Anteromesial temporal lobectomy, including surgical resection of the mesial temporal structures, is an important surgical procedure for the treatment of medically refractory temporal lobe epilepsy. Given the widespread use of this technique in appropriately screened patients with intractable focal epilepsy (not to mention other non-epileptic neurosurgical conditions, including brain tumors and vascular lesions), it is important for the treating neurosurgeon to have a comprehensive understanding of the complex anatomy and surgical technique for performing a successful resection. Here, we descri

BACKGROUND: Temporal lobe epilepsy is one of the most common types of partial epilepsy. Although surgical treatment has led to significant improvements in seizure-free rates, nearly one-third of patients still have poor seizure control after surgery. Moreover, the long-term outcome is less favorable compared to short-term outcome, with 48-58% of patients experiencing seizures five years after surgery. The aim of this study was to investigate the surgical outcomes and the predictive value of prognostic factors associated with poor surgical outcomes in temporal lobe epilepsy patients receiving s

Executive function (EF) deficits are common in epilepsy and impact quality of life. This study compares EF in idiopathic generalized epilepsy (IGE) and temporal lobe epilepsy (TLE) patients to healthy controls. Fifty-six IGE patients, 56 TLE patients, and 60 controls (matched by age) completed cognitive tests assessing attention, memory, learning, and verbal fluency. Both epilepsy groups performed worse than controls, with TLE patients showing significantly poorer verbal learning and memory compared to IGE patients. These findings suggest that TLE patients have more severe EF impairments, like

Temporal encephaloceles (TEs) are one of the cause of refractory temporal lobe epilepsy (TLE). We reviewed the neuroimaging and video-electroencephalography (EEG) records of epilepsy patients who underwent temporal lobectomy in our center to investigate frequency of TEs. We retrospectively reevaluated 294 patients who underwent epilepsy surgery in our tertiary epilepsy centre between January 2010 and March 2019 and included 159 patients (78 females, 49 %; 81 males) who had temporal lobectomy. Preoperatively, TEs were reported in 3 of 159 patients (1 female, 2 males). After reevaluation 4 more

Mesial temporal lobe epilepsy (TLE) is characterized by transient abnormal electrical activity originating in the hippocampus. The objective of this study was to characterize dynamic spatio-temporal fluctuations in hippocampal network connectivity in mTLE using functional connectivity (FC) mapping in 41 unilateral mTLE patients (28 right, 13 left) and 56 healthy control participants using 3T MRI. Dynamic FC was computed across the scan using sliding 60-s windows. This was compared to static FC computed using the whole 10-min functional MRI scan, and to the variance in the hippocampal functiona

Reasons for failed temporal lobe epilepsy surgery remain unclear. Temporal plus epilepsy, characterized by a primary temporal lobe epileptogenic zone extending to neighboured regions, might account for a yet unknown proportion of these failures. In this study all patients from two epilepsy surgery programmes who fulfilled the following criteria were included: (i) operated from an anterior temporal lobectomy or disconnection between January 1990 and December 2001; (ii) magnetic resonance imaging normal or showing signs of hippocampal sclerosis; and (iii) postoperative follow-up ≥ 24 months for

OBJECTIVE: To determine whether there is MRI-detectable mesial temporal sclerosis (MTS) in patients with sporadic benign temporal lobe epilepsy (BTLE).
METHODS: Brain MRIs were obtained from 101 consecutive, unrelated patients (51 women; mean age 37.3 +/- 17.5 years; range 10 to 83 years) with BTLE, who reported rarely or never having had seizures at the time of long-term (> 2 years) follow-up. The mean age at seizure onset was 22.3 +/- 17.4 years; the mean duration of epilepsy was 16.4 +/- 14.1 years. MRI diagnosis of MTS was based on the occurrence of hippocampal formation atrophy on T1 sli

OBJECTIVES: Psychopathology has been reported to be prevalent both before and after surgical treatment for medically intractable temporal lobe epilepsy. Individual patients were evaluated prospectively to assess the effect of anterior temporal lobectomy (ATL) on prevalence and severity of psychiatric disease.
METHODS: Psychiatric status was assessed in a consecutive series of epilepsy patients before and 6 months after ATL using a structured psychiatric interview, psychiatric rating scales, and self report mood measures.
RESULTS: A DSM-III-R axis I diagnosis was present in 65% of patients be

OBJECTIVE: Based on the promising results of randomized controlled trials, deep brain stimulation (DBS) and responsive neurostimulation (RNS) are used increasingly in the treatment of patients with drug-resistant epilepsy. Drug-resistant temporal lobe epilepsy (TLE) is an indication for either DBS of the anterior nucleus of the thalamus (ANT) or temporal lobe (TL) RNS, but there are no studies that directly compare the seizure benefits and adverse effects associated with these therapies in this patient population. We, therefore, examined all patients who underwent ANT-DBS or TL-RNS for drug-re

Robotic systems have fundamentally altered the landscape of functional neurosurgery. These allow automated stereotaxy with high accuracy and reliability, and are rapidly becoming a mainstay in stereotactic surgeries such as deep brain stimulation (DBS), stereoelectroencephalography (SEEG), and stereotactic laser ablation/MRI guided laser interstitial thermal therapy (MRgLITT). Robotic systems have been effectively applied to create a minimally invasive approach for diagnostics and therapeutics in the treatment of epilepsy, utilizing robots for expeditious and accurate stereotaxy for SEEG and M

This is a case report of a 55-year-old man with medication refractory right temporal lobe epilepsy since adolescence. He was found to have bilateral posterior cerebral calcifications on routine head computed tomography with confirmation on magnetic resonance imaging. He also had elevated antibody markers for celiac disease. He was diagnosed with the rare, but well-described syndrome of celiac disease, epilepsy, and cerebral calcifications (CEC). He failed a brief trial of gluten-free diet and went on to have a right temporal lobectomy with sustained freedom from disabling seizures. This case i

Memory decline after anterior mesial temporal resection in mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS) remains incompletely explained by structural and clinical factors, suggesting that networks may contribute to cognitive vulnerability. We investigated whether presurgical metabolic connectivity (MC), derived from fluorodeoxyglucose positron emission tomography (FDG-PET) and reflecting metabolic network organization, is associated with memory performance and postsurgical cognitive outcome in MTLE-HS. We studied 83 patients with MTLE-HS who underwent anterior mesial tempo

BACKGROUND: Self-stigma in epilepsy is conceptualized as enacted and felt stigma. Individuals with temporal lobe epilepsy (TLE) often face challenges that affect their self-perception and social engagement. This study explored the factors contributing to enacted and felt stigma among patients with TLE, considering both objective and subjective cognitive functions alongside biopsychosocial factors as explanatory variables.
METHODS: The analysis included a total of 249 patients: 169 with TLE (90 females, 18-75 years) and with temporal lobe epilepsy (TLE) and 80 with other types of epilepsy (40

INTRODUCTION: Autoimmune polyglandular syndromes (APS) encompass a broad spectrum of disorders. APS III is characterized by autoimmune thyroiditis (AT) plus any autoimmune disorder, excluding Addison's disease.
METHODS AND RESULTS: This study explores the link between GAD65 related-temporal lobe epilepsy (GAD-TLE) and APS III. We retrospectively analyzed 29 GAD-TLE patients. Eighteen patients (62%) experienced at least one autoimmune disease, and ten patients (56%) fulfilled diagnostic criteria for APS III. In addition to AT and GAD-TLE, five patients had type 1 diabetes mellitus, one patient

OBJECTIVES: Previous research has linked epilepsy of the temporal lobe (TLE) with comorbidities of psychiatric nature, yet few studies have investigated personality psychopathology in patients with TLE. This is the first study on the topic in the Arab world and the first study using the Personality Inventory for DSM-5-Brief From (PID-5-BF). Thus, this study, which is cross-sectional by nature, aims to explore the potential relationship between TLE and personality psychopathology.
METHODS: Utilizing the validated Arabic version of the Personality Inventory for DSM-5 - Brief Form (PID-5-BF), th

BACKGROUND: Impaired processing speed (PS) can affect patients with temporal lobe epilepsy (TLE). However, it is usually considered a nonspecific clinical feature and is not measured, but this raises lexical and methodological problems. This review aims to evaluate the existing terminology and assessment methods of PS in patients with TLE.
METHODS: A scoping review was conducted based on the extended guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis. The electronic literature search was conducted on Medline-PubMed, American Psychological Association-PsycINFO
