Dr Jacob Mackiewicz (1887-1966) and his sign.

Dr Jacob Mackiewicz, a Jewish-Polish neurologist, described the cruralis phenomenon related to injury of the femoral nerve or of the fourth lumbar root. In 1912, only one year after graduation maxima cum laude from Moscow University, the 25-year-old Dr Jacob Mackiewicz described the cruralis phenomenon, regarded in the German and Slavic medical literature as the Mackiewicz sign: 'the patient lies prone, the examiner lifts the thigh in one hand, with the other hand, bends the patient's knee slowly; this manoeuvre causes severe pain in the anterior part of the thigh and over the groin. This mano

Surgical anatomy of the lateral femoral cutaneous nerve for meralgia paraesthetica: A simple technical guide for surgeons and trainees alike.

Meralgia paraesthetica is an entrapment neuropathy of the lateral femoral cutaneous nerve, usually due to compression at the inguinal ligament as the nerve passes from the pelvis into the thigh. Surgical decompression of the lateral femoral cutaneous nerve is a simple and effective treatment option, but the surgical anatomy of the area is not always familiar to neurosurgeons and neurosurgical trainees alike. This paper is a simple review of the relevant surgical anatomy and the surgical steps of lateral femoral cutaneous nerve decompression, with the aim of providing the busy surgeon and train

Meralgia Paresthetica.

Meralgia paresthetica (MP) is a neurological disorder of the nervus cutaneous femoris lateralis (lateral femoral cutaneous nerve) (LFCN) characterized by a localized area of paresthesia and numbness on the anterolateral aspect of the thigh. Medical history and neurological examination are essential in making the diagnosis. However, red flags such as tumor and lumbar disc herniations must be ruled out. While the diagnosis of MP is essentially a clinical diagnosis, sensory nerve conduction velocity studies are a useful additional diagnostic tool. The first choice in management of MP is, besides t

Iatrogenic femoral nerve injury: a systematic review.

PURPOSE: Iatrogenic femoral nerve injury is a recognized complication of abdominal and pelvic surgery. It causes distress and disability and may lead to permanent motor and/or sensory sequelae. The aim of this systematic review was to explore the contemporary spectrum of this injury reported in the literature. METHODS: A systematic review of iatrogenic femoral nerve injuries reported between 2000 and October 2010 was undertaken using the electronic databases Medline, PubMed, Cochrane Library, and Google Scholar. The context, frequency, mechanism of injury, and outcome were recorded. Relevant

Nerve Entrapments in the Pelvis and Hip.

Clinical symptoms of pelvic entrapment neuropathies are widely variable and frequently nonspecific, thus rendering it difficult to localize and diagnose. Magnetic resonance imaging (MRI), and in particular MR neurography, has become increasingly important in the work-up of entrapment neuropathies involving the pelvic and hip nerves of the lumbosacral plexus. The major sensory and motor peripheral nerves of the pelvis and hip include the sciatic nerve, superior and inferior gluteal nerves, femoral nerve, lateral femoral cutaneous nerve, obturator nerve, and pudendal nerve. Familiarity with the

Surgical therapy of femoral artery pseudoaneurysm after angiography.

Seventy patients requiring surgical treatment for femoral artery pseudoaneurysm after angiographic procedures have been presented. The 10 year study period encompassed 38,822 catheter procedures (31,951 cardiology procedures and 6,871 radiology procedures) performed through the femoral artery. Presenting complaints included pain (51 percent) and neuropathy (19 percent). Additional morbidity has been discussed. The present study differs from previous ones in that many of the patients had femoral artery pseudoaneurysm formation in the common femoral artery without a history of smoking, diabetes,

Entrapment Neuropathies of the Lower Extremity.

Entrapment neuropathies in the lower limbs are a common neurologic problem and may present in any medical setting. Accurate identification and management of these nerve palsies can prevent pain, sensory loss, incoordination, and muscle weakness that may significantly affect a patient's functional mobility. In this article, the authors focus on the cause, signs and symptoms, diagnosis, and treatment of select entrapment neuropathies of the lower extremity, including palsies of the common peroneal, lateral femoral cutaneous, femoral, and posterior tibial nerves.

Pelvic nerve injury following gynecologic surgery: a prospective cohort study.

OBJECTIVE: The purpose of this study was to determine the incidence and time course of postoperative neuropathy resulting from gynecologic surgery. STUDY DESIGN: A single cohort of 616 female patients undergoing elective gynecologic surgery for benign or malignant conditions at a tertiary care academic medical center underwent a postoperative neurologic evaluation to identify postoperative neuropathy of the lower extremities. RESULTS: Fourteen peripheral nerve injuries were observed in 11 patients, making the overall incidence of postoperative neuropathy 1.8% (95% confidence interval, 1.0-3.

Meralgia paresthetica.

Meralgia paresthetica is a common but probably underrecognized syndrome caused by dysfunction of the lateral femoral cutaneous nerve. The diagnosis is based on the patient's description of sensory disturbance, often painful, on the anterolateral aspect of the thigh, with normal strength and reflexes. Sensory nerve conduction studies and somatosensory evoked potentials may be used to support the diagnosis, but both have technical limitations, with low specificity and sensitivity. Risk factors for meralgia paresthetica include obesity, tight clothing, and diabetes mellitus. Some cases are compli

Ultrasound of the Lateral Femoral Cutaneous Nerve: A Review of the Literature and Pictorial Essay.

We review the ultrasound (US) findings in patients who present with meralgia paresthetica (MP). The anatomy of the lateral femoral cutaneous nerve at the level where the nerve exits the pelvis and potential entrapment sites that can lead to MP are discussed. A wide range of pathological cases are presented to help in recognizing the US patterns of MP. Finally, our experience with US-guided treatment is discussed.

Meralgia paresthetica: diagnosis and treatment.

Meralgia paresthetica is a symptom complex that includes numbness, paresthesias, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve (LFCN). The condition can be differentiated from other neurologic disorders by the typical exacerbating factors and the characteristic distribution of symptoms. The disease process can be either spontaneous or iatrogenic. The spontaneous form is usually mechanical in origin. The LFCN is subject to compression throughout its entire course. Injuries most commonly occur as the

Entrapment neuropathies III: lower limb.

Clinicians frequently encounter compressive neuropathies of the lower extremity. The clinical history and physical examination, along with electrodiagnostic testing and imaging studies, lead to the correct diagnosis. The imaging characteristics of the compression neuropathies can include acute and chronic changes in the nerves and the muscles they innervate. We provide a detailed review of compression neuropathies of the lower extremity with an emphasis on magnetic resonance (MR) imaging characteristics. We discuss the clinical presentation, etiology, anatomical location, and MR imaging appear
