Managing dental fear and anxiety.

Fear and anxiety are primary reasons why individuals avoid visiting the dental practitioner. Dental anxiety appears to vary by type of treatment, with periodontic and endodontic patients reporting higher levels of anxiety than patients receiving restorative or prophylactic treatment. Parents who experience dental anxiety often pass along such feelings to their children. Front office employees are the first line in screening for anxiety-related behaviors while the patient is in the reception area. It is extremely important to recognize dental fear and anxiety before the patient receives oral ca…

Driving-related fear: a review.

This article reviews the research on driving-related fear (DRF). Until recently, research has concentrated almost exclusively on the effect of motor vehicle accidents (MVAs) on subsequent levels of DRF. However, recent findings have suggested that MVAs are not solely responsible for this fear reaction, and that non-MVA driving fear can be just as strong. Studies of the broader driving-fearful population have encountered difficulty with diagnostic conceptualisation of DRF, although some have investigated a possible typology of DRF. Driving skill has been a neglected issue in the DRF research, a…

[Pediatric dentistry and dental fear].

The development of dental anxiety is often related to dental experiences in the childhood. A. study (3) from Copenhagen shows that among 20-21-year-old Danes, 37% had a negative assessment of child dental care and 14% had phobic reactions to dental care. The article examines factors which could have influence on the psychologic atmosphere in the clinic and in this way the psychologic reactions of the child. Some factors are not specifically related to the dentist, such as the availability of parent/child's choice of dentist, the psychologic interaction in the treatment triangle, the specific i

High ovarian hormones present during fear extinction reduce fear relapse through a nigrostriatal dopamine pathway.

BACKGROUND: Elevated ovarian hormones during fear extinction can enhance fear extinction memory retention and reduce fear renewal, but the mechanisms remain unknown. High levels of ovarian hormones are associated with heightened dopamine (DA) transmission, a key player in fear extinction. In males, stimulation of substantia nigra (SN) DA neurons during fear extinction reduces renewal; an effect mimicked by DA D1 receptor agonist administration into the dorsolateral striatum (DLS), a primary target of the SN. The current studies tested the role of the SN-DLS pathway in estrous cycle-modulation …

Fear in the context of pain: Lessons learned from 100 years of fear conditioning research.

Human fear conditioning research since Watson's case study on "Little Albert" has vastly evolved and its impact today is reaching far beyond phobic anxiety. This review focuses on how fear conditioning research, mainly using exteroceptive conditioned stimuli (CSs) and aversive, non-noxious stimuli as unconditioned stimuli (USs), has been extended and translated to chronic pain research. We describe the different pain-related fear conditioning paradigms using proprioceptive and interoceptive CSs and painful stimuli as USs that have been developed to study specific forms of pain-related fear (i.…

Activation of medial orbitofrontal cortex abolishes fear extinction and interferes with fear expression in rats.

Pavlovian fear conditioning and extinction procedures have long been used to study the regulation of learned fear. The amygdala is vital for the association of cues and fear expression, whereas the medial prefrontal cortex (mPFC) is critical for fear regulation after extinction. The medial orbitofrontal cortex (mOFC) has an extensive connection with the fear circuit. In human studies, emotional regulation disorders, such as obsessive-compulsive disorder and post-traumatic stress disorder, are often linked to an abnormality in the orbitofrontal cortex (OFC). Therefore, in a series of experiment…

We prefer what we fear: A response preference bias mimics attentional capture in spider fear.

The extent to which emotionally significant stimuli capture visual attention remains elusive because a preference for reporting or choosing emotionally significant stimuli could mimic attentional capture by these stimuli. We conducted two prior-entry experiments to disentangle whether phobic and fear-relevant stimuli capture attention or merely produce a response bias in spider-fearful participants. Prior entry denotes the effect that attended stimuli are perceived earlier than unattended stimuli as indicated by temporal order judgments. We presented phobic (spiders), fear-relevant (snakes) an…

Enhancing Fear Extinction: Pharmacological Approaches.

Extinction is the process by which the memory of a learned conditioned association decreases over time and with introduction of new associations. It is a vital part of fear learning, and it is critical to recovery in multiple fear-related disorders, including Specific and Social Phobias, Panic Disorder, Obsessive Compulsive Disorder (OCD), and Posttraumatic Stress Disorder (PTSD). The process of extinction is also the underlying mechanism for recovery in gold-standard therapies for PTSD, including prolonged exposure, cognitive processing therapy, eye movement desensitization and procession, as…

Ictal fear during parietal seizures.

Ictal fear is characterized by a subjective sensation of fear and consistent clinical manifestations during seizures. This phenomenon is rarely observed in parietal seizures. We report anatomical electroclinical correlations between an SEEG-recorded seizure and prominent fear semiology. The seizure onset zone was quantified using the Connectivity Epileptogenicity Index (cEI) method. Occurrence of fear during seizures was related to the involvement of the left inferior parietal cortex and the superior temporal gyrus without amygdala involvement. Our case confirms that parietal seizures can prod…

Prophylactic Ketamine Attenuates Learned Fear.

Ketamine has been reported to be an efficacious antidepressant for major depressive disorder and posttraumatic stress disorder. Most recently, ketamine has also been shown to be prophylactic against stress-induced depressive-like behavior in mice. It remains unknown, however, when ketamine should be administered relative to a stressor in order to maximize its antidepressant and/or prophylactic effects. Moreover, it is unknown whether ketamine can be prophylactic against subsequent stressors. We systematically administered ketamine at different time points relative to a fear experience, in orde…

Neuroimaging of Fear-Associated Learning.

Fear conditioning has been commonly used as a model of emotional learning in animals and, with the introduction of functional neuroimaging techniques, has proven useful in establishing the neurocircuitry of emotional learning in humans. Studies of fear acquisition suggest that regions such as amygdala, insula, anterior cingulate cortex, and hippocampus play an important role in acquisition of fear, whereas studies of fear extinction suggest that the amygdala is also crucial for safety learning. Extinction retention testing points to the ventromedial prefrontal cortex as an essential region in …

Pathological fear of cot death.

Cot death (sudden infant death syndrome) is one of the most common causes of death in the first year of life. Four cases with a pathological fear of cot death are presented. All the patients were depressed and in 2 cases the fear of cot death had an obsessional quality. In all cases there were complications during pregnancy (miscarriage, threatened abortion, recurrent vomiting in last trimester). In 1 case, the patient knew 3 mothers who had suffered cot deaths; in another, the infant was gravely ill in the neonatal period. Pathological fear of cot death can be recognised by the presence of tw…

Personality traits associated with fear.

The extent to which personality characteristics are associated with fear was investigated. The amount of fear and the attributional style of 91 college students (74 women, 17 men) were measured using the Fear Questionnaire, the Fear Survey Schedule, and the Attributional Style Questionnaire. Analysis indicated that fear was significantly correlated with the responsibility for bad outcomes and with the tendency to generalize all outcomes to many situations. Findings are consistent with the view that significantly different attributional profiles emerge as fearfulness increases.
