Is there a need to identify new anti-emetic drugs?

Nausea and vomiting occur in a large number of disease conditions and as side effects of many drug treatments, including use of analgesics and anesthesia in surgery and chemotherapy in cancer treatment. Current anti-emetics provide relief from only some sources of vomiting, with more limited benefits for the control of nausea. Elucidation of forebrain pathways that generate nausea and brainstem circuitry controlling emesis are significant obstacles for the development of effective universal anti-nausea and anti-emetic treatments.

Fatal hypernatremia from saltwater used as an emetic.

An adolescent boy returned home from a party and told his parents he may have taken some pills while there. He was given saltwater to drink, in an effort to induce emesis. He vomited numerous times, then seized. Hypernatremia (195 mmol/L) was diagnosed at the community hospital, and he was transferred to a pediatric intensive care facility. He suffered numerous complications and died from cerebral herniation. This case is presented to remind physicians of the dangers of this obsolete therapy.

Guidelines for anti-emetic therapy: acute emesis.

Anti-emetic therapy has become integral to the management of patients with cancer. Goals related to complete emesis control include providin … mesis. Currently, serotonin receptor antagonists and corticosteroids are the two categories of anti-emetics that are most effective, have the fewest side-effects and are convenient to use. These agents are recommended in combination for highly emetogenic chemotherapy regimens and as single agents or in combination for moderately to highly emetogenic chemo …

Identification and characterization of the emetic effects of peptide YY.

Emesis was noted following intravenous bolus injections into dogs of a chromatographic subfraction derived from porcine small intestinal tissue extracts. The active agent was isolated from this subfraction using sequential ion-exchange and reverse-phase HPLC and demonstrated to be the recently identified regulatory peptide PYY. The threshold dose for PYY-induced emesis in the dog is less than 120 pmol/kg. Emesis was sometimes seen following large IV bolus doses of neuropeptide Y (NPY), but none was seen following IV injection of pancreatic polypeptide (PP). Dogs prepared with discrete, bilater

Extensive gastrointestinal damage following a saline emetic.

A 27-year old female received about 1 kg of salt in about 600 ml of water as an emetic after a self-poisoning attempt. Vomiting did not occur and massive necrosis of gastric, duodenal, and jejunal mucosa resulted in a severe and prolonged illness which required 31 weeks of continuous hospital care and 31 laparotomies over a period of 16 months. The dangers of salt emesis are emphasized.

Fatal outcome from administration of a salt emetic.

A case is described of the death of a young female patient following the administration of a salt emetic after a relatively minor overdose of a proprietary analgesic containing aspirin. It is suggested that death occurred as a direct consequence of the salt ingestion and that the dangers of this method of inducing emesis should be more widely appreciated.

The emetic activity of staphylococcal enterotoxins, SEK, SEL, SEM, SEN and SEO in a small emetic animal model, the house musk shrew.

Staphylococcal enterotoxins (SEs) produced by Staphylococcus aureus are the most recognizable causative agents of emetic food poisoning in humans. New types of SEs and SE-like (SEl) toxins have been reported. Several epidemiological investigations have shown that the SEs and SEl genes, particularly, SEK, SEL, SEM, SEN and SEO genes, are frequently detected in strains isolated from patients with food poisoning. The purpose of the present study was to evaluate the emetic activity of recently identified SEs using a small emetic animal model, the house musk shrew. The emetic activity of these SEs

Effectiveness of emetics to study plastic ingestion by Leach's Storm-petrels (Oceanodroma leucorhoa).

Most plastic ingestion studies rely on dissection of dead birds, which are found opportunistically, and may be biased. We used Leach's Storm-petrels (Oceanodroma leucorhoa) in Newfoundland to study the effect of dose volume, and the efficacy of emesis using syrup of ipecac as an emetic. Ipecac is a safe method of non-lethally sampling stomach contents, and recovered all ingested plastic. Almost half the storm-petrels sampled had ingested plastic, ranging from 0 to 17 pieces, and weighing 0.2-16.9 mg. Using the Ecological Quality Objective for Northern Fulmars, adjusted for storm-petrels smalle
