Drug resistance in idiopathic generalized epilepsies: Evidence and concepts.

Although approximately 10%-15% of patients with idiopathic generalized epilepsy (IGE)/genetic generalized epilepsy remain drug-resistant, there is no consensus or established concept regarding the underlying mechanisms and prevalence. This review summarizes the recent data and the current hypotheses on mechanisms that may contribute to drug-resistant IGE. A literature search was conducted in PubMed and Embase for studies on mechanisms of drug resistance published since 1980. The literature shows neither consensus on the definition nor a widely accepted model to explain drug resistance in IGE o

Awake resection of temporal encephalocele associated with drug-resistant epilepsy: illustrative case.

BACKGROUND: The association between temporal encephaloceles (TEs) and drug-resistant epilepsy (DRE) is well recognized, yet the literature regarding therapeutic management remains scarce and largely retrospective. Reported strategies vary from conservative approaches to extensive resection of encephaloceles including the adjacent epileptogenic zone (EZ). Surprisingly, although the EZ may be localized within or adjacent to eloquent areas, surgeries under awake conditions have not been described. One plausible explanation is the potential risk of intraoperative pain during surgical manipulation

Autoimmune mechanisms in drug-resistant focal epilepsy: Pathophysiology, biomarkers, and therapeutic implications.

This review explores the role of autoimmune mechanisms in drug-resistant epilepsy (DRE), with emphasis on etiology, mechanisms of drug resistance, and potential immunomodulatory interventions. A structured review of clinical and experimental studies was conducted to assess current knowledge on the interplay between molecular mechanisms and immune responses in DRE. Particular attention was given to the involvement of the mTOR pathway, blood-brain barrier (BBB) dysfunction, astrocyte activation, and overexpression of efflux transporters, as well as the presence of neuronal autoantibodies such as

Hemispherotomy for atypical teratoid rhabdoid tumor-associated drug-resistant epilepsy: illustrative case.

BACKGROUND: Drug-resistant epilepsy (DRE) following treatment for atypical teratoid rhabdoid tumor (ATRT) is rare. The authors report a case in which hemispherotomy was performed to palliate new-onset DRE after ATRT therapy. OBSERVATIONS: A 6-year-old boy with a large left thalamic ATRT underwent subtotal resection, chemotherapy, and radiation therapy. His neurological baseline remained stable, with right hemiparesis, right hemifield neglect, and apraxia of speech. Two years after tumor recurrence and treatment, he developed frequent tonic seizures of left hemispheric origin. Despite trials o

Predicting post-surgical visuospatial memory performance in people with drug-resistant epilepsy.

OBJECTIVE: Although verbal memory decline has been consistently reported following surgery for temporal lobe epilepsy, especially when surgery is performed in the language-dominant hemisphere and targets mesial temporal structures, the relation between the location and the hemisphere of surgery on visuospatial memory, as well as the factors predicting visuospatial memory decline, remains less clear. This study first examines the impact of epilepsy surgery location and laterality on visuospatial and verbal memory performance and then explores the predictors of memory change following surgery.

Individual-specific resting-state networks predict language dominance in drug-resistant epilepsy.

OBJECTIVE: This study was undertaken to reliably estimate individual-specific resting-state cortical networks and determine whether language network topography can predict task-based language dominance in drug-resistant epilepsy. METHODS: We utilized a multisession hierarchical Bayesian model (MS-HBM) trained on drug-resistant epilepsy patients to map high-quality individual-specific cortical networks in this population (n = 65) with only 6-24 min of resting-state functional magnetic resonance imaging (fMRI). We compared the quality of networks to MS-HBM models trained on healthy participants

Quantitative EEG biomarkers of cognitive performance in drug-resistant temporal lobe epilepsy.

BACKGROUND: This study aimed to explore quantitative electroencephalography (qEEG) biomarkers of cognitive performance in drug-resistant temporal lobe epilepsy (TLE) and analyze their relationship with clinical characteristics. METHODS: Cross-sectional study including adult patients with drug-resistant TLE and a control group. Resting-state, eyes-closed qEEG samples were analyzed using the fast Fourier transform approach. Power spectral density was calculated for four frequency bands: delta (1-3.9 Hz), theta (4-7.9 Hz), alpha (8-12.9 Hz), and beta (13-18 Hz). Neuropsychological tests were adm

PET imaging pitfall in recurrent oligodendroglioma with drug-resistant epilepsy: illustrative case.

BACKGROUND: MRI and PET imaging using radiolabeled amino acids offer a valuable adjunct for defining the extent of tumor growth in gliomas, assessing treatment response, and distinguishing true progression from pseudoprogression. OBSERVATIONS: The authors report the case of a 37-year-old patient with intractable epilepsy and suspected recurrence of an oligodendroglioma. MRI revealed progressive gadolinium enhancement in the right frontal lobe, and both fluorethylthyrosine PET and translocator protein imaging demonstrated high tracer uptake in the same region, raising concern for tumor recurre

Transarterial embolization versus hemispherectomy in infants with hemimegalencephaly and drug-resistant epilepsy.

BACKGROUND: Hemimegalencephaly is a severe congenital brain malformation characterized by early-onset, medically refractory seizures. The standard treatment, surgical hemipherectomy (SH), is constrained by age-related limitations. To address this gap, we developed a staged transarterial embolization (TAE) approach as a minimally invasive alternative for infants under 10-12 weeks. The comparative efficacy of TAE versus SH has not been systematically evaluated. METHODS: This single-center observational comparative effectiveness study evaluated seizure burden, neurodevelopmental outcomes, and pe

Elevated neuroinflammation and region-specific clinical correlates in drug-resistant focal epilepsy.

PURPOSE: Approximately one-third of patients with epilepsy become refractory to traditional anti-seizure medications. Evidence from animals and in humans suggests a bidirectional relationship between epilepsy and neuroinflammation, offering potential as a novel therapeutic pathway. This study investigates region-specific relationships between neuroinflammation and medication-refractory focal epilepsy using [18F]FEPPA, a next-generation translocator protein radioligand and biomarker for microglial density with favorable binding kinetics and signal-to-noise ratio compared to [11C]PK11195. Magnetoencephalography Profile of Patients with Drug-Resistant Focal Epilepsy and Normal MRI.

BACKGROUND AND OBJECTIVES: Magnetoencephalography (MEG) could be a valuable tool in the presurgical evaluation of drug-resistant epilepsy (DRE), especially when the initial evaluation is inconclusive. In this retrospective study, we describe the profile of MEG in patients with DRE and normal magnetic resonance imaging (MRI). METHODS: We included patients with focal epilepsy and normal MRI who underwent presurgical evaluation for DRE. MEG profiles of these patients, including the frequency of spikes, density of clusters, number of clusters, and concordance with video electroencephalography (VE

Dramatic improvement of drug-resistant epilepsy following cerebral infarction: a case report.

BACKGROUND: An estimated 30% of patients with epilepsy experience drug-resistant epilepsy, which is the failure to control seizures despite the use of two or more antiseizure medications. Although other treatment options are considered, these alternatives often prove ineffective. CASE PRESENTATION: A 60-year-old East Asian male patient diagnosed with drug-resistant epilepsy experienced several seizures daily despite being on eight different antiseizure medications. Seizures began at age 15. He underwent epilepsy surgery at age 34, yet the seizures persisted. An electroencephalogram revealed m

Use of ketogenic dietary therapy for drug-resistant epilepsy in early infancy.

OBJECTIVE: There is growing evidence that ketogenic dietary therapy (KDT) can be safely and efficiently used in young children, but little evidence exists on its use in newborns. Developmental and epileptic encephalopathies starting in the neonatal period or early infancy usually present a poor prognosis. The aim of this study was to evaluate effectiveness, safety, and survival of infants younger than 3 months of age with drug-resistant epilepsy in whom KDT was used. METHODS: A retrospective study was conducted to evaluate neonates and infants younger than 3 months who started KDT for drug-re
