Non-surgical endodontics: contemporary biomechanical preparation of the root canal system.

Treatment of apical periodontitis requires disinfection of the root canal system through means of biomechanical preparation. This is achieved by a process of shaping the root canal space while simultaneously using antibacterial agents to reduce microbiological load in the root canal system in order for inflammation of the periradicular tissues to resolve and the body to heal itself. The fundamental principles underlying this process have not changed in decades; however, in contrast, the armamentarium available to the clinician continues to evolve rapidly. Nickel-titanium file design has made a

Regenerative Endodontics: A Potential Solution for External Root Resorption (Case Series).

INTRODUCTION: Regenerative endodontic procedures (REPs) are aimed to treat apical periodontitis and promote root maturation of immature necrotic teeth. However, REPs are not intended to be a primary indication for treating or arresting external root resorption (ERR). The purpose of this study was to describe REP treatment in the cessation of ERR. METHODS: Four cases (5 teeth) of posttraumatic immature teeth diagnosed with necrotic pulp and apical periodontitis or chronic apical abscess were treated with REPs using plasma-rich fibrin as a scaffold. All the teeth showed ERR and have been follow

Local applications of tetracyclines in endodontics and dental trauma: a review.

Local application of antibiotics within the root canal system may be a more effective mode for delivering the drug. Tetracyclines have been used to remove the smear layer from instrumented root canal walls, for irrigation of retrograde cavities during periapical surgical procedures, and as an intracanal medicament. Substantivity of tetracyclines has been shown for up to at least 12 weeks. BioPure MTAD is effective in removing the smear layer. However, the antimicrobial efficacy against E faecalis of 1.3% NAOCl/MTAD compared with that of the combined alternate use of 5.25% NAOCl and 15% EDTA is

Povidone-iodine and isopropyl alcohol as disinfectants in preparation for endodontics.

The clinical effectiveness of povidone-iodine as a disinfectant in teeth isolated by a rubber dam was compared to the effectiveness of isopropyl alcohol. The results showed no statistically significant difference between povidone-iodine or isopropyl alcohol for disinfecting intact enamel surfaces. When a scrub-type application of povidone-iodine was used, preliminary removal of plaque with pumice did not increase its effectiveness. Both agents were significantly less effective at 15 minutes after application than they were at 90 seconds after application.

Endodontic diagnosis in dogs.

Complete endodontic evaluation should diagnose endodontic involvement, provide a treatment plan, and evaluate the true success or failure probabilities. Evaluation of the age, physical and oral health of the patient, the endodontic anatomy, clinical and radiographic findings can provide a treatment plan and a predictable prognosis.

[Medications for endodontic use].

Medication can never disinfect a badly shaped canal and successful treatment will depend on the endocanal preparation. Despite the indisputable antibacterial efficacy of Rocklès 4 in vitro, use of this medicament should be banned because of its toxicity, its mutagenic and antigenic action and its long-term inefficacy in vivo. The same goes for all other antiseptics, anti-inflammatory agents and antibiotics mentioned, that should be discarded because of their harmful effects on patients or their inefficacy on endodontic flora. Our endodontic medicine cabinet should only contain the following pr

Mutagenicity of endodontic sealers.

Four endodontic sealer materials and some of their chemical constituents were subjected to Ames' test for mutagenicity with the Salmonella/microsome assay. Extracts of two zinc oxide-eugenol based materials and of one gutta-percha-zinc oxide-chloroform based sealer were negative in the test. Extracts of a synthetic polymer material, based on epoxy-bis-phenol A, induced mutations in Salmonella typhimurium TA 100 as did extracts of the epoxy-bis-phenol A resin alone. Formaldehyde, an active ingredient from one of the ZnO-based materials, induced mutations in both Salmonella typhimurium TA 98 and

Federated Learning in Endodontics: A Framework for Privacy-Preserving Multicentre Artificial Intelligence.

INTRODUCTION AND AIM: High-quality artificial intelligence (AI) models in endodontics require access to diverse, well-annotated datasets. This review introduces federated learning (FL) as a privacy-preserving framework for collaborative AI in endodontics. METHODS: In this narrative review, a comprehensive literature search was conducted across databases, encompassing studies published up to April 2026. The search strategy was intentionally broad to facilitate a thorough exploration of the evolution of the relevant concepts. However, to ensure consistency in comprehension and analysis, the rev…

Beyond the apex: anatomical risk assessment to prevent extrusion injuries in endodontics.

Extrusion of irrigants or obturation materials during endodontic therapy may result in significant complications, ranging from postoperative pain to nerve damage and chronic inflammation. Many of these adverse outcomes can be prevented through detailed anatomical assessment and thoughtful treatment planning. This article explores how anatomical complexities, including root apex morphology, proximity to vital structures, and apical resorption, contribute to extrusion risk.

Evaluating the concept of three-dimensional printing guided endodontics in the dog.

INTRODUCTION: Guided endodontics represents an effective method for achieving safe and reliable endodontic surgery in human medicine. However, it is rarely employed in small animal dentistry. This study employed finite element analysis and three-dimensional (3D) printing techniques to explore the feasibility of guided endodontics in Beagle mandibular teeth. METHODS: The methodology included the processing of Computed Tomography (CT) data, the creation of mathematical and 3D printing templates of the root canal pathways, and the evaluation of dog 3D printing guided endodontics compared with cl…

The Other Side of Endodontics: Vital Pulp Therapy on Mature Permanent Teeth.

Vital pulp therapy (VPT) was once a treatment modality that was only considered for immature permanent teeth to allow for the continued development of the radicular system if the pulp was vital. With the advent of bioceramics and bioactive materials such as calcium silicate cements, the applications for VPT have greatly increased, giving dentists a treatment option other than nonsurgical root canal therapy (RCT) for mature teeth previously diagnosed to have an irreversible pulpitis. This approach can serve as a less invasive, simpler procedure than RCT while allowing for the preservation of na

Cell-Based Regenerative Endodontics for the Treatment of Irreversible Pulpitis: AnIn VivoInvestigation.

INTRODUCTION: This study aims to investigate the ability of umbilical cord mesenchymal stem cells (UC-MSC) to enhance the regeneration of pulp-dentin complex in immature permanent teeth with irreversible pulpitis. METHODS: A total of 32 mandibular premolar teeth with immature apices in 5 dogs were used in this in-vivo randomized controlled trial (RCT). Eight healthy teeth without pre-existing pathosis served as the positive control samples and received no treatment, while in another 8 teeth, the pulp was completely extirpated (negative control). Class V cavities were prepared to induce inflam

Minimally invasive endodontics: a new era for pulpotomy in mature permanent teeth.

Emerging clinical and histologic evidence is challenging the long-established dogma that root canal treatment (RCTx) is the only therapeutic option for preservation of vital mature permanent teeth diagnosed with irreversible pulpitis or carious pulp exposure. Vital pulp therapy procedures like pulpotomy are not only technically simpler and more economical, but also afford patients a host of other benefits over conventional RCTx. This narrative review provides an update on the contemporary understanding of pulp pathophysiology and defence mechanisms, the proposed new diagnostic terminologies fo

A novel patient-specific three-dimensional drug delivery construct for regenerative endodontics.

Evoked bleeding (EB) clinical procedure, comprising a disinfection step followed by periapical tissue laceration to induce the ingrowth of undifferentiated stem cells from the periodontal ligament and alveolar bone, is currently the only regenerative-based therapeutic approach to treating pulp tissue necrosis in undeveloped (immature) permanent teeth approved in the United States. Yet, the disinfection step using antibiotic-based pastes leads to cytotoxic, warranting a biocompatible strategy to promote root canal disinfection with no or minimal side-effects to maximize the regenerative outcome
