[Acute Diverticulitis - Treatment without Antibiotics?].

In patients with acute uncomplicated diverticulitis current guidelines recommend that antibiotics should not be used reflexively or routinely, but rather selectively: starting with Hinchey stage 1b, i.e. in the case of localized pericolic abscess. Antibiotic-free diverticulitis therapy is therefore generally possible up to Hinchey stage 1a. We recommend carefully evaluating the patient, taking exclusion criteria into account and re-examining the patient after 48-72 hours. Keywords: Diverticulitis, antibiotics, treatment without antibiotics, complications.

Appendiceal diverticulitis diagnosed by ultrasound.

Appendiceal diverticulitis is a rare entity that may present clinically simulating appendicitis. The importance of its diagnosis lies in the higher risk of complications including malignant transformation. The presence of small cystic appendicular peripheral images with thickened wall and hyperechoic surrounding fat are the diagnostic key.

Emergency medicine updates: Acute diverticulitis.

INTRODUCTION: Acute diverticulitis is a condition commonly seen in the emergency department (ED). Therefore, it is important for emergency medicine clinicians to be aware of the current evidence regarding the diagnosis and management of this disease. OBJECTIVE: This paper evaluates key evidence-based updates concerning acute diverticulitis for the emergency clinician. DISCUSSION: Diverticulitis is a complication of diverticulosis and most commonly affects the sigmoid and descending colon in Western countries. History and examination can suggest the diagnosis, with abdominal pain and tenderne…

Expert Commentary on Complicated Diverticulitis.

Complicated diverticulitis encompasses a variety of clinical scenariors as outlined in the well-written and succinct review by Drs Mendez and Garcia-Henriquez. These range from acute presentations, such as abscesses and free perforations with peritonitis, to more chronic, insidious sequelae such as fistulas and strictures. The contemporary surgical management of diverticular disease has evolved considerably with better appreciation of the pathophysiology and natural history. Just as in uncomplicated diverticulitis, there has been a significant shift toward a less aggressive surgical strategy in…

Management of Freely Perforated Diverticulitis.

A healthy 65-year-old woman presents to the emergency department with a 12-hour history of sudden-onset severe lower abdominal pain. This is her first episode. She reports nausea, vomiting, and anorexia. Her last colonoscopy was at age 60, and was normal, except for diverticulosis of the sigmoid colon. Physical examination is significant for fever, tachycardia, and generalized abdominal pain with rebound tenderness. Pertinent laboratory findings include a leukocytosis and metabolic acidosis. A CT scan is obtained and is consistent with freely perforated diverticulitis, including a thickened si…

Epiploic Appendagitis Mimicking Recurrent Diverticulitis.

Here, we report a case in which a patient with an extensive history of diverticulitis of the sigmoid colon presented with left lower quadrant abdominal pain similar to her previous episodes of diverticulitis. An initial diagnosis of diverticulitis was made based on her history and exam, intravenous antibiotics were given, and an elective surgical resection was considered. However, a subsequent CT scan revealed epiploic appendagitis with no evidence of diverticulitis. Though uncommon, in patients with a history of recurrent diverticulitis, alternative causes of left lower quadrant abdominal pai…

Meckel's diverticulitis: a laparoscopic approach.

Meckel's diverticulum is an infrequent malformation (2% of the population) and the most frequent complication is bleeding followed by diverticulitis. This usually occurs with clinical signs and symptoms of acute appendicitis in adults. The diagnosis requires a high index of suspicion and is based on complementary tests in most cases (mainly CT).

Laparoscopic approaches to complicated diverticulitis.

BACKGROUND: The objective of this article is to review the evolving role of laparoscopic surgery in the treatment of complicated diverticulitis. PURPOSE: The authors attempted to give readers a concise insight into the evidence available in the English language literature. This study does not offer a systematic review of the topic, rather it highlights the role of laparoscopy in the treatment of complicated diverticulitis. CONCLUSIONS: New level 1 evidence suggest that observation rather than elective resection following nonoperative management of diverticulitis with abscess and/or extralumi…

Diagnosing and managing acute diverticulitis.

Diverticulosis is the most common structural abnormality of the colon and affects up to one third of those over the age of 60 years. Its complications account for more than 3,000 deaths per year. Acute diverticulitis is the most common complication and 10-25% of patients with diverticulosis may have one or more episodes. Patients present with left-sided abdominal pain that localises in the left iliac fossa. This left-sided pain reflects the underlying predominant left-sided distribution of the diverticula on this side. Patients may also present with right-sided or suprapubic pain, caused eithe…
