Clinical Signs and Offending Food Allergens in Canine Cutaneous Adverse Food Reactions in Malaysia, Singapore and Indonesia: A Prospective Study.

BACKGROUND: Skin responses to dietary components in dogs are referred to as cutaneous adverse food reactions (CAFRs). The most frequently reported offending food allergens in dogs from Australia, Europe and North America are beef, dairy products, chicken, wheat and lamb. OBJECTIVES: To determine the clinical signs and common offending food allergens associated with CAFRs of dogs from Malaysia, Singapore and Indonesia. MATERIALS AND METHODS: Forty-nine client-owned dogs with nonseasonal pruritus were referred to dermatology speciality practices in Malaysia and Singapore, and general practices

[Rectal bleeding in infancy; could it be an allergy?].

Rectal bleeding in infants is frequently regarded as an alarming symptom for healthcare providers and often causes significant concern and anxiety among parents. In this article, we discuss two infants who were urgently referred to pediatric services for further evaluation. Both infants showed no signs of illness. The diagnosis in one case was ingested maternal blood and in the other Food Protein-Induced Allergic Proctocolitis (FPIAP), triggered by cow's milk. FPIAP is a non-IgE-mediated food allergy. FPIAP can also occur in exclusively breastfed infants when the lactating mother consumes cow'

Eosinophilic gastroenteritis presenting with severe anemia and near syncope.

Eosinophilic gastrointestinal disorders or eosinophilic digestive disorders encompass a spectrum of rare gastrointestinal disorders that inc … infiltration is considered definitive for diagnosis. Corticosteroid therapy, food allergen testing, elimination diets, and elemental diets are considered effective treatments for eosinophilic gastroenteritis. The treatment and prognosis of eosinophilic gastroenteritis is determined by the severity of the clinical manifestations. We describe a 24-year-old woman with …

Urticaria.

Urticaria is a problem often as vexing to the physician as to the patient. The approach to the patient with hives first demands a search for … amiliarity with the antihistamines and their relative advantages in the various forms of urticaria. Elimination diets are of diagnostic as well as therapeutic value: pencillin-free, yeast-free, and salicylate-free diets are particularly useful. Therapeutic trials of tetracycline, nystatin and griseofulvin may be helpful, while corticosteroids and specific desensiti …

Pediatric Eosinophilic Esophagitis Endotypes: Are We Closer to Predicting Treatment Response?

Eosinophilic esophagitis (EoE) is a chronic, food antigen-driven gastrointestinal disease that is characterized by esophageal eosinophilia. Currently, there are no Food and Drug Administration (FDA)-approved treatments for EoE, but the two most commonly prescribed therapies include topical corticosteroids and food elimination diets. Clinical trials have revealed a significant proportion of cases that are resistant to topical corticosteroids, and although we define EoE as a food antigen-driven disease, not all patients with EoE respond to elimination diets or even elemental diets. The varied re…

PCR analysis of a prescription vegetarian diet and use in three dogs with cutaneous adverse food reactions.

BACKGROUND: Cutaneous adverse food reaction (CAFR) is diagnosed by performing an elimination diet trial utilizing prescription or home-cooked diets followed by provocative challenge. OBJECTIVES: To report findings of PCR analysis of a prescription vegetarian diet (RCV) for undeclared proteins of animal origin, as well as to describe its utilization for diagnosis and management of dogs suspected of having CAFR. ANIMALS: Three client-owned dogs. METHODS: PCR analysis of RCV for 11 mammalian species and poultry. In three dogs, clinical examination, cytology, aerobic culture (if indicated) and

Nutritional management of cow's milk allergy in children: An update.

Cow's milk is one of the most common foods responsible for allergic reactions in children. Cow's milk allergy (CMA) involves immunoglobulin E (IgE)- and non-IgE-mediated reactions, the latter being both variable and nonspecific. Guidelines thus emphasize the need for physicians to recognize the specific syndromes of CMA and to respect strict diagnostic modalities. Whatever the clinical pattern of CMA, the mainstay of treatment is the elimination from the diet of cow's milk proteins. The challenge is that both the disease and the elimination diet may result in insufficient height and weight gai

Laboratory Monitoring of Nutritional Deficiencies in Children Following Restrictive Diets: A Narrative Review and Risk-Based Considerations.

INTRODUCTION: Restrictive diets are increasingly encountered in pediatric practice and may be adopted voluntarily or prescribed for medical conditions. Although they can support normal growth when appropriately planned, exclusion of nutritionally important foods may increase the risk of nutrient inadequacy. This narrative review examined nutritional deficiencies and laboratory monitoring in children following plant-based, food-allergy elimination, gluten-free, ketogenic, and protein-restricted diets for inherited metabolic disorders. METHODS: Targeted searches of PubMed, Scopus, and Web of Sc

Effect of potato on acid-base and mineral homeostasis in rats fed a high-sodium chloride diet.

Excessive dietary NaCl in association with a paucity of plant foods, major sources of K alkaline salts, is a common feature in Western eating habits which may lead to acid-base disorders and to Ca and Mg wasting. In this context, to evaluate the effects of potato, rich in potassium citrate, on acid-base homeostasis and mineral retention, Wistar rats were fed wheat starch (WS) or cooked potato (CP) diets with a low (0.5 %) or a high (2 %) NaCl content during 3 weeks. The replacement of WS by CP in the diets resulted in a significant urinary alkalinisation (pH from 5.5 to 7.3) parallel to a rise

Dietary Treatment of the Irritable Bowel Syndrome.

Most patients with functional gastrointestinal disorders report that food ingestion appears to exacerbate their symptoms and consequently co … inant irritable bowel syndrome (IBS), and atopy. There is some preliminary evidence to suggest that elimination diets based on immunoglobulin G food antibody testing may possibly have therapeutic potential in IBS, but this requires confirmation.

Diagnosis and management of childhood food allergy.

The pediatrician plays a pivotal role in the initial diagnosis of food allergy. Alternative diagnoses are considered as a careful history, physical examination, and directed laboratory tests determine the type of adverse reaction and the responsible food. Through elimination diets in infants, appropriately selected tests for specific IgE, and, in some cases, supervised oral food challenges, a diagnosis is secured. Treatment consists of strict dietary elimination with provisions for emergency management of accidental ingestions. Referral to an allergist and dietitian is made as warranted by the…

Implementing Dietary Modifications and Assessing Nutritional Adequacy of Diets for Inflammatory Bowel Disease.

Guidelines for dietary recommendations and nutritional therapy for patients with inflammatory bowel disease (IBD) are lacking, and patients are moving toward popular defined diets for relief of symptoms and inflammation. However, many proposed diets involve elimination of specific foods or food groups and may exacerbate or inadequately replete micronutrient deficiencies that are prevalent in patients with IBD at baseline. Further, limited data are available to guide clinicians on the use of dietary protocols for IBD. This article reviews dietary risk factors for IBD and common beliefs about di

Effect of dietary calcium, phosphate and vitamin D deprivation on the pharmacokinetics of 1,25-dihydroxyvitamin D3 in the rat.

The in vivo regulation of circulating 1,25(OH)2D3 concentrations by vitamin D status and by dietary calcium and phosphate deficiency was studied. Adult rats were cannulated in the jugular vein and the clearance of physiological doses of 1,25(OH)2D3 monitored. In vitamin D-replete rats we investigated the effects of dietary calcium and phosphate deficiency on the elimination half life of 1,25(OH)2D3 The results showed no effect of dietary phosphate deficiency on the elimination half life of 1,25(OH)2D3. Dietary calcium deficiency resulted in a small increase of the 1,25(OH)2D3 elimination half

Effectiveness of dietary interventions in managing pediatric gastroesophageal reflux disease: a comprehensive systematic review.

BACKGROUND: Pediatric gastroesophageal reflux disease (GERD) is a common chronic digestive disorder that significantly impacts children's growth, development, and quality of life. Traditional management of GERD often involves pharmacological interventions, such as proton pump inhibitors (PPIs) and histamine-2 receptor antagonists (H2RAs), which raise concerns about side effects and long-term safety. Recently, interest has shifted toward dietary interventions as complementary or alternative approaches to managing GERD symptoms. OBJECTIVE: This systematic review aims to evaluate the efficacy of

Low FODMAP and Gluten-Free Diet Implementation: A Guide for the General Practitioner.

The low fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAP) and gluten-free diets are increasingly used as both therapeutic and diagnostic interventions for pediatric patients with gastrointestinal disorders. The low FODMAP diet is primarily indicated for children with irritable bowel syndrome or disorders of brain-gut interaction when symptoms persist. Implemented in 3 phases (elimination, reintroduction, and personalization), the diet helps identify individual triggers and improve symptoms, such as bloating and abdominal pain. In contrast, the gluten-free diet

The impact of gluten-containing and gluten-free diets on intestinal inflammation: Evaluation of fecal calprotectin in a healthy rat model.

OBJECTIVE: Gluten-free diets are increasingly adopted by individuals without gluten-related disorders, despite limited evidence for anti-inflammatory benefit. We evaluated whether varying gluten exposure influences intestinal inflammation in healthy rats using fecal calprotectin (fKp) as the outcome. PATIENTS AND METHODS: Forty newly weaned male Wistar albino rats were randomized to four isocaloric, iso-nitrogenic diets for 12 weeks (n=10/group): (1) standard wheat-containing control, (2) gluten-free animal protein-based diet, (3) control diet supplemented with 6% vital wheat gluten, and (4)

Influence of vitamin deficiency and level of dietary protein on the incidence of leg problems in broiler chicks.

Diets containing either 22 or 30% protein were supplemented with a vitamin mix where one of 11 added vitamins were singly eliminated from the mix. Male, day-old broilers were fed these diets to 3 weeks of age, and weight gains, feed utilization, and leg problems were recorded. In general, the higher protein diet did not result in a greater incidence of leg problems; however, it did alter performance of several of the vitamin-deficient diets as compared to the lower protein diet. Of the 11 vitamins studied only nonsupplementation of riboflavin markedly reduced weight gain and feed utilization d

Nutrition and thyroid disease.

PURPOSE OF REVIEW: The aim of this review was to determine, based on the most recent findings, the involvement of trace elements and vitamins critical for thyroid function and combating thyroid disease. RECENT FINDINGS: Nutritional guidance is pivotal to reducing the risk of thyroid disease and to managing it when it arises, this meaning the prescription of diets rich in such micronutrients as iodine, selenium, iron, zinc, and vitamins B12, D3, and A. Most of the above micronutrients are good antioxidants, building up an anti-inflammatory profile, reducing thyroid autoantibodies and body fat,
