Infective endocarditis develops when aggravating conditions damage the endothelial lining of the heart and create a nidus of infection. The nidus triggers a cytokine-mediated inflammatory response, which can lead to platelet aggregation and thrombus formation. Bacteria or fungi in the blood can then adhere to the thrombus and colonize, proliferate, and form vegetations. Staphylococcus aureus, Streptococcus species, and Enterococcus species comprise more than 80% of identified bacterial pathogens in cases of infective endocarditis. Endocarditis should be considered in any patient with fever or

Seven young to middle-aged patients with Haemophilus parainfluenzae endocarditis are reported. Three patients had underlying heart disease and three patients had recent events predisposing for endocarditis. The clinical presentation was subacute or acute and new pathologic murmurs were uncommon. Diagnosis was prolonged because of difficulties in isolating the organism. Routine subculturing of blood cultures to chocolate agar with incubation in CO2 is recommended. A prominent complication, occurring in six patients, was major arterial occlusion secondary to emboli. Antibiotic control of infecti

INTRODUCTION: Rheumatic and congenital heart diseases account for the majority of hospital admissions for cardiac patients in India. Tetralogy of Fallot is the most common congenital heart disease with survival to adulthood. Infective endocarditis accounts for 4% of admissions to a specialized unit for adult patients with a congenital heart lesion. This report is unique in that a severe stenotic lesion of the mitral valve, probably of rheumatic aetiology, was noted in an adult male with Tetralogy of Fallot.
CASE PRESENTATION: An unusual association of rheumatic mitral stenosis in an adult Ind

We herein report two cases of proteinase 3-anti-neutrophil cytoplasmic antibody (PR3-ANCA)-related nephritis in infectious endocarditis. In both cases, the patients were middle-aged men with proteinuria and hematuria, hypoalbuminemia, decreased kidney function, anemia, elevated C-reactive protein (CRP) levels, and PR3-ANCA positivity. Each had bacteremia, due to Enterococcus faecium in one and Streptococcus bovis in the other. One patient received aortic valve replacement therapy for aortic regurgitation with vegetation, and the other underwent tricuspid valve replacement therapy and closure o

A 23-year-old Japanese male with no evidence of previous heart disease was presented with bicuspid aortic valve and a life threatening acute aortic regurgitation due to subacute bacterial endocarditis. By echocardiographic techniques, a precise diagnosis was made based on the following findings: 1) premature mitral valve closure, 2) snowfall-like echoes between the systolic aortic cusps, 3) eccentricity of a diastolic aortic valvular echo. The echocardiographic diagnosis was confirmed on surgery, in which aortic valve replacement was performed with satisfactory postoperative results.

Patent ductus arteriosus in adults is usually an isolated lesion with a small to moderate degree of shunt, as a larger shunt becomes symptomatic earlier in childhood. The classic murmur of patent ductus arteriosus may be the first clue to its presence, or it may be detected accidently by transthoracic echocardiography, computed tomography, or magnetic resonance angiography for an unrelated condition. The percutaneous approach is safe and effective in more than 98% of patients. Subacute bacterial endocarditis prophylaxis is not indicated routinely except for 6 months following the closure percu

Roth spots are white-centred retinal haemorrhages, previously thought to be pathognomonic for subacute bacterial endocarditis. A number of other conditions can be associated with Roth spots. In this case, the authors describe the association of Roth spots and pernicious anaemia. This association has been rarely described in the medical literature. Correct diagnosis and treatment with intramuscular vitamin B(12) injections resulted in complete resolution of the anaemia and Roth spots. The authors hope to alert clinicians to think of various differentials of Roth spots, and initiate prompt inves

Mitral regurgitation associated with secundum atrial septal defect is described in 4 patients, each with a different mitral lesion: rheumatic valvular disease, congenitally cleft valve, subacute bacterial endocarditis with disruption of the chordae tendineae, and traumatic valve rupture. The pathological spectrum of mitral valve disease associated with atrial septal defect is reviewed, and it is suggested that structural abnormality of the mitral valve may accompany the atrial septal defect. More general awareness of this association will allow the surgeon more accuracy in defining and repairi

In a prospective clinical and bacteriological study of 192 consecutive patients with septicemia admitted during the period 1967--1971 to a hospital for infectious diseases, 31 cases of proved (group I) and 10 with probably (group II) bacterial endocarditis were observed and analysed with regard to a variety of factors. The incidence of bacterial endocarditis was 3 per 1 000 admissions. The mean age of the patients was 52.6 years; there was no sex predominance. In about one third of the patients apparently normal valves were involved. Underlying non-cardiac factors were present in two thirds, e

Antibiotics with small but definable mortality, such as chloramphenicol, should not be used when safer drugs will suffice. Antibiotics with a low (1 to 5 per cent) morbidity should not be used when safer drugs are available. Therefore, cleocin, minocycline, or oral erythromycin estolate should rarely be used and regular erythromycin base is almost always preferable. Fever should not be treated with antibiotics since they are not antipyretics. "Colds" should not be treated with antibiotics, but antibiotics should be administered to patients with a history of chronic bronchitis, sinusitis, and r

Infective endocarditis (IE) is uncommon and has, in the past, been most often caused by viridans group streptococci (VGS). Due to the indolent nature of these organisms, the phrase 'subacute bacterial endocarditis', so-called 'SBE', was routinely used as it characterised the clinical course of most patients that extended for weeks to months. However, in more recent years, there has been a significant shift in the microbiology of IE with the emergence of staphylococci as the most frequent pathogens, and for IE due to Staphylococcus aureus, the clinical course is acute and can be associated with

Soma Weiss, a brilliant clinician, researcher, and teacher at Harvard Medical School, cared for Alfred S. Reinhart, who succumbed to subacute bacterial endocarditis in his final year at Harvard Medical School. Reinhart recorded his observations and experiences while a patient in the Thorndike Memorial Laboratory at the Boston City Hospital, and Weiss incorporated these in a paper some 10 years after Reinhart's death. In the tradition of Francis Weld Peabody, Weiss used Reinhart's memoir to emphasize the importance of listening to patients, of projecting sympathy, and of understanding and respo

A solid-phase monoclonal rheumatoid factor (mRF) assay for circulating immune complexes (IC) is described. Microtiter plates are coated with mRF, serum diluted 1 : 40 is added, and complexed IgG is detected with protein A coupled to peroxidase. Selective binding was obtained with aggregated human gamma-globulin and soluble IC prepared in vitro. Sucrose density gradient ultracentrifugation studies indicate that mRF binds intermediate-size IC (between 7S and 19S). Abnormal levels of IC were found in 78% of sera from patients with rheumatoid arthritis, 67% with systemic lupus erythematosus, and 8

Nodular regenerative hyperplasia of the liver, an uncommonly reported and poorly defined clinicopathological entity, obscured clinical diagnosis and was misdiagnosed on hepatic biopsy in a recent case. Approximately 19 cases are recorded in the English literature. Six patients had Felty's syndrome, about 12 patients had congestive heart failure, and the patient under discussion had subacute bacterial endocarditis. Light- and electron-microscopic examination was utilized to define nodular regenerative hyperplasia pathologically. Features common to all reported cases are discussed but elucidatio

White-centered, flame-shaped retinal hemorrhages, also known as Roth spots, occur in several diseases, most often in subacute bacterial endocarditis. Other conditions such as leukemia, hypertensive retinopathy, and pre-eclampsia are other causes. Here, we present the case of a 32-year-old female with sickle cell anemia (HbSS) who was treated at the hospital after experiencing a sickle cell crisis. Fundus examination was performed to rule out sickle cell retinopathies, suggesting the presence of Roth spots in the retina which are rarely seen in sickle cell disease.
