Management of pathological fracture neck of the femur following recent osteomyelitis in a child.

Fracture neck of the femur is rare in children and occurs following severe trauma. Several recommendations have been made for the treatment of displaced transcervical fracture type 11 (Delbet classification). However there are no recommendations when such a fracture occurs after recent acute osteomyelits of the neck of the femur. The management of a case is described with the outcome after 36 months.

[Bone density measurements of the proximal femur in relation to torsion of the femoral neck].

The DEXA-method allows for bone mineral density measurements of different regions of the skeletal system. It is known that results obtained from the spine with this two-dimensional method may be biased in cases of spondylosis, spondylarthritis, and compression fractures; similarly biased results can be expected at the proximal femur due to morphometric peculiarities. The influence of anteversion and rotation of the femoral neck has been investigated in the present study. The results indicate that errors due to rotation as occurring in incorrect positioning or in arthrogenic variabilities mainl

Evaluation of osteoporosis in patients with fracture neck of femur using conventional radiography.

Osteoporosis was assessed at the hip (Singh's index) and calcaneus (calcaneal index) using conventional radiography in 80 patients (31 men, 49 women) with fracture neck of femur. Forty four patients had intracapsular fracture neck of femur (ICFN), and 36 extracapsular fracture neck of femur (ECFN). Fractures occurred more with increasing age in both groups, with female preponderance in the older age group. Osteoporosis was more severe as age advanced. Singh's index decreased from 5.2 to 3 (ICFN), and from 4.1 to 2.65 (ECFN) with age. Calcaneal index decreased from 4.4 to 2.9 (ICFN) and from 4.

[Osteoid osteoma of the femur neck with synovitis simulating chronic monoarthritis of the hip].

The authors report a case of subperiosteal osteoid osteoma of the femoral neck in a 14 year-old girl presenting as chronic hip monoarthritis with narrowed coxofemoral joint space and non specific histological synovitis. They analyse the synovitis which is associated with some osteoid osteomas (hip, elbow, knee, ankle bones) emphasizing the "pseudo-arthritis of the hip" picture and insist that all cases of unexplained chronic monoarthritis of the hip should suggest subperiosteal osteoid osteoma of the neck.

Hand grip--a simple test for morbidity after fracture of the neck of femur.

Seventy-six female patients, having sustained a fracture of the neck of femur, were assessed preoperatively by measuring their muscle strength in the form of grip using a hand-held dynamometer and by measuring serum albumin. A grip strength of less than 15 kg was found to be a reliable predictor of subsequent postoperative complications in female patients over the age of 80. Serum albumin had no predictive value. There was no significant difference in complication rates between different surgeons.

Spontaneous fractures of the neck of the femur occurring in pregnancy in the absence of pregnancy osteoporosis.

Case report on a patient suffering from spontaneous fractures of the neck of the femur which occured during her first pregnancy at the age of 23 in the left leg and during another pregnancy seven years later in the right leg. Both fractures occured during the eighth month of pregnancy. Special attention is drawn to the fact that local or general osteoporosis which, according to literature, should be obligatory for the occurence of spontaneous necrosis of the femoral head and/or fractures of the neck of the femur, could be demonstrated neither during the first nor the second pregnancy.

Heparin prophylaxis of deep vein thrombosis in patients with a fractured neck of the femur.

A double-blind controlled clinical trial of the efficacy of the prophylactic subcutaneous administration of calcium heparin in preventing deep vein thrombosis has been conducted in 50 patients with a fractured neck of the femur. Heparin was given every eight hours in a dose of 100 international units per kilogram of body weight and was commenced as soon as possible after the patient's admission to hospital. Treatment was continued for two weeks. In the placebo group, 12 of 25 patients developed a deep venous thrombosis and in five of these patients the thrombosis propagated to the thigh. There

Neck of femur fractures treated with the femoral neck system: outcomes of one hundred and two patients and literature review.

INTRODUCTION: The recently developed femoral neck system (FNS) for treatment of femoral neck fractures (FNF), comprises theoretical biomechanical advantages compared to other implants. The aim of this study was to validate the safety and to report outcomes of patients treated with the FNS. METHOD: A retrospective multicentric analysis of patients treated by FNS with a minimum of three months of follow-up. Details analysed from three medical centres were operative duration, estimated blood loss, initial hospitalisation duration, fixation quality as well as complications and reoperation rate. P

Bilateral Neglected Neck of Femur Fracture in a Cerebral Palsy Child: Case Report.

CASE PRESENTATION: A 15-year-old spastic quadriplegic cerebral palsy girl, with a Gross Motor Function Classification scale III, presented with bilateral neglected femoral neck fractures, 2 months after a seizure episode, due to inability to walk. Bilateral subtrochanteric valgus osteotomies and fixation using dynamic hip screws were done. Functional lengthening of soft tissue was achieved by femoral shortening. Postoperatively, partial weight bearing was allowed after 3 weeks and progressed gradually to full weight-bearing. The prefracture activity level was achieved, with satisfactory radiog

Medial buttress plate use in neck of femur fracture fixations: A systematic review.

BACKGROUND: Femoral neck fractures, particularly Pauwels type II and III, pose significant challenges due to their vertical instability and susceptibility to complications such as non-union and avascular necrosis (AVN). Medial buttress plates (MBPs) have emerged as a promising adjunct in fixation, offering biomechanical advantages by neutralizing shearing forces and enhancing stability. However, the clinical efficacy of MBPs across different fixation techniques, plate configurations, and positioning remains unclear. PURPOSE: This study aimed to (1) analyse outcomes of femoral neck fracture fi

Locking Plate Fixation System for Intracapsular Fracture Neck of Femur in Young Patients.

BACKGROUND: Intra-capsular fractures of the femoral neck in young patients are almost always treated with surgical fixation to preserve the native hip anatomy and biomechanics. Multiple Cannulated hip screws and the sliding hip screw have been the hallmark fixation devices for these injuries. The use of locking cannulated hip screws to a side plate was developed to mitigate the biological and mechanical downfalls of these devices. To report the outcome following the use of a locking plate fixation system in the management of intracapsular fractures of the femoral neck in young patients. MATER

Bipolar versus monopolar hemiarthroplasty for displaced femur neck fractures: a meta-analysis study.

INTRODUCTION: Hemiarthroplasty is commonly performed to treat femoral neck fractures. Still, there is a lack of consensus concerning the best component for hemiarthroplasty: unipolar and bipolar implants. Last meta-analysis on this topic is outdated, and an update of current evidences is required. The purpose of this study is to conduct a meta-analysis comparing the unipolar versus bipolar implants for hemiarthroplasty, focusing on the clinical scores, perioperative data, further complications and mortality rate. MATERIALS AND METHODS: In September 2019, the main databases were accessed: all

Arthroplasty infection rates in fractured neck of femur: single vs dual antibiotic cement.

INTRODUCTION: Mortality rates following hip arthroplasty range between 10% and 40% after 12 months. A higher rate is attributed to postoperative complications, of which surgical site infection is one of the most significant. In an effort to reduce surgical site infection following arthroplasty, antibiotics can be added to the cement used. The primary aim of this study was to determine whether dual antibiotic impregnated cement can reduce the rate of deep surgical site infection in patients following cemented arthroplasty for fractured neck of femur compared with single antibiotic impregnated c

Secondary prevention following fractured neck of femur: a survey of orthopaedic surgeons practice.

Fractured neck of femur is one of the most common reasons for hospital admission in the elderly population. Despite this little attention is given to the area of secondary prevention. To assess the variation in clinical practice among orthopaedic surgeons with regard to the use of secondary preventive measures in relation to fractured neck of femur. A postal survey using a self-completed questionnaire among all orthopaedic surgeons throughout the Republic of Ireland. The study questionnaire addressed unit information prophylaxis for deep venous thrombosis and investigation and secondary preven

Vitamin D status among patients with fractured neck of femur in Hong Kong.

Vitamin D deficiency leads to secondary hyperparathyroidism initially and then to mild osteomalacia, both of which conditions may be aymptomatic and may predispose to bone fracture. To assess the importance of vitamin D deficiency in predisposing to fractured neck of femur, we studied the vitamin D status, dietary intake and socio-economic characteristics in 69 patients with fractured neck of femur (group A), 28 normal subjects with age above 60 (group B), and 101 normal volunteers (group C). Patients with fractured neck of femor had significantly lower levels of serum 25-hydroxy-cholecalcifer
