Emergency esophago-gastro-duodenoscopy was performed in 45 consecutive patients (26 men, 19 women, mean age 64 [23-91] years) with non-varicose upper gastrointestinal bleeding. The bleeding was arrested endoscopically by circumferential injection of an average of 6 (2-10) ml 1% polidocanol solution. In 10 patients an injection treatment was not possible because of extensive erosion or massive arterial bleeding. Bleeding recurred in seven patients 12-48 hours after initial injection. A 48 year old woman presented with obstructive jaundice 10 years back. Upper gastrointestinal endoscopy revealed a growth infiltrating the ampulla of Vater, which was confirmed to be adenocarcinoma on cytology. Forty-eight patients with dermatitis herpetiformis were investigated with duodenoscopy, endoscopic biopsy, and conventional upper jejunal biopsy. Dependence of healing of a chronic ulcer of the duodenum on structural changes of its mucosa was studied. A 64-year-old female aborigine presented with acute cholangitis and obstructive jaundice for three days. Abdominal ultrasonography showed dilatation of the common bile duct, intrahepatic ducts and a linear tubular structure in the common bile duct. We studied the effects of synthetic protease inhibitors on the motility of the human duodenal papilla. Before and after the intravenous administration of gabexate mesilate (GM) or nafamostat mesilate (NM), the duodenal papillary pressure was measured with a catheter tip pressure transducer under duodenoscopy. Eleven patients wtih gallstone-related acute pancreatitis underwent urgent duodenoscopy and diathermy sphincterotomy; six had stones impacted at the papilla of Vater, and the remainder had stones in the bile duct.
