The hydropsfinder: an electronic device for the diagnosis of endolymphatic hydrops.

An electronic device has been developed which can diagnose endolymphatic hydrops clinically. The special unit can be used with any standard commercial evoked potential equipment currently employed for electrocochleography and auditory brainstem responses. This clinical tool enables the investigator to distinguish the hydropic human ear from the normal and other pathological conditions.

Detection of delayed endolymphatic hydrops by the furosemide test.

Delayed endolymphatic hydrops, an entity which can be differentiated from Meniere's disease, was first reported simultaneously by Nadol et al. (1975) and Wolfson & Leiberman (1975) and specified by Schuknecht (1978). They diagnosed it from the anamnesis of patients and confirmed the hydropic condition of the labyrinth solely by evidence obtained from surgical procedures such as labyrinthectomy. The aim of the present study was to emphasize the usefulness of the furosemide test for distinguishing this entity before surgery by detecting the hydropic condition in the labyrinth. In these 3 years,

Ongoing Discrepancy Between Unilateral Meniere's Disease and Bilateral Endolymphatic Hydrops on Hydrops MRI.

OBJECTIVE: In the era of Hydrops MRI, a clinical dilemma has emerged: while Meniere's disease (MD) is diagnosed unilaterally, Hydrops MRI demonstrates endolymphatic hydrops (EH) bilaterally. This study aims to clarify this ongoing discrepancy. METHODS: Fifty-six MD patients underwent an inner ear test battery followed by 3 T Hydrops MRI. Eight unilateral MD patients with EH on both ears were classified as Group A and subdivided into Groups A1 (symptomatic EH on the affected ear, n = 8 ears) and A2 (asymptomatic EH on the opposite unaffected ear, n = 8 ears). Another 48 unilateral MD patients

Dynamics in Endolymphatic Hydrops & Symptoms in Meniere's Disease After Endolymphatic Duct Blockage, Preliminary Results.

Objective: The purpose of the present study was to evaluate the dynamics of endolymphatic hydrops (EH) and symptoms in a group of patients who underwent endolymphatic duct blockage (EDB) for treatment of intractable Meniere's Disease (MD), and to explore a metric for verifying the effectiveness of EDB procedure. Methods: A total of 22 patients with intractable MD patients who underwent EDB participated in the present study. EH was visualized using locally enhanced inner ear magnetic resonance imaging (MRI) prior to and following surgery. The vestibular hydrops ratio (VHR) in the second MRI exa

Imaging of endolymphatic hydrops: A comprehensive update in primary and secondary hydropic ear disease.

BACKGROUND: Since the first description by Hallpike and Cairns, the excess of endolymphatic fluid, also known as endolymphatic hydrops (EH), has been established as being the main biomarker in patients with Menière's disease. Recently, the concept of primary (PHED) and secondary hydropic ear disease (SHED) has been introduced. PHED corresponded to Menière's disease while SHED was defined as the presence of EH in patients with pre-existing inner ear disease. OBJECTIVE: In this article, we would like to summarize the methodology of hydrops exploration using MRI and the previously published radi

Detecting endolymphatic hydrops in patients with cranial nerve VIII schwannoma using hydrops MR examination.

PURPOSE: This study utilized Hydrops MRI in patients with cranial nerve (CN) VIII schwannoma to assess the concomitance with endolymphatic hydrops (EH), aiming to elucidate the mechanism of hydrops formation in these patients. METHODS: Twenty-six patients diagnosed as CN VIII schwannoma including vestibular schwannoma (VS) in 24 and intracochlear schwannoma (ICS) in 2 were enrolled. Fifteen patients received radiosurgery and 11 patients opted for a wait-and-scan approach. All patients underwent an inner ear test battery, followed by Hydrops MRI. Based on Hydrops MRI, 6 patients (23%) with pos

Endolymphatic Hydrops in Fluctuating Hearing Loss and Recurrent Vertigo.

Background: Endolymphatic hydrops (EH) is the histopathological hallmark of Ménière's disease (MD) and has been found by in vivo magnetic resonance imaging (MRI) in patients with several inner ear syndromes without definite MD criteria. The incidence and relevance of this finding is under debate. Purpose: The purpose of the study is to evaluate the prevalence and characteristics of EH and audiovestibular test results in groups of patients with fluctuating audiovestibular symptoms not fulfilling the actual criteria for definite MD and compare them with a similar group of patients with definite

Delayed Effect of Active Pressure Treatment on Endolymphatic Hydrops.

OBJECTIVE: To identify eventual correlations between the effect of low-pressure treatment and endolymphatic hydrops in Ménière patients. MATERIAL AND METHODS: The study group consisted of subjects affected by definite Ménière disease (2015) and a severe degree of disability, who received a ventilation tube with or without a low-pressure treatment before undergoing a surgical procedure (vestibular neurectomy). After the placement of the ventilation tube, the subjects were either left alone with the tube or received 1 month of self-administered low-pressure therapy with a portable device. In al

Endolymphatic hydrops in asymptomatic ears in unilateral Ménière's disease.

OBJECTIVES/HYPOTHESIS: The aim of our study was the objective assessment of endolymphatic hydrops in asymptomatic ears in unilateral Ménière's disease with a noninvasive electrophysiological test and investigation of significant clinical signs. The null hypothesis was that there would be no signs of endolymphatic hydrops in the asymptomatic ear. STUDY DESIGN: Prospective study using the traveling wave velocity test for endolymphatic hydrops. METHODS: The traveling wave velocity test was used in conjunction with standard audiological tests to investigate both ears of 181 Ménière's patients at

Selective vestibular ablation by KTP laser in endolymphatic hydrops.

OBJECTIVES AND HYPOTHESIS: Vertigo, the cause of disability in many patients with Ménière's disease, may be the result of the effects of endolymphatic hydrops on the semicircular canals. We hypothesize that intractable vertigo may be controlled by destruction of the semicircular canal neuroepithelium using visible light lasers without the need for extensive fenestration of the bony labyrinth. This study was designed to assess the cochlear effects of potassium titanyl phosphate (KTP) laser-assisted triple semicircular canal ablation (TSCA) in endolymphatic hydrops. STUDY DESIGN: Randomized, pr

Modulation of inner ear pressure in experimental endolymphatic hydrops.

Physiologic study of the guinea pig model of endolymphatic hydrops has been used to better understand the manifestations of Meniere's disease. Quantitative vestibular function testing demonstrates some changes in the hydrops subjects, especially if an additional stress such as postural inversion is imposed. Understanding the hydrodynamic changes on the endolymphatic and perilymphatic fluids by the hydrops condition as well as postural inversion may be useful in explaining the vestibular findings. These pressure effects may alter the micromechanics of the inner ear or possibly induce a relative

Relationship between changes in hearing function and volumes of endolymphatic hydrops after endolymphatic sac drainage.

Background: Endolymphatic sac drainage (ELSD) may have a positive effect on endolymphatic hydrops (EH) and may help to preserve inner ear function. However, the relationship between changes in EH volumes and hearing function after ELSD has not been described. Objectives: We aimed to reveal the factors related to changes in hearing and EH following ELSD. Material and Methods: Twenty-one patients who received ELSD were enrolled. Pure tone audiometry and 3-T magnetic resonance imaging (MRI) 4 h after intravenous injection of gadolinium enhancement were performed just before surgery and 2 years la
