Different antithrombotic strategies after left atrial appendage closure with the LACbes occluder: protocol of the DAAL trial.

INTRODUCTION: The purpose of this study is to evaluate the safety and efficacy of two different antithrombotic strategies, dual antiplatelet therapy (DAPT, aspirin and ticagrelor) and direct oral anticoagulant (DOAC, rivaroxaban), after left atrial appendage closure (LAAC) using the LACbes occluder. METHODS AND ANALYSIS: This study is a prospective, randomised, controlled and multicentre clinical trial that will compare the clinical efficacy of antiplatelet and anticoagulation therapy after LAAC with the LACbes occluder. It is planned to enrol 296 subjects with non-valvular atrial fibrillatio

One-stage versus two-stage video-assisted thoracic surgery for synchronous bilateral pulmonary nodules: protocol for a prospective, multicentre, randomised controlled clinical trial in China (OTVATS-2).

INTRODUCTION: With the increasing detection of synchronous bilateral pulmonary nodules in clinical practice, both one-stage and two-stage video-assisted thoracic surgery (VATS) have been widely applied. This study protocol describes the OTVATS-2 trial, which aims to compare perioperative complications and long-term outcomes between these two surgical strategies. METHODS AND ANALYSIS: A prospective, multicentre, randomised controlled clinical trial will be conducted to compare the surgical outcomes of one-stage versus two-stage VATS in patients with synchronous bilateral pulmonary nodules. Decision-making and institutional support for ECMO and ventilator allocation during the COVID-19 pandemic: a nationwide cross-sectional survey of designated physicians at Japan's advanced critical care and emergency centers.

BACKGROUND: During the COVID-19 pandemic, physicians in Advanced Critical Care and Emergency Centers (ACCECs) were required to make ethically and emotionally challenging decisions regarding the allocation of extracorporeal membrane oxygenation (ECMO) and mechanical ventilation (MV). Although expert-group guidance existed, no legally binding triage criteria were established in Japan. How these physicians balanced clinical indicators with ethical considerations—and whether institutional support systems helped or hindered these decisions—remains insufficiently understood. METHODS: We conducted a

Building a library of acute traumatic spinal cord injury images across Canada: a retrospective cohort study protocol.

INTRODUCTION: MRI is increasingly recognised as a valuable tool for assessing prognosis and predicting outcomes following traumatic spinal cord injury (SCI). Several potential MRI biomarkers have been identified, but efforts are still needed to improve the accuracy and feasibility of these biomarkers in clinical practice. This study aims to build a national Canadian SCI imaging repository for storing and analysing imaging data for SCI, with the goal of improving SCI MRI biomarkers to predict outcomes and inform clinical management. METHOD AND ANALYSIS: As a substudy of the Rick Hansen SCI Reg

'When I had concerns about my own patients…I was told to keep quiet': experiences of moral injury among healthcare workers under Covid-19 vaccine mandates in Alberta, Canada.

OBJECTIVES: Although compliance is often interpreted as consensus and dissent as "vaccine hesitancy", misinformation, or deficient trust, less is known about how healthcare workers experienced Covid-19 vaccine mandates or how their legitimacy was constructed and contested. METHODS: We analyzed open-ended survey responses from 80 respondents within a convenience sample of 189 healthcare workers in Alberta, Canada. Informed by Weberian ideal types, responses were classified by normative orientation toward mandates and analyzed thematically. RESULTS: Most respondents (82.5 %) expressed negative

Comparison of the effects of norepinephrine and phenylephrine on shivering and hypothermia in patients undergoing caesarean section under spinal anaesthesia at a tertiary hospital in China:a randomised, double-blind, controlled trial protocol.

INTRODUCTION: Peripheral vasodilation causes a redistribution of body temperature from the core to the periphery, resulting in shivering and hypothermia. These are normal pathological and physiological processes during spinal anaesthesia. Two drugs, norepinephrine and phenylephrine, have peripheral vasoconstrictive effects. It is unclear the effects of norepinephrine and phenylephrine on shivering and hypothermia in patients undergoing caesarean section under spinal anaesthesia. METHODS ANALYSIS: 240 eligible parturients will be recruited for this randomised, double-blind, controlled trial an

Protocol for the cost-effectiveness of parenteral iron and oral iron therapy for first-line management of anaemia among pregnant women in a natural programme setting in Gujarat.

INTRODUCTION: Maternal anaemia is a major public health issue in India. The government of India recommends parenteral iron to manage moderate and severe grades of anaemia. In contrast to its clinical efficacy, the cost-effectiveness of intravenous iron sucrose and ferric carboxymaltose is not yet established in Indian context. This article illustrates the protocol of health technology assessment to evaluate the cost-effectiveness of intravenous therapy on the improvement of haemoglobin concentration over oral therapy. METHODS AND ANALYSIS: The study will be carried out in two districts of Guj

An ethical analysis of medication treatment for opioid use disorder (MOUD) for persons who are incarcerated.

Opioid use disorder (OUD) is highly prevalent among persons who are incarcerated. Medication treatment for opioid use disorder (MOUD), methadone, buprenorphine, and naltrexone, is widely used to treat OUD in the community. Despite MOUD's well-documented effectiveness in improving health and social outcomes, its use in American jails and prisons is limited.Several factors are used to justify limited access to MOUD in jails and prisons including: "uncertainty" of MOUD's effectiveness during incarceration, security concerns, risk of overdose from MOUD, lack of resources and institutional infrastr

Prospective multicentre open-label randomised controlled trial of 3-month versus 12-month dual antiplatelet therapy after implantation of the new generation biodegradable polymer sirolimus TARGET-eluting coronary stent: protocol of the TARGET DAPT trial.

INTRODUCTION: Dual antiplatelet therapy (DAPT) with aspirin and thienopyridine is required after placement of coronary stents to prevent thrombotic complications. However, current recommendation for duration of DAPT remains controversial. Firehawk is a biodegradable polymer applied to recessed abluminal grooves, sirolimus target-eluting stent associated with early excellent healing response and almost complete strut coverage, as well as possibly reduced myocardial ischaemic events. But the optimal DAPT duration for such a new generation stent is less known. Therefore, the present trial seeks t

Pathogen genomic surveillance in health care: emerging ethical dilemmas in outbreak detection and patient notification.

Genomic surveillance detects healthcare-associated outbreaks that are often missed by traditional methods, offering substantial benefits for infection prevention. However, its use introduces complex ethical questions regarding patient notification and disparities in outbreak detection between institutions with and without genomic capabilities. In this commentary, we apply the Centers for Disease Control and Prevention's 2020 ethical considerations regarding patient notification and the Ethical Infection Prevention and Control (EIPAC) framework to two hypothetical scenarios involving genomic su

Teaching OB/GYN residents bioethics within a Catholic healthcare context.

Residents entering training in the specialty of Obstetrics and Gynecology (OB/GYN) often have misconceptions as to what medical interventions Roman Catholic healthcare institutions prohibit, and why certain restrictions are placed on the provision of reproductive health options that are otherwise legally available to women. The Ethical and Religious Directives for Catholic Healthcare Services, produced by the United States Conference of Catholic Bishops seeks to provide a stable framework upon which reproductive health decisions can be based. However, Catholic healthcare ethics may conflict wi

Public involvement and engagement in scientific research and higher education: the only way is ethics?

BACKGROUND: Involving and engaging the public in scientific research and higher education is slowly becoming the norm for academic institutions in the United Kingdom and elsewhere. Driven by a wide range of stakeholders including regulators, funders, research policymakers and charities public involvement and public engagement are increasingly seen as essential in delivering open and transparent activity that is relevant and positively impacts on our society. It is obvious that any activities involving and engaging members of the public should be conducted safely and ethically. However, it is n

Effectiveness of a participatory approach to develop school health interventions in four low resource cities: study protocol of the 'empowering adolescents to lead change using health data' cluster randomised controlled trial.

INTRODUCTION: Comprehensive local data on adolescent health are often lacking, particularly in lower resource settings. Furthermore, there are knowledge gaps around which interventions are effective to support healthy behaviours. This study generates health information for students from cities in four middle-income countries to plan, implement and subsequently evaluate a package of interventions to improve health outcomes. METHODS AND ANALYSIS: We will conduct a cluster randomised controlled trial in schools in Fez, Morocco; Jaipur, India; Saint Catherine Parish, Jamaica; and Sekondi-Takoradi

Implications of Critical Perspectives for Psychiatric Practice.

"Critical psychiatry" encompasses a diverse spectrum of critiques challenging foundational assumptions and practices within psychiatry, emphasizing the role of power, relational dynamics, and sociocultural contexts in understanding psychological distress and disability. We provide an overview of these topics relevant to medical ethics. Major themes explored here include critiques of biological reductionism, psychiatric classification and diagnosis, overmedicalization, models of psychopharmacology, iatrogenic harm, institutional corruption, and epistemic injustice. The article advocates for a c

Efficacy of zinc supplementation for young infants with clinical severe infection in Tanzania: study protocol for a randomised controlled trial.

INTRODUCTION: Innovative interventions will be essential for countries in sub-Saharan Africa to achieve the 2030 Sustainable Development Goal for child mortality. Infections among young infants, including sepsis, meningitis and pneumonia, continue to cause a large burden of morbidity and mortality in low-income and middle-income countries. Zinc is an essential micronutrient with a well-established role in human health and immune system function, and supplementation may improve survival and treatment outcomes for infants with bacterial infections. METHODS AND ANALYSIS: We will conduct an indiv

Case studies in transplant ethics.

Clinicians and other health care professionals involved in the evaluation and management of organ transplant recipients and organ donors quite commonly encounter ethically complex scenarios during the course of their work. The response to such challenging situations may be influenced by personal experiences and beliefs, programmatic or institutional policy, existing legislation, published literature, and consultative analysis by experts in the field of biomedical ethics. As organ transplantation continues to evolve in its scope, complexity, and global reach, there is a growing need for transpl
