Modeling the impact of tuberculosis interventions on epidemiologic outcomes and health system costs.

BACKGROUND: Tuberculosis (TB) programs must invest in a variety of TB specific activities in order to reach ambitious global targets. Uncertainty exists surrounding the potential impact of each of these activities. The objective of our study was to model different interventions and quantify their impact on epidemiologic outcomes and costs from the health system perspective. METHODS: Decision analysis was used to define the TB patient trajectory within the health system of three different countries. We considered up to seven different interventions that could affect either the natural history

Prenatal development is linked to bronchial reactivity: epidemiological and animal model evidence.

Chronic cardiorespiratory disease is associated with low birthweight suggesting the importance of the developmental environment. Prenatal factors affecting fetal growth are believed important, but the underlying mechanisms are unknown. The influence of developmental programming on bronchial hyperreactivity is investigated in an animal model and evidence for comparable associations is sought in humans. Pregnant Wistar rats were fed either control or protein-restricted diets throughout pregnancy. Bronchoconstrictor responses were recorded from offspring bronchial segments. Morphometric analysis

[Epidemiology of human schistosomiasis in Mauritania. The right bank of the Senegal River as model].

A study was performed to determine the transmission and prevalence of human schistosomiasis in the Mauritanian side of the Senegal River basin. Parasitological investigations by Kato-Katz and urine filtration conducted on 1,259 school children indicated a mean prevalence of S. haematobium--29.0%, 25.9% and 34.3%, respectively, in the children of the lower, middle and high valley. Only the school children of the lower delta valley were infected by S. mansoni with a mean prevalence rate of 21.5%. The malacological investigations carried out in the water points of each visited village highlighted

An epidemiological model for West Nile virus: invasion analysis and control applications.

Infectious diseases present ecological and public health challenges that can be addressed with mathematical models. Certain pathogens, however, including the emerging West Nile virus (WN) in North America, exhibit a complex seasonal ecology that is not readily analysed with standard epidemiological methods. We develop a single-season susceptible-infectious-removed (SIR) model of WN cross-infection between birds and mosquitoes, incorporating specific features unique to WN ecology. We obtain the disease reproduction number, R0, and show that mosquito control decreases, but bird control increases

Temporal dynamics of mortality risk from short-term air pollution exposure in Canada: An epidemiological analysis using time-varying non-linear models.

BACKGROUND: Short-term exposure to air pollution is linked to adverse health outcomes, yet most studies assume constant associations over time and focus on densely populated regions. Evidence on temporal changes and impacts in less populated areas remains limited. OBJECTIVE: We aimed to (1) estimate non-linear associations between mortality and fine particulate matter (PM2.5), as well as between mortality and a mixture of PM2.5, ground-level ozone (O3) and nitrogen dioxide (NO2), (2) characterize non-linear temporal trends, (3) expand analysis beyond the most populated regions, and (4) assess

Use of Large Language Models to Classify Epidemiological Characteristics in Synthetic and Real-World Social Media Posts About Conjunctivitis Outbreaks: Infodemiology Study.

BACKGROUND: The use of web-based search and social media can help identify epidemics, potentially earlier than clinical methods or even potentially identifying unreported outbreaks. Monitoring for eye-related epidemics, such as conjunctivitis outbreaks, can facilitate early public health intervention to reduce transmission and ocular comorbidities. However, monitoring social media content for conjunctivitis outbreaks is costly and laborious. Large language models (LLMs) could overcome these barriers by assessing the likelihood that real-world outbreaks are being described. However, public heal

Avian influenza virus cross-infections as test case for pandemic preparedness: From epidemiological hazard models to sequence-based early viral warning systems.

Pandemic preparedness starts with an early warning system of viruses with a pandemic potential. Based on information collected in a multitude of surveys, hazard models were developed identifying influenza viruses presenting a pandemic threat. Scores are attributed for 10 viral traits by expert panels which identified avian influenza viruses (AIV) belonging to subtypes H7N9 and H5N1 as representing the greatest pandemic risk. In 2013, more than 100 human cases infected with AIV H7N9 were observed in China. Case fatality rate (CFR) was high (27%), but the human-to-human transmission rate was low

SEIRV epidemiological model for COVID 19 with Holling type II functional response.

Data-driven mathematical models are recognized as a significant tool for understanding infectious disease dynamics and have been valuable in public health research for an extended period of time. In this paper, we develop an SEIRV model that includes susceptible, exposed, infectious, and vaccinated populations to investigate the dynamics of the SARS-CoV-2 virus. We incorporate a Holling type-II functional response in our model, which has been widely useful in studying ecological systems. This approach enables us to conduct comprehensive analysis by integrating analytical, computational, and st

Integrating epidemiological modeling and time-series forecasting to optimize pandemic patient allocation.

BACKGROUND: This paper addresses the challenge of allocating patients to healthcare facilities with limited capacities during infectious disease outbreaks. The method is based on a hierarchical time-series model to forecast hospital bed demand and a mixed-integer nonlinear mathematical model for allocating patients among a set of regions to minimize disease spread. METHODS: Our contributions include (1) a new hierarchical time-series model for forecasting hospital bed demand to enhance regional predictive accuracy, (2) a mathematical model that integrates the forecasting model with a Susceptib

Deterministic and ANN-based modelling of measles transmission using real epidemiological data.

This study aims to develop a nonlinear SVLIQR epidemic model to investigate measles transmission dynamics by incorporating vaccination, latency, quarantine, and treatment effects. The qualitative properties of the model, including positivity and boundedness of solutions, are established. The basic reproduction number is derived using the next-generation matrix method, and the existence of disease-free and endemic equilibrium points is examined. The local stability of the disease-free equilibrium is analysed using the Routh-Hurwitz criterion, while global stability is studied through the Castil

Epidemiological and Clinical Behavior of Snakebite in the Pediatric Population, Associated with a Logistic Regression Model.

Introduction: Snakebite envenomation is recognized by the World Health Organization (WHO) as a neglected tropical disease. In Colombia, snakebites are frequent due to the diversity of ecosystems and snake species, and children represent a particularly vulnerable population. Objective: This study aimed to characterize the epidemiological and clinical behavior of snakebite envenomation in the pediatric population and to identify factors associated with its severity through the application of a multinomial logistic regression model. Methods: An exploratory analysis was conducted on 170 pediatric

Development of a dynamical model to enhance understanding of epidemiology of schistosomiasis in school-aged-children.

School-aged-children are most vulnerable to schistosomiasis as exemplified by age-prevalence profiles although current understanding of these patterns needs improvement. Using epidemiological data from the southern shoreline of Lake Malawi, we investigated the dynamics of Schistosoma transmission and the main determinants of Schistosoma infection risk using a transmission dynamic model, considering urogenital and intestinal schistosomiasis respectively. Specifically, we assessed whether the proximity of primary schools to the immediate shoreline was a major geospatial and epidemiological deter

Global, regional and national epidemiology of alopecia areata: a systematic review and modelling study.

BACKGROUND: Alopecia areata (AA) is a chronic autoimmune disease that leads to a high psychiatric, economic and systemic disease burden. A comprehensive understanding of AA epidemiology is essential for evaluating healthcare source utilization; however, a systematic approach to summarizing epidemiological data on AA is lacking. OBJECTIVES: To investigate systematically the global, regional and national incidence and prevalence of AA. METHODS: A structured search was conducted using the databases MEDLINE, Embase, Cochrane Library, Web of Science, SciELO and Korean Journal Database from their

A prevalence-based transmission model for the study of the epidemiology and control of soil-transmitted helminthiasis.

Much effort has been devoted by the World Health Organization (WHO) to eliminate soil-transmitted helminth (STH) infections by 2030 using mass drug administration targeted at particular risk groups alongside the availability to access water, sanitation and hygiene services. The targets set by the WHO for the control of helminth infections are typically defined in terms of the prevalence of infection, whereas the standard formulation of STH transmission models typically describe dynamic changes in the mean-worm burden. We develop a prevalence-based deterministic model to investigate the transmi

Mind your Ps: A probabilistic model to aid the interpretation of molecular epidemiology data.

BACKGROUND: Assessing relatedness of pathogen sequences in clinical samples is a core goal in molecular epidemiology. Tools for Bayesian analysis of phylogeny, such as the BEAST software package, have been typically used in the analysis of sequence/time data in public health. However, they are computationally-, time-, and knowledge-intensive, demanding resources that many laboratories do not have available or cannot allocate frequently. METHODS: To evaluate a faster and simpler alternative method to support the routine interpretation of sequence data for epidemiology, we obtained sequences fo

Epidemiology, Management, Quality of Testing and Cost of Syphilis in Germany: A Retrospective Model Analysis.

BACKGROUND: A multi-dimensional model can be a useful tool for estimating the general impact of disease on the different sectors of the healthcare system. We chose the sexually transmitted disease syphilis for our model due to the good quality of reported data in Germany. METHODS: The model included gender- and age-stratified incident cases of syphilis (in- and outpatients) provided by a German statutory health insurance company, as well as seroprevalence data on syphilis in first-time blood donors. Age standardized rates were calculated based on the standard German population. The test quali

Quest for Optimal Regression Models in SARS-CoV-2 Wastewater Based Epidemiology.

Wastewater-based epidemiology is a recognised source of information for pandemic management. In this study, we investigated the correlation between a SARS-CoV-2 signal derived from wastewater sampling and COVID-19 incidence values monitored by means of individual testing programs. The dataset used in the study is composed of timelines (duration approx. five months) of both signals at four wastewater treatment plants across Austria, two of which drain large communities and the other two drain smaller communities. Eight regression models were investigated to predict the viral incidence under var

Effect of change in vaccine schedule on pertussis epidemiology in France: a modelling and serological study.

BACKGROUND: In April-May, 2013, France modified its pertussis vaccination schedule, which uses the acellular pertussis vaccine, from three primary doses at 2, 3, and 4 months of age and a first booster at 16-18 months of age (former schedule) to two primary doses at 2 and 4 months of age and a first booster at 11 months of age (new schedule). We aimed to assess the subsequent effect of the vaccine schedule change on pertussis epidemiology in France. METHODS: In this modelling study, using data collected between Jan 1, 2012, and Dec 31, 2019, from French national surveillance sources, we analy

Neonatal Abstinence Syndrome: Review of Epidemiology, Care Models, and Current Understanding of Outcomes.

The incidence of neonatal abstinence syndrome owing to prenatal opioid exposure has grown rapidly in recent decades and it disproportionately affects rural, non-white, and public insurance-dependent populations. Treatment consists of pharmacologic and nonpharmacologic interventions with wide variability in approaches across the United States. Standardizing clinical assessment, minimizing unnecessary interruptions, and prioritizing nonpharmacologic and family-centered care seems to improve hospital outcomes. Neonatal abstinence syndrome may have long-term developmental and biological effects, b
