The significance of femoral fractures in children.

This study was undertaken to determine the clinical circumstances of femoral fractures in children. The study was a retrospective investigation of 117 consecutive patients with femoral fractures admitted to The Children's Mercy Hospital, Kansas City, Missouri, from December 1975 to September 1979. Of the 117 patients. 41 had associated injuries. No patient whose only injury consisted of a closed-single femur fracture had evidence of shock. Of 24 children under 2 years of age, 19 sustained injury in an abusive incident. Of 27 children 5 to 6 years of age, 18 sustained injury in an auto-pedestri

Treatment of periprosthetic femoral fractures after femoral revision using a long stem.

BACKGROUND: Periprosthetic femoral fractures are becoming increasingly common and are a major complication of total hip arthroplasty and hemiarthroplasty. The treatment of periprosthetic femoral fracture after femoral revision using a long stem is more complex and challenging. The purpose of this study was to identify the clinical and radiographical features of periprosthetic femoral fractures after revision using a long stem. METHODS: We report a retrospective review of the outcomes of treatment of 11 periprosthetic fractures after femoral revision using a long stem. Eleven female patients w

Management of subtrochanteric femoral fractures and metastases using long proximal femoral nail.

We report our initial experience with a new reconstruction nail, the long proximal femoral nail (L.PFN), in the treatment of subtrochanteric femoral fractures and metastases. We performed 52 L.PFN in 49 patients over a period of 18 months with an average follow-up period of 47.7 weeks. Group I consisted of 24 patients, who had L.PFN for traumatic subtrochanteric femoral fractures. Group II consisted of 25 patients, who had L.PFN for femoral metastases and pathological fractures. (Three bilateral.) In nine patients in group I, the fracture was extending to the intertrochnateric region with invo

Results of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Locking Compression Plate.

The treatments of subtrochanteric femoral fractures are a challenge. It accounts about 10.0% to 34.0% of all hip fractures with a high complication rate. This area consists of mostly cortical bone with high stress generation thus heal slowly. The fracture is too proximal to adequately control with implants for femoral shaft and too distal to control with implants for intertrochanteric fractures. The intrinsic insecurity of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus crumple. Extramedullary implants are associated with higher

Postoperative avascular necrosis of the femoral head in pediatric femoral neck fractures.

This study aimed to evaluate the relevant factors of postoperative avascular necrosis of the femoral head (AVN) in children with femoral neck fracture. This study retrospectively analyzed the clinical data of 28 children with femoral neck fractures treated at our center between July 2016 and January 2019. The average age was 9.3 (range, 4.4-14) years with 75% male participants. Fracture classification was based on the Delbet classification: there were four, seven, 15, and two cases of type I, II, III, and IV fractures, respectively. Displacement degree was based on the Garden classification. S

Contralateral reversed distal femoral locking plate for fixation of subtrochanteric femoral fractures.

PURPOSE: Subtrochanteric fractures of the femur are being managed successfully with various intramedullary and extramedulary implants with reasonable success. However, these implants require precise placement under image intensifier guidance, which exposes the surgeon to substantial amount of radiation. It also restricts the management of these fractures at peripheral centers where facility of image intensifiers is not available. Keeping this in mind we designed this study to identify if contralateral reversed distal femoral locking plate can be used successfully without the use of image inten

Distal femoral valgus deformity following plate fixation of pediatric femoral shaft fractures.

BACKGROUND: This study investigated the frequency and potential risk factors associated with the development of distal femoral valgus deformity following plate fixation of diaphyseal femoral fractures in children. METHODS: Records of eighty-five skeletally immature patients who underwent plate fixation of a diaphyseal femoral fracture at a tertiary-care pediatric center from January 2003 to December 2010 were reviewed. Demographic data and clinical information were analyzed. Radiographic measurement of the distance from the distal plate edge to the distal femoral physis and of the anatomic la

Mortality following atypical femoral fractures in men.

Atypical femoral fractures (AFFs) are an uncommon complication of bisphosphonate (BP) treatment for osteoporosis. The purpose of this report was to determine differences in mortality among men treated with BPs who experienced AFFs, hip fractures, or typical subtrochanteric/femoral shaft (ST/FS) fractures. We included men from the Veterans Affairs Corporate Data Warehouse who filled at least one BP prescription over a nearly 20-yr span (October 1, 1999, to December 31, 2022). Typical ST/FS and hip fractures after a first filled prescription for a BP were identified using ICD codes. Radiographic

[Osteosynthesis of subtrochanteric femoral fractures in adults].

Subtrochanteric femoral fractures show an increasing incidence but are different from pertrochanteric and intertrochanteric fractures mostly with respect to the injury mechanics and treatment. These fractures predominantly affect geriatric, fragile and comorbid patients suffering from osteoporosis and sarcopenia. The reduction and minimally invasive stabilization of the mostly dislocated fractures using intramedullary nail systems is the method of choice but can be technically demanding. The aim of treatment is to restore mobility as quickly as possible to avoid exacerbation of comorbidities a

[Interprosthetic femoral fractures : Osteosynthesis, megaendoprosthesis, limb salvage?].

Interprosthetic femoral fractures (IFF) involve fractures occurring between total hip and knee arthroplasty and represent an increasing challenge, with complication rates of up to 57% and a mortality rate of 31%. The treatment is complex and requires an individual, often multidisciplinary approach. Risk factors are related to the patient, implant and surgical technique. Classification systems, such as the one proposed by Pires enable a systematic categorization based on fracture location and morphological features of the fracture. The treatment options range from angular stable plate osteosynt

[Nonunions after intertrochanteric and subtrochanteric femoral fractures].

INTRODUCTION: The overall frequency of proximal femoral fractures means that we are repeatedly confronted with failed healing and implant failure, despite a relatively low nonunion rate especially in intertrochanteric fractures (< 5%). The aim of this paper is to present our approach to treating these nonunions of the proximal femur and discuss the treatment results. MATERIAL AND METHODS: Between 2009 and 2023, patients with nonunion of the proximal femur were retrospectively identified and analyzed. Age, gender, time to revision, the Weber-Cech classification of pseudarthrosis and radiograph

Flexible Intramedullary Nailing of Pediatric Femoral Fractures.

BACKGROUND: Flexible intramedullary nailing is performed for femoral fractures in pediatric patients who may be too large for spica cast immobilization but who have substantial growth remaining and who are not a candidate for rigid intramedullary nailing. Flexible nailing allows the surgeon to obtain correct alignment of the femur fracture so as to allow for healing without a lower extremity deformity. DESCRIPTION: The patient is positioned on a radiolucent table, flexible nails are chosen according to the diameter of the medullary canal, medial and lateral incisions are made along the distal

[Diaphyseal and metaphyseal femoral fractures in childhood].

Femoral fractures in childhood are rare and are usually the result of severe trauma. Conservative treatment (overhead extension, hip spica cast) can be used in children up to 3 years of age and beyond that elastic stable intramedullary nailing (ESIN) is the method of choice. The prognosis is very good if the surgical technique is adequate. In adolescents > 50 kg in body weight solid nails implanted via the greater trochanter are an alternative (adolescent lateral femoral nail, ALFN). External fixators and plate osteosynthesis are indicated in special situations. Femoral neck fractures are very

Reduction Techniques for Young Femoral Neck Fractures.

Young femoral neck fractures remain challenging fractures to treat. Reduction has repeatedly been shown to be the single most important determinant of patient outcome, and treating surgeons should do all they can to achieve anatomic reduction because this is an outcome variable within the surgeon's control. Whether an open or closed reduction is performed, we hope to provide the reader with reduction strategies when faced with this difficult fracture pattern. In addition, we hope to review indications and techniques for using the various treatment implants/modalities that are currently availab

Outcome of femoral fractures in poliomyelitis patients.

BACKGROUND AND PURPOSE: As patients who were afflicted with poliomyelitis during the outbreaks in the past are aging, lower extremity osteoporotic fractures are becoming more frequent. Fixation in deformed, porotic bone, coupled with muscle weakness and imbalance creates a unique challenge when treating these fractures as does their reduced rehabilitation potential. The aim of this study was to investigate the outcome of femoral fractures in surviving poliomyelitis patients. PATIENTS AND METHODS: Sixty-five patients with 74 femoral fractures were treated between 1990 and 2014. Clinical outcom

Femoral midshaft fractures: expandable versus locked nailing.

Femoral midshaft fracture is one of the most common clinical injuries and is often caused by high-energy traffic accidents. Intramedullary nailings, plates, and external fixators are all used as treatment alternatives for a variety of patients depending on fracture location, displacement, comminution, soft tissue condition, and local tradition. Locked intramedullary nailing is currently the preferred treatment method for most diaphyseal fractures and has good clinical results. The goal of this study was to compare expandable and locked intramedullary nailing for the treatment of AO type 32A an

Outcomes of war related femoral neck fractures.

INTRODUCTION: In the civilian trauma literature, femoral neck fractures in young adults are considered an orthopedic 'urgency'. To our knowledge, there have been no studies looking at the outcomes of these injuries in the combat setting. The purpose of this study is to determine the outcomes of war related femoral neck fractures; the rates avascular necrosis associated with these injuries as well as the effect time to fixation has on the development of avascular necrosis. METHODS: We performed a retrospective review of 21 patients who sustained combat related femoral neck fractures from Octob
