Ulcers-Reply.

Previous research has shown that observers can make reliable judgments about the relative mass of moving objects that collide in animated displays. One popular explanation of this is that observers' judgments are based on an internal model of Newtonian dynamics. An alternative explanation is that these judgments are based on measurable optical properties that are correlated with relative mass. To better understand this issue, the present investigation reanalyzed the data from three previous studies by Mitko and Fischer (2023), Sanborn et al. (2013), and Todd and Warren (1982), and it replicate

Diagnostic Imaging of Diabetic Foot Disorders.

Plain, weight-bearing radiography is the preferred first-line imaging. Dependent on the suspected pathology, further imaging is indicated. In a soft tissue infection, an abscess has to be excluded, for example, with ultrasound. Osteomyelitis has a typical triad including osteolysis, periosteal reaction, and bone destruction in radiography, but signs are often delayed. MRI is the gold standard for diagnosis of osteomyelitis with high intensity in T2-weighted and STIR images and intermediate to decreased reticulated hazy intensity in T1-weighted images. In comparison, bone marrow edema is also b

Segmentation Approaches for Diabetic Foot Disorders.

Thermography enables non-invasive, accessible, and easily repeated foot temperature measurements for diabetic patients, promoting early detection and regular monitoring protocols, that limit the incidence of disabling conditions associated with diabetic foot disorders. The establishment of this application into standard diabetic care protocols requires to overcome technical issues, particularly the foot sole segmentation. In this work we implemented and evaluated several segmentation approaches which include conventional and Deep Learning methods. Multimodal images, constituted by registered v…

The biomechanics of diabetic foot amputation.

According to the International Diabetes Federation, approximately 463 million adults live with diabetes mellitus (DM), a number projected to increase to 700 million by 2045; a diabetic foot ulcer (DFU) will occur in about 15% of that population. Multiple factors contribute to the development of those wounds including diabetic peripheral neuropathy, biomechanical imbalances, trauma, and peripheral vascular disease. In addition, 85% of all lower limb amputations in patients with diabetes are preceded by a DFU resulting in significant biomechanical challenges for these patients, many of who never…
