The authors report a case of balloon dacryocystoplasty in a patient who had a dilated lacrimal sac found in conventional surgery. A 52-year-old woman presented with persistent epiphora after a failed balloon dacryocystoplasty. She underwent an external dacryocystorhinostomy. The lacrimal sac was found to be dilated at its lower part, adjacent to the nasolacrimal ductus. Flaps were created more superiorly than usual. The operation was unsuccessful after 3 months. Previous balloon dacryocystoplasty, which is reported to be a harmless procedure, may pose some difficulty in subsequent conventional

We evaluated the catheterization results and follow-up echocardiographic data in all (n = 31) patients undergoing balloon dilatation for congenital aortic valve stenosis at Skejby University Hospital from May 1987 to October 1996. Patients were between three weeks and 35 years of age (median 12.1 years). Peak-to-peak systolic pressure gradient was reduced by 59%, from 74 +/- 17 to 30 +/- 20 mmHg (p < 0.0001). Thyomas are benign or malignant lymphoepitelial tumors often associated with parathymic syndromes. A 74-year-old subject developed a recurrent thyoma over 15 year with bronchial dilatations and repeated bronchopulmonary and sinus infections. This clinical presentation suggests Good's syndrome which combines thyoma, bronchial dilatations and hypogammaglobulinemia.

Cystic dilatation of the biliary tract is a rare disorder, more common in children, with a high morbidity and mortality rate. Death may follow one of the possible complications (recurrent cholangitis, biliary cirrhosis, cholangiocarcinoma). The Authors report the case of an asymptomatic adult woman. According to CT scan and ERCP the lesion found was classified as type IVb of Todani's classification. The cyst was removed and a Roux en-Y reconstruction was performed. This procedure is currently associated to a low mortality rate and the best long term results.

The relationship between levels of biliary amylase measured at operation and clinical features was studied in a series of 55 children with congenital biliary dilatation (choledochal cyst) who presented between 1976 and 1993. A wide spectrum of aortic stenosis, results of aortic balloon valvotomy or dilatation will be variable. The author point out the unsatisfactory results with balloon valvotomy in young adults, which are contrary to those obtained in children. The place of this procedure in the palliative therapy of the aortic stenosis is discussed. However we need to develop more objective criteria and optimal timing to evaluate the results of this intervention.

We performed balloon catheter dilatation in four patients with a failed dacryocystorhinostomy. Epiphora was relieved and a patent lacrimal drainage system was achieved after the procedure in three of the four. The fourth patient had a transient partial response. Epiphora recurring in one patient 3 1/2 months after the procedure responded to a second dilatation.

From a series of balloon dilatations of alimentary tract strictures 11 patients with different types of gastric and oesophagogastric anastomotic stenoses are reported. The dilatation of gastric outflow tract obstructions was highly effective in the treatment of retention. In 7 out of 8 cases with gastric outflow stenosis surgery could be entirely avoided and replaced by balloon dilatation. The effect of various therapeutic procedures (surgery, percutaneous transluminal dilatation and antihypertensive drugs) was analyzed in 161 patients with renovascular hypertension. The results show that each of the three methods effectively lowers blood pressure. Our experience with five cases of cystic dilatation of the extrahepatic bile ducts is reported and compared with the literature. The following conclusions have been reached: (1) The etiology of this anomaly is primarily congenital but may involve an acquired component.
