Canal wall up tympanoplasty for middle ear cholesteatoma in adults: modeling cartilage.

OBJECTIVES: The authors report their experience with a variant cartilage tympanoplasty technique in a canal wall up (CWU) procedure for middle ear cholesteatoma, comparing reliability with other techniques. Functional results are analyzed according to ossicular chain status. MATERIALS AND METHODS: A retrospective study was performed in adults operated on with CWU tympanoplasty for middle ear cholesteatoma. The surgical technique of "modeling" cartilage is described. RESULTS: One hundred and nine patients were included (113 procedures). There were four recurrences (3.5%). Mean follow-up was 4

[Bilateral bisphosphonate-associated osteonecrosis of the external ear canal : a rare case].

Osteonecrosis of the mandible is a well-known and dreaded complication after the administration of bisphosphonates. Only a few cases of bisphosphonate-associated osteonecrosis of the external ear canal have been described. Especially after long-lasting bisphosphonate therapy, for example in patients with multiple myeloma, an ulceration of the floor of the bony external auditory canal should be treated surgically and be distinguished from radioosteonecrosis, malignant external otitis, or carcinoma of the external ear.

Temporomandibular joint herniation into the external ear canal through foramen of Huschke.

Foramen of Huschke, which is also known as the foramen tympanicum, is an anatomical variation of the tympanic portion of the temporal bone. This foramen is located on the anteroinferior aspect of the external auditory canal, posteromedial to the temporomandibular joint. Herein, a rare case of foramen Huschke with a herniation of the soft tissues around the temporomandibular joint that result in severe otalgia and its treatment are presented.

Computed tomographic evaluation of middle ear aeration following intact canal wall tympanoplasty.

The condition of the mastoidectomy cavity following intact canal wall tympanoplasty (ICWT) is always of great interest to the otosurgeon. We evaluated the status of postoperative aeration in a stable ear following ICWT for various types of chronic otitis media using high-resolution computed tomography (CT). The mastoidectomy cavity in all the cases of simple suppurative otitis is totally aerated and that in over 60% of the cases of adhesive otitis, attic type cholesteatoma and adhesive type cholesteatoma is obliterated by a soft tissue density mass. The size of aerated spaces in the postoperat

Establishing critical differences in ear-canal stimulus amplitude for detecting middle ear muscle reflex activation during olivocochlear efferent measurements.

Objective: Assessments of the medial olivocochlear reflex (MOCR) may have clinical utility. The MOCR is measured using contralateral inhibition of otoacoustic emissions but concurrent activation of the middle ear muscle reflex (MEMR) confounds test interpretation. MEMR activation can be detected using the change in ear-canal stimulus amplitude without versus with an MOCR elicitor. This study provides a description of how critical differences in ear-canal stimulus amplitude can be established.Design: Clicks were presented in right ears without and with a contralateral MOCR elicitor. Ear-canal s

Long-term status of middle-ear aeration post canal wall down mastoidectomy.

BACKGROUND: Retraction pocket theory is the most acceptable theory for cholesteatoma formation. Canal wall down mastoidectomy is widely performed for cholesteatoma removal. Post-operatively, each patient with canal wall down mastoidectomy has an exteriorised mastoid cavity, exteriorised attic, neo-tympanic membrane and shallow neo-middle ear. OBJECTIVE: This study aimed to clinically assess the status of the neo-tympanic membrane and the exteriorised attic following canal wall down mastoidectomy. METHODS: All post canal wall down mastoidectomy patients were recruited and otoendoscopy was per

Attenuating the ear canal feedback pressure of a laser-driven hearing aid.

Microphone placement behind the pinna, which minimizes feedback but also reduces perception of the high-frequency pinna cues needed for sound localization, is one reason why hearing-aid users often complain of poor sound quality and difficulty understanding speech in noisy situations. In this paper, two strategies are investigated for minimizing the feedback pressure (thereby increasing the maximum stable gain, MSG) of a wide-bandwidth light-activated contact hearing aid (CHA) to facilitate microphone placement in the ear canal (EC): (1) changing the location of the drive force and its directi…

A case of ear canal black pigment foreign body mimicking a melanoma.

Melanoma of the ear canal is extremely rare; due to its hidden area, most would present late in their history. We present the first case reported, of a black pigment foreign body mimicking a melanoma in the external auditory canal. A 14 year old Chinese male presented with right sided otalgia and itch. Otoscopic examination revealed an irregular black naevus in the right auditory canal. An intraoperative excision showed a black pigment foreign body giant cell reaction. Clinicians should be aware of the possibility of a foreign body granuloma of the ear canal that might mimic a melanoma.

The floating mass transducer for external auditory canal and middle ear malformations.

OBJECTIVE: The indications for the Med-El Vibrant Soundbridge, currently limited to patients with sensorineural hearing loss and normal middle ear function, have been extended to include patients with conductive or mixed hearing loss because of severe acquired or congenital ossicular chain defects. Patients with congenital aural atresia have combined malformations of the external auditory canal and the middle ear, often accompanied by severe mixed hearing impairment. Long-term results of traditional surgical techniques for treatment of congenital aural atresia show a persistent air-bone gap in

Superior semicircular canal dehiscence associated with external, middle, and inner ear abnormalities.

We report a case of bilateral superior semicircular canal dehiscence (SSCD) in a 38-year-old man who presented with congenital hearing loss without vestibular symptoms. This case is unusual due to the association of SSCD with multiple congenital anomalies of the external, middle, and inner ears, but without any other craniofacial or systemic developmental anomalies. Findings of multiple malformations of temporal bone structures in this case lend support to the theory that SSCD may have an underlying developmental or congenital etiology.

[What clinical significance do vascular anomalies of the outer ear canal have?].

Vascular anomalies of the internal auditory canal (IAC) are relatively common, being found in 20-25% of magnetic resonance imaging (MRI) studies performed in patients with unilateral sensorineural hearing loss. They are usually asymptomatic and their diagnosis is almost always based on MRI, as they are generally undetected by routine clinical examination. Recently, the clinical relevance of these IAC anomalies and the effectiveness of vascular decompression have been debated. We present a series of 7 vascular anomalies of the IAC found in a series of 83 consecutive MRI scans performed in a scr

Acoustic mechanisms that determine the ear-canal sound pressures generated by earphones.

In clinical measurements of hearing sensitivity, a given earphone is assumed to produce essentially the same sound-pressure level in all ears. However, recent measurements [Voss et al., Ear and Hearing (in press)] show that with some middle-ear pathologies, ear-canal sound pressures can deviate by as much as 35 dB from the normal-ear value; the deviations depend on the earphone, the middle-ear pathology, and frequency. These pressure variations cause errors in the results of hearing tests. Models developed here identify acoustic mechanisms that cause pressure variations in certain pathological

Electrocochleographic effects of ear canal pressure change in subjects with Menière's disease.

This study examined the relationship between objective changes in electrocochleographic (ECochG) parameters and subjective changes in symptoms in a group of patients with Menière's disease/endolymphatic hydrops (MD/ELH) after application of positive and negative atmospheric pressure changes to the sealed ear canal. Our goals were to further examine the therapeutic effects of atmospheric pressure change on the treatment of MD/ELH, as reported by a series of Swedish studies, and to examine the utility of ECochG for monitoring these effects. By combining a clinical immittance unit and an evoked p…

Equivalent ear canal volumes in children pre- and post-tympanostomy tube insertion.

Pre- and postoperative equivalent ear canal volume measures were obtained from a group of 334 children ranging in age from 6 weeks to 6.7 years. The purpose of the study was to develop volumetric guidelines for the determination of tympanostomy tube patency. For children 4 years and older, almost no ambiguity existed in making this determination accurately. For younger children, the pre- and postoperative distributions overlap. A criterion value of greater than or equal to 1.0 cm3 as an indicator of a tympanic membrane perforation appears to yield the lowest possible error rate. When both pre-…

Skull base Langerhans cell histiocytosis presenting as persistent ear discharge with granulation-like tissue in the external ear canal.

Langerhans cell histiocytosis (LCH) is a neoplastic disease characterised by the deposition of granulomatous lesions in various tissues in the body. LCH of the lateral skull base commonly presents with scalp lesions, post-auricular lesions, otalgia and persistent ear infections. A toddler was brought to the ENT clinic with persistent left ear canal granulation tissue and bloody and malodorous discharge. After failed medical treatment with antibiotic and steroid topical drops. The ear was examined under general anaesthesia. Large friable granulation tissue was seen obstructing the external audi…
