Necrotizing fasciitis and mediastinitis.

Necrotizing fasciitis and mediastinitis are two rare but grave complications of orofacial infections. The clinical presentation, diagnostic modalities, anatomic pathways, microbiologic make-up, surgical management, and the use of adjuvant HBO therapy for both entities have been discussed.

[Retroperitoneal postoperative necrotizing fasciitis].

This is a report on clinical experience had with 17 patients presenting necrotizing fasciitis--a complication ever more frequently encountered. The case material is distributed in two group differing by origin and clinical course of the complication. In group one (n = 11) it is a matter of postoperative development of postoperative complication, consistent with the classical "per continuitatem" and "per contiguitatem" mechanisms, while in group two (n = 6) the process originates, evolves and speads within the retroperitoneal space proper. Comprehensive microbiological examinations performed in

Necrotizing fasciitis: CT characteristics.

PURPOSE: To establish computed tomographic (CT) criteria for the diagnosis of necrotizing fasciitis. MATERIALS AND METHODS: Twenty CT scans in 20 patients with pathologically proved necrotizing fasciitis were reviewed retrospectively for fascial thickening, fat infiltration, focal fluid collection, soft-tissue gas, muscle involvement, and intra-abdominal extension; the findings were correlated with clinical factors, including associated illnesses, disease site, treatment, and outcome. RESULTS: Average patient age was 57.8 years; there were 13 men and seven women. Four patients (20%) died. As

Necrotizing fasciitis of perineum.

Twenty-eight patients with necrotizing fasciitis of the perineum were treated at the Mount Sinai Medical Center from 1971 to 1979. Ten of the 28 died of the disease, for a mortality rate of 36%. The mortality rate was even higher when the lesion was located in the perianal area and associated with diabetes mellitus and delayed surgical intervention. Necrotizing fasciitis is a rare disease that involves both superficial and deep fascia. It begins with fascial necrosis at onset and rapidly progresses to surrounding fascial planes, eventually involving overlying skin and underlying muscle. Eviden

Necrotizing fasciitis of genitalia.

A review of 12 patients with necrotizing fasciitis of the genitalia is presented. All patients had rapidly progressing cellulitis and necrosis of the fascia caused by a combination of various organisms, most frequently by Escherichia coli. Beta streptococcus, which is generally thought to be the causative organism in these cases, was present in only 4 patients. Management consisted of extensive surgical debridement, urinary and/or fecal diversion, antibiotics, and necessary reconstructive procedures.

Necrotizing fasciitis of the finger.

A 62 year old woman with a medical history of hypertension presented to her local general practice with moderate pain in the right ring finger which worsened over a few hours after onset. Her condition progressed swiftly into sepsis and required hospital admission in the intensive care unit. Over the course of 24 h the patient was diagnosed with necrotizing fasciitis in the finger but unfortunately despite treatment with antimicrobials and surgical debridement the patient's finger could not be saved and she had amputation of her finger.

Necrotizing fasciitis of the breast.

Necrotizing fasciitis is a potentially fatal condition that can affect any part of the body. It can occur after trauma, around foreign bodies in surgical wounds, or can be idiopathic. We describe a case of necrotizing fasciitis involving the breast following an initial debridement of an inflammatory lesion.

The diagnosis of necrotizing fasciitis.

PURPOSE OF REVIEW: A delay in the diagnosis and appropriate treatment of necrotizing fasciitis has clearly been demonstrated to increase mortality. However, paucity of specific cutaneous signs makes early recognition extremely difficult. This review highlights recent developments in the approaches to the diagnosis of necrotizing fasciitis. RECENT FINDINGS: A clinical staging of necrotizing fasciitis is proposed to better define the progression of the disease. Several clinical subtypes of necrotizing fasciitis have been described recently with hyperacute and sub-acute variants. Imaging techniq

[Necrotizing fasciitis of the thigh].

INTRODUCTION: Necrotizing fasciitis is a rare and severe infectious disease with infectious necrosis often extending in depth to the subcutaneous tissue in the absence of rapid medical-surgical treatment. We report two cases of necrotizing fasciitis of the thigh, which underline the principle clinical and therapeutic characteristics of this often underrated disease. OBSERVATION: The first case concerns a young 21 year-old woman without remarkable medical history who, following injury from the thorn of a palm tree, developed an aerobic germ necrotizing fasciitis which regressed following medic

Necrotizing fasciitis of the extremities.

Necrotizing fasciitis is a limb- and life-threatening soft-tissue infection. Eighteen patients with necrotizing fasciitis of the extremities were reviewed. These infections occurred most commonly after minor trauma. Associated chronic debilitating diseases were present in 13 patients. All but two infections were polymicrobial. The overall mortality rate was 33%. Death was caused by persistent wound sepsis in three and systemic septic complications in spite of apparent local infection control in three. At the admission physical examination the condition may resemble a benign, low-grade cellulit

Necrotizing fasciitis of the neck.

Necrotizing fasciitis (N.F.) is a rare but serious infection of subcutaneous tissues and deep fascia with resulting skin gangrene and septicaemia. It is due to mixed anaerobic and aerobic organisms. It has been reported under a variety of synonyms. We describe two cases of necrotizing fasciitis of the neck probably secondary to chronic dental infection, one was diagnosed early and the other late with very different outcomes. We highlight the importance of early and aggressive surgical treatment to complement parenteral antibiotics covering both aerobic and anaerobic organisms.

Necrotizing fasciitis of the vulva.

Sixteen cases of necrotizing fasciitis arising from or involving the vulva are presented. Fifteen of the 16 women had diabetes mellitus. Five of them died. All but one of these cases were managed by very extensive excision of all involved tissues. These cases demonstrate the extreme seriousness of any infection involving the vulva in a diabetic patient. Such patients should have the infected area explored while they are under anesthesia as soon as possible. If any necrosis is found, radical excision to include removal of all necrotic, indurated, and pale tissues is mandatory.
