Trans-superior meatal surgery: A targeted approach for isolated posterior ethmoid diseases.

BACKGROUND: Surgical treatment for posterior ethmoid diseases has historically been performed through a trans-ethmoid approach which usually required medialization of the middle turbinate, a middle meatal antrostomy and total ethmoidectomy. This can destabilize the basal lamella of the middle turbinate and also sacrifices the integrity of ostiomeatal complex and the healthy bulla if the patient has the disease only in the posterior ethmoid sinus. The aim of this study is to present of a novel minimally disruptive approach for the management of isolated posterior ethmoid diseases. METHODS: Ret

Association between the insertion type of the uncinate process and the development of Frontal Sinus Mucoceles - is there a relationship?

BACKGROUND: The frontal sinus drainage pathway is formed by either the ethmoid infundibulum or the middle meatus, which depends on the superior insertion of the uncinate process. According to Landsberg & Friedman the are 6 types of superior uncinate process insertion. The aim of this study is to find an association between the uncinate process insertion type and the development of frontal sinus mucoceles. METHODS: Fifty sinus CT scans were analysed. Exclusion criteria were previous nasal surgeries, malignancy, polyposis and an insufficient image quality. Superior insertions of the uncinate pr

Understanding the formation of maxillary sinus in Japanese human foetuses using cone beam CT.

The formation of the maxillary sinus (MS) is tied to the maturation of the craniofacial bones during development. The MS and surrounding bon … fically surrounded the deciduous tooth germs. By 30 weeks of gestation, the uncinate process of the ethmoid bone formed a border with the maxilla. The distance from the midline to the maximum lateral surface border of the MS combined with the width from the midline to the maximum lateral surface border of the inferior nasal concha showed a high positive correl …

Severe combined immunodeficiency pig as an emerging animal model for human diseases and regenerative medicines.

The supraorbital ethmoid air cell (SOEC) is an anatomical variation of the ethmoid air cell system that pneumatizes the orbital plate of the frontal bone. It affects the frontal recess configuration by opening posterior and lateral to the frontal sinus ostium. This cross-sectional observational study includes 100 patients with Supra orbital ethmoid air cells including various pathologies. We picked up the first 100 patients who had SOEC in all the CT PNS done for various sinonasal pathologies. The incidence of the cell was about 43%. The most common type was the cell extending up to the medial

Review of olfactory cleft roof anatomy.

Progress in skull-base endoscopic surgery has improved our knowledge of olfactory cleft (OC) anatomy. This article presents a review of current knowledge concerning the OC roof: descriptive anatomy, radiological exploration, and endoscopic observation. The OC is a narrow area in the most superior part of the nasal cavity. Its roof is the thinnest structure separating the nasal cavities from the brain; it comprises three superimposed tissues: nasal mucosa, ethmoid cribriform plate (ECP), and dura mater. The ECP comprises the anterior ECP containing the ethmoidal slit (ES) medially and the cribr

The uncinate process: a histological and morphological study.

The uncinate process (UP) has become a familiar anatomical landmark to otolaryngologists due to the increasing popularity of functional endoscopic sinus surgery (FESS) in recent years. This study examines the histological and morphological features of UPs removed from a group of patients undergoing FESS operations for chronic sinusitis. The structure of the UP by light microscopy (LM) is reported. A relatively high proportion of goblet cells and seromucous glands was noted. Scanning electron microscopy (SEM) identified four surface cell types on the UP. Furthermore, four categories of surface

Inverted papilloma with osteogenesis in the anterior ethmoid and frontal sinuses.

OBJECTIVE: We report a rare case of new bone formation (osteogenesis) within an inverted papilloma, occurring in the ethmoid and frontal sinuses. METHOD: The histopathological and radiological findings, differential diagnosis, and treatment of tumour-induced osteogenesis are discussed. RESULTS: A 68-year-old man complained of headache and left-sided, purulent rhinorrhoea of five years' duration. On nasal endoscopy, lesions similar to inflammatory nasal polyps were seen. Pre-operative radiological images demonstrated opacification of the left nasal cavity and anterior ethmoid and frontal sinu

[Mucopyocele of the ethmoidal labyrinth].

An isolated affection of a single sinus is a rare occurrence in patients presenting with chronic sinusitis (especially in children). Clinically, chronic ethmoiditis usually has the form of a cattarhal (not infrequently latent) inflammatory process. When the opening of the sinus is closed during a long time, its accumulated contents cause the bone walls to distend and become thinner. A large amount of mucous exudate leads to the development of mucocele (pyocele). A case of mucopyocele in the ethmoidal labyrinth in a 7 year-old girl is reported in conjunction with the results of its treatment.

[Phinocerebral mucormycosis--report of a case].

Rhinocerebral mucormycosis is a rare and fatal deep fungus infection occurring in debilitated patients. Its reports have been recently increased because of the increase of such conditions as uncontrolled diabetics, leukemia, and cancer. A 60-year-old male suffering from continuous headache and fever was diagnosed as meningitis, and repeated culture of cerebrospinal fluid was negative. Antibiotic therapy was ineffective. CT scan revealed brain abscess in the right frontal lobe and bone defect over the right posterior ethmoidal sinuses. Both maxillary and ethmoidal sinuses were operated on 38 ye

Clinical anatomy of the nasal process of the frontal bone (spina nasalis interna).

OBJECTIVES: During endonasal frontal sinusotomy with the sharp spoon, a solid piece of bone is frequently encountered anterior to the neo-ostium. This bone may be referred to as the nasal process of the frontal bone or internal nasal spine (spina nasalis interna). A prominent spina may render an extended sinusotomy difficult and may call for use of the drill. A series of anatomic measurements is presented to illustrate the 3-dimensional anatomy of the spina nasalis and the regional anatomy. METHODS: A maximum endonasal frontal sinusotomy (Draf type I-II procedure, nasofrontal approach type II

Bone: the final frontier for Staphylococcus aureus penetration in chronic rhinosinusitis.

UNLABELLED: The superantigenic properties of Staphylococcus aureus have been implicated in increasing the inflammatory process in airway diseases. Local formation of IgE antibodies against staphylococcal enterotoxins by secondary lymphoid tissue in nasal polyps has been demonstrated. Staphylococcus aureus is known to colonize the nasal mucosa, and has been found invading the nasal submucosa and intracellularly. OBJECTIVE: To evaluate the limits of Staphylococcus aureus invasion in the upper airway. MATERIAL AND METHODS: Inferior turbinate samples from 3 patients without sinus disease, 6 ethm

[Transnasal endoscopic surgery].

We have performed operations of transnasal endoscopic surgery successfully. This study was intended to analyze and discuss the method of operation on the ostium meatus unit, the removal of polyps in sphenoethmoidal recess or superior turbinate and our viewpoint of ethmoid sinus operation. We conclude that (1) the 70 degree nasal endoscope is the main instrument in endoscopy; (2) an endoscope and a head lamp should be used alternatively in order to proceed the operation quickly and smoothly; (3) the bone wall should not be broken rashly and a good drainage should be the main purpose in ethmoid
